Primary Liver Cancer - Hepatocellular Carcinoma

Mr. Joseph Mormot, 71 years of age, was diagnosed with primary liver cancer (hepatocellular carcinoma) and after a few sessions of chemotherapy he was told by his doctors that there was no help for him and that he only had a few months to live. He came to the Issels Treatment Center on June 9, 2010. He was in a very bad shape from pain, swollen legs, and jaundice. When he was admitted, his Billrubin was 20, and by the end of his treatment it had already dropped to 5. His bile duct is open, which allows for normal liver functions; he doesn't have any pain, his legs are not swollen and he can walk freely. In his own words: "When the doctors talk to you, you can see they talk from their heart...they treat you as a friend...and it was very supportive for me. I never experienced such great care before..."

Many people can relate to making some type of sacrifice for the one they love. For some, its a move far away from family and friends. For others, its changing jobs or schools to be closer to their beloved. For 46 year-old Don Schwabe from Boise, Idaho, it was giving of himself in a way he had never imagined. Three years ago at the age of 37, Dons wife Heather Schwabe was diagnosed with primary sclerosing cholangitis, a chronic liver disease caused by progressive inflammation and scarring of the bile ducts in the liver. The cause is unknown but often the diagnosis is linked to a prior diagnosis of inflammatory bowel disease. Primary sclerosing cholangitis (PSC) leads to liver damage and, eventually, liver failure. Liver transplant is the only known cure. The time from first diagnosis to liver transplant is 12 to 18 years, on average. Known for her eternal optimism, Heather tried not to think about the eventual and perhaps inevitable need for a liver transplant. That need came much sooner than anyone would have expected when Heather was diagnosed with bile duct cancer, a known complication of PSC, in July 2009. Her doctors in Seattle delivered the news, along with a recommendation and referral to Mayo Clinic in Rochester, Minnesota. Within two weeks, Heather and Don arrived at Mayo Clinic and began learning the implications and treatment options for Heathers disease. It was quickly determined that, in addition to undergoing chemotherapy treatment for her bile duct cancer, Heather would soon need a liver transplant. What neither of them would have imagined at that time was that Don would end up being Heathers donor, and their surgeries were scheduled on the date they met 14 years ago. Solid Pseudopapillary tumor of the pancreas is an epithelial neoplasm of low malignant potential mostly of young women with solid, cystic and papillary architecture and acinar, ductal and endocrine differentiation. The diagnosis is based on finding of pseudopapillae covered by several layers of epithelial cells on FNA obtained by EUS. Tha management is surgical. Ένας από τους πλέον σπάνιους "καλοήθης" όγκους του παγκρέατος είναι το ψευδοθηλωματώδες νεόπλασμα το οποίο όμως έχει κακόηθες δυναμικό και θα πρέπει να αφαιρείται χειρουργικά όταν ανακαλύπτεται. Το μόρφωμα αυτό παρατηρείται κυρίως σε νεαρές γυναίκες στην τρίτη δεκαετία της ζωής τους και τις περισσότερες φορές είναι ασυμπτωματικό και αποτελεί τυχαίο εύρημα στα πλαίσια διερεύνησης άλλων μη ειδικών συμπτωματων. Συνήθως συναντώνται ανατομικά στο σώμα η στην ουρά του παγκρέατος πράγμα που καθιστά την χειρουργική τους αφαίρεση δυνατή χωρίς σημαντική νοσηρότητα. Σε περίπτωση μεταλλαγής τους δίνουν μεταστάσεις κυρίως σε ήπαρ και περιτόναιο. Ακόμα όμως και οι μεταστατική νόσος είναι χαμηλής κακοήθειας με ποσοστά βιωσιμότητας στα 5 χρόνια άνω του 95%. Το βίντεο που ακολουθεί είναι από νεαρά ασθενή με επιγαστραλγία η οποία στην αξονική κοιλίας ευρέθηκε να έχει μια μάζα στο σώμα το παγκρέατος. Έγινε ενδοσκοπικός υπέρηχος με λήψη βιοψίας FNA η οποία έδειξε ψευδοθηλωματώδη νεόπλασμα παγκρέατος. Η ασθενής χειρουργήθηκε με επιτυχία και δεν έλαβε περαιτέρω θεραπεία.

