The Never ending saga that is my search for the right antidepressant may be finally at a close. I recently went to a new psychiatrist and told him I was having serious thoughts of death and blackness beyond normal levels. I was annoyed and fed up because I was now on my fourth antidepressant and I was still feeling like life wasn't worth living. Finally I was given a drug that has changed my outlook on life enormously. It is called Sifrol. Escitalopram (Lexapro) has FDA approval for the treatment of major depressive disorder in adolescents and adults, and generalized anxiety disorder in adults. In European countries and Australia, it is approved for depression (MDD) and anxiety disorders, these include: general anxiety disorder (GAD), social anxiety disorder (SAD), obsessive-compulsive disorder (OCD), and panic disorder with or without agoraphobia. Escitalopram was approved by regulatory authorities for the treatment of major depressive disorder on the basis of four placebo controlled, double-blind trials, three of which demonstrated a statistical superiority over placebo. Controversy existed regarding the effectiveness of escitalopram compared to its predecessor citalopram. The importance of this issue followed from the greater cost of escitalopram relative to the generic mixture of isomers citalopram prior to the expiration of the escitalopram patent in 2012, which led to charges of evergreening. Accordingly, this issue has been examined in at least 10 different systematic reviews and meta analyses. The most recent of these have concluded (with caveats in some cases) that escitalopram is modestly superior to citalopram in efficacy and tolerability. In contrast to these findings, a 2011 review concluded that all second-generation antidepressants are equally effective, although they may differ in onset and side effects. Treatment guidelines issued by the National Institute of Health and Clinical Excellence and by the American Psychiatric Association generally reflect this viewpoint. In 2018, a systematic review and network meta-analysis comparing the efficacy and acceptability of 21 antidepressant drugs showed escitalopram to be one of the most effective antidepressants in head-to-head studies. Pramipexole (Mirapex, Mirapexin, Sifrol) is a dopamine agonist of the non-ergoline class indicated for treating Parkinson's disease and restless legs syndrome. CBS Iowa Updated: Fri, Apr 03 2015, 10:29 PM EDT Her career is in insurance, but Patti Ireland's passion is photography. For years, Ireland would travel across the country photographing wild birds in their habitat. But a photography trip two years ago, would be her last. A trip to the doctor changed everything. After an asthma problem, her doctor gave her prednisone, a drug that's also known to weaken the immune system. As an extra precaution, she says her doctor then gave her a strong antibiotic called levofloxin, brand name Levaquin. It was a pre-emptive strike on any potential illness, not because she had symptoms. She says, "48 hours after I was given the first pill, I woke up with tremors, which you can hear in my voice and see in my hands. Ringing in my ears that sounds like living in a tent full of cicadas." Ireland wondered if the Levaquin pill was causing it. "I called the doctor and she said, 'Oh, no, it must be the prednisone. Keep taking it.'" She stopped taking the prednisone but took one more Levaquin pill. That was it. "8 days after stopping it," she says "I was in a hotel room in Florida on a photographic trip and I woke up in the middle of the night feeling like there wre hot pokers in my body. I couldn't walk. I had trouble breathing. I was panicking." Now two years later, the tremors are constant. Basic tasks like getting a drink of water are a challenge. There are no more photography hikes into the woods and her physical and cognitive issues forced her to retire early from her career. Dr. Julie Buchkina, MD, did not prescribe the Levaquin, but now treats Ireland's symptoms. She says, "It appears the symptoms she has experienced since then, are permanent." She goes on to say, "Ever since she took the Levaquin, she has struggled with new health conditions that she previously didn't have." There is a black box warning right now on Levaquin, the strictest warning a drug can have. But it warns of tendon issues like tendon ruptures and tendonitis, not the nerve and cognitive damage Patti is living with. The year after Patti took the two pills, in 2013, the FDA recommended warnings on the label and they are now posted to include the potential for nerve damage called peripheral neuropathy. The same report also points out the long-term damage to mitochondria cells, problems that could lead to diseases like Alzheimer's, Parkinsons and ALS. Dr. Buchina says, "Currently there is no official warning on Levaquin to warn abou the risk of increased damage to mitochondria, but that is something the FDA is looking into." Ireland doesn't blame the doctor who prescribed it. She says it was