GERD, gastroesophageal reflux disease or heartburn could develop due to drinking water/juice or soft drink during, right before or after the meal! there are 2 very important physiology rules you need to know in order to understand why you can not drink with your meal! 1. pylorus-lower valve, opens when pH of 2 is reached in the stomach and duodenum has pH of 7! when you drink water you dilute HCL (hydrochloric acid) of the stomach! 2. both valves: pylorus and LES (Lower Esophageal Sphincter) will become "tired" after about 1 hr 30 min! Meaning: they will relax and let food and stomach content out. if you drink more then 2 oz of any liquid with your meal, then you are diluting HCL of the stomach , as a result food will stay in the stomach longer then 1hr 30 min, as a result LES will get open and food and HCL will go up into esophagus! This is your GERD!!! These are the eight common cause of GERD which i regulatory see in my practice. 1. Increased Intra-abdominal pressure. Usually form excess weight around the waist. 2. Stretched stomach due to extra large portions. 3. Decreased production of Hydrochloric acid. This can be worsened by commonly proscribed medicines for Acid reflux and GERD (Proton Pump Inibitors and H2 antagonist such as omeprazole, prilosec, nexium, protonix,) 4.Delution of HCl due to drinking too much fluid with, right before, or right after a meal. 5. Why don't drugs like aciphex, prilosec, nexium, or protonix work for me? I get this question alot in my medical practice. The answer is these drugs are perfect if you over produce Hydrochloric acid. However, Rarely do I come across a patient who truly over produces HCL. Most of the time their Acid reflux or Gastroesophageal reflux is caused by something else: Diet, weight, overeating, or LOW production of hydrochloric acid. Yes, Acid reflux can be caused by LOW PRODUCTION of HCl, you didn't read that wrong. How to stop Proton Pump Inhibitors (Protonix)

You cannot stop it abruptly, it may cause severe GERD. Discontinue slowly, substituting one dose of PPI with medication from the other group: H-2 blocker (Cimetidine) or Antacid (TUMS) There is no rule how fast you may go. Its may take you 2-4 weeks or even longer. At the same time consider incorporating other natural substances such as Aloe Vera juice or DGL. If you have GERD (Acid Reflux/ Gastroesophageal Reflux Disease) due to low production of HCL (Hydrochloric Acid) or low stomach acid, there are food that will increase production of HCL. They used in many cultures either to stimulate or substitute for HCL. We can use this old-fashioned knowledge today. Here are examples: kimchi, sauerkraut, hot and sour soup, bitter herbs (Swedish bitters) . All used as a condiment as a small portion 10 minutes before the meal. Beer. Could it cause Acid Reflux? Beer has two ingredients that can contribute to GERD acid reflux or Heartburn. The high concentration of simple carbohydrates (sugar) will create hyperinsulinemia. This will result in a drop in sugar levels in the blood (hypoglycemia). It is now know that hypoglycemia will stimulate parietal cells to produce more Hydrochloric acid (HCl). Alcohol from the beer will degrade the mucosal protective layer that lines your stomach. Constipation and GERD? Magnesium treatment. Constipation. Could Constipation Contribute to GERD? Hm... Yes: https://www.youtube.com/watch?v=ijTSuDeOpdw Constipation is a very common problem ,but how it could contribute to gastroesophageal reflux disease? Constipation means that food does not move from upper digestive tract down, meaning that person has no stool for more then a day. Food stays in the large and small intestine. When new food arrives, digestive tract still has partially digested food and "new" food has almost no place to go. Also, when food stays in the digestive tract for more then 48hs, it's become rotten and a lot of gas got form. Gas irritates the digestive tract creating an inflammation. Low grade inflammation in large and small intestine could easy spread into the stomach. Low grade inflammation in the stomach will cause decreased production of HCL (hydrochloric acid). The consequences of low HCL: food will stay in the stomach more then 1 hr 30 min and LES (low esophageal sphincter) will become "tired" , small opening will get form and food with HCL may go into the esophagus, creating GERD/ gastroesophageal reflux disease / heartburn. Physiologically our stomach can not produce a lot of HCL. If you keep drinking with you meal and dilute HCL stomach will try to compensate, but would not be able. the more water you drink, the more HCL stomach have to produce, eventually you will have so much liquid in the stomach. HCL and water may go to Esophagus and cause GERD. If you drink a lot of water with you meal, stomach is not going to absorb it. Physiologically small amount of water could be absorb if person is severely dehydrated. Water get absorbed in the small and large (mainly) intestine Do not drink more then 2-3 oz of any liquid with your meal! Why tomatoes cause inflammation? Are are tomatoes an acidic food? But an environment in the stomach is more acidic then tomatoes! Why coffee does not cause GERD in some people, but tomatoes do? This video is my attempt to address this question. Stomach is made with different kinds of cells. Parietal Cell, will make HCL (hydrochloric acid) that will digest protein. Foveolar cells or surface mucous cells are mucus-producing cells which cover the inside of the stomach too, protecting it from the corrosive nature of HCL. Its possible that some chemical compounds of tomato damage those mucus-producing cells and stomach becomes unprotected, in othere