Coronary Artery Disease (CAD) Anatomy, Nursing, Heart Disease, Pathophysiology, Treatment Part 1

Coronary artery disease (also called coronary heart disease) NCLEX lecture video on the nursing, anatomy, pathophysiology, treatment, and signs and symptoms of coronary artery disease. CAD is the build up of fatty plaques due to atherosclerosis that causes the arteries of the heart to become narrow. This leads to restrictive blood supply to the heart muscle which leads to chest pain (stable or unstable angina) to myocardial infraction (MI). The main coronary arteries that supply the heart with blood include the left and right coronary artery. The left coronary artery branches off into the left circumflex artery and left anterior descending artery. The right coronary artery branches off into the right marginal artery and posterior descending artery. If ischemia has been present for a long time collateral circulation can develop as well. This is where tiny blood vessels re-route the blood supply to the heart to pass the blockage. Signs and symptoms of heart disease include: chest pain (especially with activity and is relieved with rest which is known as stable angina), shortness of breath, and feeling very tired. Coronary artery disease is diagnosed in various ways: lipid profile which measures the cholesterol levels, EKG, stress test, and heart catheterization. CAD is treated with medications (discussed in part 2 of this series), heart catheterization (most commonly with stent placement) or coronary artery bypass graft surgery. For example, it could result from pressure on nerves when your arm is bent under your head as you fall asleep. Or it could be from pressure on nerves when you cross your legs too long. In either case, the "pins and needles" effect -- which is usually painless -- is soon relieved by removing the pressure that caused it. In many cases, however, tingling in the hands, feet, or both can be severe, occurring at irregular times or continuously day and night. It also can accompany other symptoms. such as pain, itching, numbness, and muscle wasting. In such cases, tingling may be a sign of nerve damage. Such nerve damage is known by doctors as peripheral neuropathy because it affects nerves distant from the brain and spinal cord, often in the hands and feet. There are more than 100 different types of peripheral neuropathy. Over time, peripheral neuropathy can worsen, resulting in decreased mobility and even disability.  Diabetes is one of the most common causes of peripheral neuropathy, accounting for about a third of cases. In diabetic neuropathy, tingling and other symptoms often first develop in both feet and go up the legs, followed by tingling and other symptoms that affect both hands and go up the arms. About two-thirds of people with diabetes have mild to severe forms of nerve damage. In many cases, these symptoms are the first signs of diabetes. So here is the main point I want to make, tingling and numbness in the feet may be the first signs that you have a serious peripheral neuropathy and that you may have diabetes. Diabetes puts you at risk of heart attack, stroke, kidney failure, blindness and a long list of other serious complications. So that tingling you have could be your only warning sign of a serious complication developing elsewhere in your body. Don’t ignore this symptom and please get a check up.

1. What are the best foods to eat before and after exercising? Answer: Foods that contain carbohydrate are quick and easy to digest for energy. Eat foods like bread, crackers, pretzels, dry cereal, 100% fruit juice, or sports drink with calories. But if weight loss is a goal and you aren't at risk of hypoglycemia, just drink water ditto for after exercise. Unless your workout was particularly exhausting or lasted several hours, you do't need food. 2. Can I get rid of diabetes? Will it ever go away? Answer: Once you are diagnosed with type 1 or type 2 diabetes, you have it for the rest of your life. To stay healthy, keep your blood sugar, blood pressure, and cholesterol under good control. To do so, eat healthy, be physically active, and take your prescribed medications. Also get the tests and checks you need to detect complications early and treat them aggressively. 3. How often should I replace my glucose meter? Answer: The only reason to replace your meter is if you determine it is not working correctly with its strips. To check, use the control solution that comes with your meter. To use control solution, put a drop (similar to the amount of blood you need) on a strip and do a test. Your vial of strips gives you a glucose range in which the result should fall. If it's in the range, your meter and strips are working correctly. If not, contact the manufacturer, which will offer advice and possibly a free replacement meter. 4. Should I always carry glucose tablets? Answer: Your risk for hypoglycemia depends on the category of blood glucose-lowering medication(s) you take, not your type of diabetes. Some medications can cause hypoglycemia, including insulin; those in the sulfonylurea category, such as glyburide (Diabeta, Glynase, Micronase), glipizide (Glucotrol), and glimepiride (Amaryl); and those in the glinide category, such as repaglinide (Prandin) and nateglinide (Starlix). If you take one of these medications, always carry treatment. 5. Is it true that complications of diabetes can be delayed and even prevented? Answer: Yes! Research shows that the earlier your diabetes is diagnosed and the sooner you start to aggressively get your blood sugar, blood pressure, and cholesterol into target ranges, the healthier you can be over the years. To stay healthy and detect any complications early, make sure your health care provider orders all the tests and checks you need. Let your provider know if you have any signs or symptoms of a potential problem. 6. Will I need to be on insulin the rest of my life? Why can't I take pills instead? Answer: If you have type 2 diabetes and were put on insulin, it's likely you need it to bring your blood sugar down farther than pills could. In this case, you'll likely need to take insulin injections the rest of your life. If, however, you started taking insulin when you had an infection, needed surgery, or were hospitalized for a medical reason, your need for insulin may be temporary. When the stress abates, you may be able to taper or stop taking insulin and get back to your previous medication regimen.

