This video is about a treatment for men who already have prostate cancer. It is about adding an occasional testosterone boost to regular drug therapy to treat prostate cancer. Dr Sam Denmeade reports on encouraging trials of this approach. The small numbers in these trials mean your doctor will not be recommending this to you as a part standard treatment, as yet, except possibly in a clinical trial. When this video was made in February 2017, clinical trial openings were limited to the USA. But, if you wish to understand a treatment which could become a game-changer in future, this is for you. How Doctors Determine Prostate Cancer Risk and Select Treatment Doctors examine a wide range of factors when determining prostate cancer risk. These include consulting National Comprehensive Cancer Network guidelines, the cancer's determined Gleason score, X-ray imaging and the initial staging of the disease. From there, doctors determine the risk of the cancer's spread to other organs. If a prostate cancer is determined to be low-risk and unlikely to spread, treatments like active surveillance can be deployed. If, however, a cancer is deemed higher risk and more likely to spread doctors will take a more aggressive treatment approach. But again, doctors will consider many options and recommend for you and what NCCN and other physicians have recommended. It is essential for doctors to use all of the available data when considering what your options are and the best options may be for your risk factor. Dr. Bevan-Thomas is the Medical Director for Prostate Cancer Live. A graduate of Duke University, Rich received both a master's degree in anatomy and his medical degree from St. Louis University School of Medicine. Matthew Cooperberg, MD, genitourinary cancer specialist, University of California San Francisco, discusses the impact of immunotherapy on the treatment of prostate cancer. Cooperberg believes immunotherapy treatment should be used earlier in the course of the disease. In the recent phase III sipuleucel-T (Provenge) trial in which men with the lowest quartile of PSA had a survival advantage of more than one year over those with the placebo arm. Study findings showed that men who were treated later had less of a benefit. Today, sipuleucel-T is used even earlier for the treatment of men with prostate cancer, and Cooperberg feels that this change is appropriate.

Robert Dreicer, MD, Division of Hematology/Oncology, Department of Medicine, University of Virginia Health System, discusses resistance to antiandrogen therapy in prostate cancer. Therapies such as abiraterone acetate (Zytiga) and enzalutamide (Xtandi) can be used in sequence and provide significant activity, says Dreicer. However, there are patients who are intrinsically unresponsive to these drugs upfront. When a drug is administered, there is a decline in the response rate to the sequential use of that drug, according to Dreicer. The AR-V7 blood test may help both patients and doctors to make decisions on treatment choices when patients become resistant to certain therapies. Dreicer says that the field is beginning to understand some of the complexities of resistance in prostate cancer, but there is still much work to be done.

Epidemiology lacks functions to measure quality of life and is unaware of the toxicity of androgen deprivation therapy. Hazard rate is a good measure for quality of life. Hazard rate is the probability of a patient to die next year. Declining quality of life raises hazard rate particularly in patients treated with ADT . In the presentation “Young Breast Cancer Patients have worse Outcomes” I show that following treatment hazard rate rises. Which I attribute to hormone deprivation therapy. I shall now show that androgen deprivation therapy ADT accelerates aging. Cancer is an evolving dynamic system. In a previous presentations I described the colon vector field. Androgen deprivation changes profoundly cancer dynamics, using the data from SEER database. From the time of diagnosis and treatment prostate cancer hazard rate rises. Treatment raises hazard rate ! Prostate cancer vitality approaches its endpoint (death) faster than colon cancer. It ages faster and will die sooner than colon cancer. Androgen deprivation accelerates aging. Elderly suffer from the greatest vitality loss. Law Regardless what causes cancer, from the third year and onward cancer hazard rate declines and host resistance against cancer rises. This is also the destiny of prostate cancer. Yet medicine annihilated its host resistance with ADT. Cancer is a viral disease. Each cancer is initiated by a unique virus which is tropic to the tissue in which cancer originates. Cancer is a response to viral infection.