The stage of a pancreatic cancer is the most important factor in choosing treatment options and predicting a patient's outcome. A staging system is a standardized way in which is described the extent that a cancer has spread. The main system used to describe the stages of cancers of the pancreas is the American Joint Committee on Cancer (AJCC) TNM system. The TNM system for staging contains 3 key pieces of information: T describes the size of the primary tumor(s), measured in centimeters (cm), and whether the cancer has spread within the pancreas or to nearby organs. N describes the spread to nearby (regional) lymph nodes. M indicates whether the cancer has metastasized (spread) to other organs of the body. (The most common sites of pancreatic cancer spread are the liver, lungs, and the peritoneum - the space around the digestive organs.) In the following video the cancer is 3 cm and has invade the vessels of the celiac axis (T4), has spread to the regional lymph nodes (N1), but there was no evidence of metastasis to the distant organs by CT scan of chest and abdomen (M0). The patient's cancer was deemed unresectable and the patient received palliative therapy with CPN (celiac plexus neurolysis) for the pain related to cancer invasion of the celiac ganglia, and later chemotherapy and radiation therapy as well. Η σταδιοποίηση του καρκίνου του παγκρέατος (έκταση της νόσου κατά τη διάγνωση), είναι ο πιο σημαντικός παράγοντας για την επιλογή θεραπείας και την πρόβλεψη της εξέλιξης της νόσου. Το κύριο σύστημα που χρησιμοποιείται για να περιγράψει τα στάδια του καρκίνου του παγκρέατος είναι το TNM σύστημα της Αμερικάνικης Γαστρεντερολογικής Εταιρείας. Το σύστημα TNM περιέχει 3 βασικά κομμάτια πληροφοριών: Τ περιγράφει το μέγεθος του πρωτοπαθούς όγκου σε εκατοστά, (cm), και κατά πόσον ο καρκίνος έχει εξαπλωθεί στο πάγκρεας ή στο κοντινά όργανα. Ν περιγράφει τη εξάπλωση του καρκίνου στους γειτονικούς λεμφαδένες. M δείχνει εάν ο καρκίνος έχει κάνει μετάσταση σε άλλα όργανα του σώματος. (Η πιο κοινή τοποθεσίες του παγκρέατος εξάπλωση του καρκίνου είναι το ήπαρ, τους πνεύμονες, και του περιτοναίου - χώρος γύρω από το πεπτικά όργανα. Στο βίντεο που ακολουθεί ο καρκίνος ήταν μεγαλύτερος από 3 εκατοστά και είχε διηθήσει τα αγγεία του Αλληρείου τρίποδα (Τ4), είχε διηθήσει τους λεμφαδένες της περιοχής γύρω από το πάγκρεας (Ν1,) αλλά δεν είχε δώσει μεταστάσεις σε απομακρυσμένα όργανα όπως φάνηκε από τις αξονικές θώρακος και κοιλίας. (Μ0) Ο καρκίνος κρίθηκε ανεγχείρητος και ο ασθενής έλαβε παρηγορητική θεραπεία με CPN (celiac plexus neurolysis) για τον πόνο που του δημιουργούσε η διήθηση του καρκίνου στα νευρικά γάγγλια της περιοχής (βίντεο 2), και αργότερα ακτινοβολία και χημειοθεραπεία.

Cystic neoplasm of the pancreas is a relatively uncommon condition covering a wide spectrum of pathology. Although a minority of pancreatic cystic tumors are neoplastic, proper diagnosis of these neoplasms is important owing to their varied clinical course and behavior. The clinicopathologic features of pancreatic cystic neoplasms, include mucinous cystic neoplasms, intraductal papillary mucinous neoplasms, solid pseudopapillary tumors, and serous microcystic adenomas. The increasing incidence as a result of routine imaging tests in asymptomatic patients presents a diagnostic and therapeutic problem to the clinician. The following video is from an 82 years old lady with incidental finding on a C/T scan a cystic lesion in the head of her pancreas and an other cystic lesion in the left lobe of her liver. Τα πρωτοπαθή κυστικά νεοπλάσματα του παγκρέατος είναι όγκοι της εξωκρινούς μοίρας του παγκρέατος, συνήθως καλοήθεις. Πρόκειται για κυστικούς σχηματισμούς, που πιθανώς σχηματίζονται από τα κύτταρα των εκκριτικών πόρων και των κεντρολοβιακών κυττάρων του παγκρέατος. Οι κυριότεροι ιστολογικοί τύποι των κυστικών νεοπλασμάτων του παγκρέατος είναι: Α) Ορώδες κυσταδένωμα ή μικροκυστικό Β) Βλεννώδες κυσταδένωμα ή μακροκυστικό κυσταδένωμα ή κυσταδενοκαρκίνωμα (MCN) και Γ) Eνδοπορικό θηλώδες βλεννώδες νεόπλασμα (IPMN) Το βίντεο που ακολουθεί είναι από μία ασθενή 82 ετών η οποία λόγω επιγαστραλγίας έκανε μία αξονική τομογραφία τομογραφία κοιλίας στην οποίαν ανευρέθη κυστικό μόρφωμα στην κεφαλή του παγκρέατος και ένα δεύτερο στον αριστερό λοβό του ήπατος.