Epidermodysplasia Verruciformis or the tree man illness is characterized by the formation of horn like structures on the skin and warts the take up an overwhelming amount of surface area on the body. The skin on the hands and feet becomes tough and thick turning a brown or yellow color resembling branches of a tree. Several types of HPV contribute towards this condition, but HPV 5 and HPV 8 are the ones that are most often associated with the mutation. Unfortunately, with tree man illness even when the warts are surgically removed that isn't a guarantee that they won't return. In the case of one man, Dede Koswara, who had 95 percent of his warts removed in 2008 only for them to resurface soon after. Even if one can't be completely cured, after several treatments the warts have been shown to at least grow back smaller each time. 5. Sirenomelia, the proper name for the better known Mermaid Syndrome, is the mutation that causes infants to be born with what looks like a mermaid tail. In the womb the legs start to rotate and are fused together due to only developing a single artery that cannot produce the blood flow needed to form the lower body completely. In the 19th century the afflicted fetuses were preserved in jars and displayed for people in 'freak shows' as mermaid children. It's an extremely rare condition and only affects 1 in 100,000 babies, but it is much more likely to occur in identical twins. Those born with Sirenomelia usually die within a few a days of being born and those who do survive don't live a very long life. 4. Progeria is the genetic mutation that causes accelerated aging in children. It's a rare condition caused by a mutation in the gene named LMNA. The children usually all display very similar characteristics, they lose body fat and hair. When they are born the babies look healthy and normal until around 18-24 months when the mutation starts to accelerate and becomes visible through their appearance. Sadly children with Progeria tend to die at an average age of 14 due to heart disease. 3. Not all mutations are visible on the outside nor are they all a hindrance to everyday life. The children of the Moken people, located off the coasts of Thailand and Myanmar, have developed the ability to see clearly underwater in depths up to 75 feet. The mutation of their eyes is actually the ability for the eye muscles to change the shape of the lens to increase light refraction, leading to underwater vision. This evolved ability allows them to scope out tiny brown clams deep underwater even while they look indistinguishable amongst the tiny brown rocks near them. Something that's hard for others to do even while wearing goggles. These sea-gypsies dominate the water! 2. Proteus syndrome or elephant man syndrome causes the overgrowth of body parts in a asymmetrical fashion. It varies from person to person and can affect any part of the body. At the age of 6-18 months the growths can start to occur and get more severe with age. Proteus syndrome can even cause neurological abnormalities that lead to seizures and vision loss, but also affect the person on an intellectual level. "The Elephant Man" a movie the brought to the light the real life man, Joseph Carey Merrick, who had Proteus syndrome and was a "freak" in the Victorian times. He was put on display for people to gawk at and weird theories as to why he was born the was he was started to bloom. One farfetched claim was that his mother was frightened by an elephant and that must be the reason. 1. Cyclopia is the birth defect whose name was inspired by the Cyclopes giants in Greek mythology. The condition causes a single eye that is located where the base of the nose usually takes its place. The nose can be absent or can be located above the eye in the form of a proboscis. Only one of the two eye sockets that we are normally born with forms which gives them their unique appearance. All of the causes leading to children being born with Cyclopia come from a unusually high presence of toxins in the body of the mother. The toxins in the body that cause the mutation could be from alcoholism, diabetes, and even from ingesting a toxic weed known as Corn Lily during pregnancy. Cyclopia is a fatal condition due to the underdeveloped facial features and other deformities to the heart and vital organs.

In other words, should people with more muscle mass and less body fat be healthier despite the same body mass index? The authors of this study wanted to shed some light on that question. Body mass index, also known as BMI, is an objective way to describe someone’s height and weight. Overweight and obese BMI are associated with a ton of diseases such as diabetes, heart disease and strokes. Research has shown that in individuals with a BMI over 25 (classified as overweight) and a BMI over 30 (classified as obese) the risks for the following conditions also increases: Death from any cause (all cause mortality) Coronary heart disease (heart attack) Type 2 diabetes Cancers (endometrial, breast, and colon) Hypertension (high blood pressure) Dyslipidemia (high cholesterol, triglycerides) Stroke (cerebrovascular accident) Liver and Gallbladder disease Sleep apnea and respiratory problems Osteoarthritis (a degeneration of cartilage and its underlying bone within a joint) Gynecological problems (abnormal menses, infertility) Metabolic syndrome

In patients with diabetes, blood pressure is comorbidity or a coexisting factor. We also know that a high number of diabetes patients, something around 50 to 70% of Type 2 diabetes patients will have blood pressure or hypertension. But it is difficult to say the cause and the effect. So what is causing the blood pressure it is not known if the diabetes is causing the b blood pressure or vice versa. So we keep doing that the coexisting blood pressure in a diabetes individual, it is very vital for the wellbeing of the patient and also to prevent the complications of the patients. But also means that we have to be very careful in managing the range of blood pressure. It is not yet proven that diabetes causes blood pressure. In medial terms, high blood pressure is called as hypertension. Primary hypertension is more common among the individuals. Diabetes does nit feature in secondary hypertension. So it is a coexisting disease. The cause and effect is not yet proven and still needs more research and the drugs that are coming to the market are more fine-tuned and customized to these patients. So all in all I would say that it is not justified to say that diabetes is causing the blood pressure, but they do coexist.