words: tomatoes cause inflammation. HCL will damage stomach lining creating an inflammation in the stomach and it could spread to the esophagus. This inflammation is perceived as GERD (acid reflux/heartburn). Silent GERD may cause Asthma, chronic cough, but no heartburn. Why? Gastroesophageal reflux disease (GERD or heartburn) may manifest typically with heartburn , however, atypical GERD has laryngitis, asthma, cough, noncardiac chest pain, postnasal drip and cough, throat clearing . The diagnosis of these atypical manifestations may be difficult because most patients do not have heartburn. GERD (acid reflux). Hiatal Hernia Treatment. Why bother? Hiatal hernia often is the cause of GERD ( acid reflux, heartburn). Surgical treatment of hiatal hernia is ineffective because underlining causes such as obesity, constipation, chronic cough are not addressed. All those causes create increased intra-abdominal pressure and push stomach through the diaphragm into the chest cavity. When I (or chiropractor) treat patients with GERD due to hiatal hernia, I always try to address causes at the first appointment: obesity, constipation, chronic cough, acid reflux and GERD. Taking PPIs (Prilosec, Prevacid, Aciphex, pantoprazole (Protonix)) for GERD due to hiatal hernia does not make a lot of sense because you will be taking them every day for the rest of your life or until you fixed your hiatal hernia. PPIs have "black box warning", meaning that you cannot take them for more then 4-8 weeks. GERD, H. pylori, how I treat it in my practice. Often people diagnosed with GERD and H. pylori at the same time. H. pylori is bacteria that lives in the mucosal layer of the stomach. It will create inflammation, probably contribute to inflammation in the esophagus. Its treated with PPI, amoxicillin 1 g, and clarithromycin 500 mg (Biaxin) twice daily or Bismuth subsalicylate 525 mg or subcitrate 300 mg, metronidazole 250 mg, and tetracycline 500 mg, four times daily; and PPI twice daily. sometimes it's difficult to eradicate the bacteria (H pylori) and when patient wishes not to take medications again and again, i treat him/her symptomatically. I treat both: H. pylori and GERD. For H.pylori i use anti-microbial agents. For GERD/heartburn I use herbs that will coat digestive tract with mucus. I will use DGL to decrease inflammation. GERD, Heartburn, or Acid reflux: How often can I take antacids? A common question I get about antacids is: "How often can I take antacids?" This is a important question to ask because antacids are made of salts, like Calcium salts (Ca), or Magnesium salts (Mg), and Aluminum salts (Al). Tums and PEPCID are primarily made of Calcium salts. After ingestion, the antacids will start an acid- base chemical reaction, lowering the acidity (or Ph) of your stomach acid. This will relieve any Acid reflux symptoms, but will have two side effects. First, the nutrient absorption will decrease. This is a result of weak stomach acid, which is needed for proper digestion. Second, any extra Ca will be absorbed into the blood stream causing a disbalance between Calcium and Magnesium. Disbalance of Calcium in the blood will have to be brought to equilibrium. The kidneys will filter out the extra Calcium from the blood. GERD/Acid Reflux does not get better or even can get worth on Zantac or Prilosec. Why? Zantac belongs to category of H2 blockers and blocks production of HCL in the stomach. its older drug, been on the market for about 20 years and we believe has fewer side effects. Prilosec is in the group of PPIs (proton pump inhibitors )together with Nexium, Prevacid. They block production on H+ by inhibiting H+ pump mechanism, part of HCL (Hydrochloric Acid) production. Large portion sizes can stretch the stomach straining the lower esophageal sphincter (LES). Constipation means that food does not move from upper digestive tract down, meaning: person has no stool for more then a day. Food stays in the large and small intestine. When new food arrives, digestive tract still has partially of digested food and "new" food has almost no place to go. Also, when food stays in the digestive tract for more then 48 hs, it's become rotten and a lot of gas get form. Gas irritates the digestive tract creating an inflammation. every part of our digestive tract has a purpose. the purpose of stomach is to brake chemical bonds of protein with HCL. Hard food (cheese, peaces of meat, olives, whole wheat bread...) can not be broken in the stomach with the help of water. do an experiment: put in a glass meat, peace of cheese and olive and pore on top apple cider vinegar (as an acid), see how long it will take to brake those items (my guess: 2-3-4 days) and if you add water there, the food may just become rotten and never will be digested! the place to brake this food is mouth. People that are more competitive, highly organized, ambitious, impatient, highly aware of time management and/or aggressive are labeled Type A. It means that sympatitetic system is dominant in those people. The effect of Sympathetic system on the digestive is: decrease production of digestive juices, HCL, pancreatic enzymes, bile and decreased motility from top to bottom. As a result food cannot be digested. Food will stay in the stomach for a long period of time and LES (Lower Esophageal Sphincter ) will relax and let food go up into the esophagus. People who have Type A personality and GERD need to relax and activate parasympathetic system! how to do it? its a very good question! Before i start treating those kind of patients i'll make sure that they do not have hyperthyroidism (TSH, T3, T4), pheochromocytoma (VMA) and elevated cortisol levels.