given to her in good faith. She also cannot sue the drug maker, as it was a generic version of Levaquin. She now just wants changes including regulation of this powerful antibiotic, adding "These drugs need to be regulated. They should only be used as they were designed, which is in life threatening situations where an infection isn't responding to any class of antibiotic." Gabapentin/Neurontin, is a medication used primarily for seizure disorders and pain associated with the nerves. It can be used for nerve pain related to the herpes virus or shingles. It is also commonly used by diabetics who have suffered nerve pain. I find that this medication continues to work wonder for me, even after daily use for 3+ years. It helps prevent my migraines (at least it seems to) and once I have one, it helps the dull the pain. It also has help with occasional back pain problems. It has been a life saver for my insomnia. I am able to cope better with my day to day task because this medication helps me cope with my Clinical Depression, of which has haunted me since the passing of my father. It is important to obtain baseline vital signs: this medication will lower the blood pressure. In addition, make sure the patient’s SBP is within parameters before administering. Most hospital protocols require SBP (systolic blood pressure) to be greater than 90, if it is not contact the MD for further orders. When giving sublingual or oral spray nitroglycerin you will monitor the patient’s blood pressure and chest pain rating throughout. After the first dose of nitro, you can give a 2nd dose of the medication in 5 minutes if the patient is still having chest pain and the patient’s blood pressure is within parameters. If the patient is still having chest pain after a 2nd dose, a 3rd dose can be given within 5 minutes if the patient's blood pressure is within parameters. No more than 3 doses are given. Symptoms of schizophrenia can be broken down into 2 categories, Positive and Negative Symptoms. Positive symptoms include behaviors or sensations that are not normally present. Examples include hallucinations, delusions, and catatonia. These symptoms are thought to be related to an excess of dopamine. I remember this by remembering that “doPamine has a P in it”. So P for Positive and P for Dopamine. Negative symptoms are the absence of normal behavior. Examples include a lack of initiative, diminished speech, disheveled appearance & flat affect. These symptoms are thought to be related to an excess of serotonin. As we will see antipsychotics affect dopamine and serotonin to varying levels. The indications for this class of drugs includes psychosis (mainly schizophrenia), Mania (mainly bipolar disorder), aggression and Tourette's disease. Typical Antipsychotics primarily block dopamine receptors in a non-specific manor. Therefore, these drugs work best for positive symptoms, and have little effect on negative symptoms. The non-specific mechanism of the drug also means there are lots of side effects. Some of these medications come in a slow release injectable form so they can be used in non-compliant and aggressive patients. There are a lot of high yield side effects so we will break them down one by one Neuroleptic Malignant Syndrome (or NMS) is a rare but potentially fatal adverse reaction of typical antipsychotics. It involves fever, altered mental status, rigidity and autonomic instability (such as tachycardia, hypertension, diaphoresis etc.). You may also see elevated myoglobin in blood or urine and elevated Creatine Kinase (CK). One of the ways I think about it is that it looks kinda sorta like Serotinin Syndrome that you can see with antidepressatns. If you see this you have to emergently stop the medication, provide supportive care and consider adding Dantrolene Extrapyramidal Symptoms (or EPS) are due to blockage of Nigrostriatal dopamine. It can present with a number of different symptoms. Akasthisia is a general sensation of restlessness Acute Dystonia is involuntary continuous muscle contractions often of the neck. Another common presentation of acute dystonia is Oculogyris Crisis when your eyes get locked looking upward and you have to lean over to see Dyskinesia (AKA Pseudoparkinsonism) presents like Parkinson’s Disease with symptoms like a pill rolling tremor, cogwheel rigidity & bradykinesia (or slow movement) Tardine Dyskinesia (or TD) is uncontrollable facial tics, grimacing & tongue movements As scary as these symptoms may look, they are generally not medical emergencies. In most cases you will continue to use the drug with perhaps a reduction in the dose or the addition of an anticholinergic mediation like Benzatropine or Diphenhydramine. Tardive Dyskinesia is the exception and requires cessation of the medication as it can be permanent. Usually you would switch a patient with TD to a 2nd gen antipsychotic. Hyperprolactinemia is a side effect due to Blockage of Tuberoinfundibular dopamine. It presents just like any other disease that increases prolactin.
