Acne Treatment with Birth Control Pills and Antiandrogens
Antiandrogen treatment of acne Antiandrogen treatment may be considered for adult women. Those who fail systemic antibiotics, relapse after a course of isotretinoin or for women who have symptoms of hyperandrogenism are likely to benefit. Symptoms of hyperandrogenism include irregular menses, infrequent or light menses, mild hirsutism, oily skin and androgenetic alopecia. Women with premenstrual flares, adult onset acne, acne worsening as an adult and acne on the mandibular line and neck may also benefit. Women with ovarian cysts and hirsutism may also benefit. Patient selection Failure of oral antibiotic Recurrences after isotretinoin Irregular menses Light menses Mild hirsutism Increased oiliness Androgenetic alopecia Treatment There are two approaches. Combination oral contraceptives ( COC) reduce circulating androgens. Spironolactone is an peripheral androgen receptor blockers. The two drugs are frequently combined to achieve optimal results. Oral contraceptives All COCs can be effective for mild to moderate acne. They are often ineffective for more severe cases. COC suppress pituitary gonadotropins. This results in ovarian hormone suppression. They also raise circulating sex hormone binding globulin which decreases circulating free testosterone. All of today’s low-dose COC s are estrogen dominant and are beneficial in women with acne regardless of which formula is used. Use COC containing the lowest dose of each hormone that limits the early-cycle side effects of nausea/vomiting, breast tenderness, weight gain and headache. Several months of treatment are required. Concomitant intake of a COC and an antibiotic prescribed for acne does not impair the efficacy of the COC. Products Ortho-TriCyclin Estro-Step Alesse Yasmin Ortho-TriCyclin and Estro-Step are approved for treating acne. Estrostep is formulated so that the ethinyl estradiol dose rises as pills are used during a cycle. It may be better tolerated than higher dose estrogen formulations. Alesse is the lowest dose estrogen formulation. It may be better tolerated in women with bloating or nausea on higher dose formulations. Yasmin is an oral contraceptive (OC) with a new progestin called drospirenone. Drospirenone has an antimineralocorticoid and antiandrogenic effect similar to that of spironolactone and can cause potassium retention. Whether this antimineralocorticoid effect leads to less weight gain than with other OCs is still unclear. The effect of Yasmin on acne appears to be similar to that of other OCs. Information to date does not support any increase in the risk of venous thromboembolism with use of the drospirenone-containing OC as compared with any other combination OC. Spironolactone Spironolactone is the androgen receptor blocker used in the United States. Active metabolites have a half-life of 14 to 16 hours. Improvement in acne is variable. Spironolactone Start 50 mg qAM ?2 to 4 weeks 50 mg bid Maximum dose 200 mg/d Monitor blood pressure Monitoring K not necessary Spironolactone 50 mg once each day is prescribed for 2 to 4 weeks and increased to 100 mg/d as tolerated. Doses as high as 200 mg/d are sometimes necessary. The medication may be given as a single morning dose. Higher doses are divided and given twice each day. Lower the dose after several months of treatment if the patient is clear. Spironolactone is safe to use for months or years. The simultaneous use of oral contraceptives may be helpful. Spironolactone is often beneficial when used as monotherapy without oral antibiotics. Check blood pressure at each visit. Monitoring potassium levels in healthy young women is not necessary. Side effects Menstrual irregularities Breast tenderness Fatigue Headache Side effects increase with higher doses but the drug is generally well tolerated. Side effects are menstrual irregularities, breast tenderness, fatigue, and headache.

THE PILL | My 5 1/2 Year Birth Control Experience (Diane-35)
• Why was I on it? - extreme pain, cystic acne, unwanted body hair • How long was I on it? - 5 1/2 years • How did it perform for what I took it? - no more period pain, very little acne, amount of unwanted hair the same • Unexpected positives? - I quit smoking, my breasts grew, my head hair got thick, my body shape went hourglass • Unexpected negatives? - extreme hunger/ cravings, extremely frequent UTI's, weight gain, looking puffy (breast had weird shape) severe mood swings, queasiness in the morning (felt pregnant), difficulty loosing weight, no more bleeding, no libido, no lubrication (uncomfortable painful when stimulated) / no sex drive that resulted in a change in personality: not taking care of myself in how I dress how I look - sloppy unkempt • How did I quit? - water fasting • Had I planned on quitting? - was hesitant because of period pain returning but knew I had to stop taking them and everybody kept telling me how bad they are for my liver (pickling my liver nonalcoholic) • Period after quitting? - very light at 1st, very slight pain, got a bit more intense over the following months, also vegan changes • How long did it take for the pill to leave my system completely? - 4 to 5 months • Unexpected positives after quitting? - Immediate ease in wright loss, breasts looking normal, felt like myself again (no more foggy brain), libido back, taking care of myself, no mood swings • Unexpected negatives? - my hair fell out (bald patches, clumps of hair in the bath, hair brush), acne came back with a vengeance ( rarely cystic but more of it) • Things that I was fearing and didn't happen? - that the pain would come back, that my boobs would completely deflate, that I would grow a mustache and manly hair all over, and the fact that I might have to go back on them and have to take them the rest of my life • Final thoughts? - I don't regret taking the pills, it did help me quit smoking, I do have a more feminine figure, maybe it helped with the pain and cystic acne long term, but all in all I wouldn't want to be on them again. Medications And Hormones Needed To Transition From Male To Female
What Medications Are Involved for MTFs? Various kinds of medication can be used to change the levels of sex hormones in your body. Some work on the part of your brain that stimulates sex hormone production, some work on your testicles (which produce testosterone), and some work directly on the cells in your body that respond to sex hormones. Some of these medications are also hormones, and some are another type of chemical. Typically MTF hormone therapy involves estrogen, medication to block testosterone, or a combination of the two. Sometimes a progestagen is added to the mix. Estradiol https://tblockers.com/estrogen/ Aldactone https://tblockers.com/spironolactone/ 1. Estrogen Estrogen is the main hormone responsible for promoting “female” physical traits. It works directly on tissues in your body (e.g., makes breasts develop) and also indirectly suppresses your testosterone. Estrogen can be taken in different ways: • pill (oral application) • skin patch or gel (transdermal application) • injection (intramuscular application) For reasons that aren’t understood, estrogen seems to cause blood clots less when it is taken through the skin rather than by pills or injections. For this reason, transdermal estrogen is usually recommended to anyone who is over age 40, a smoker, or otherwise at risk for blood clots. Transdermal estrogen also tends not to elevate triglycerides (a type of fat in the blood) as much as estrogen taken by pill/injection, so it is recommended if you are at risk of heart disease or stroke. There are different chemical formulations of estrogen. Usually 17-betaestradiol (patch = Estradot®, Estraderm®, or Oesclim®; pill form = Estrace®) is used because it has the least health risks. Conjugated estrogens (e.g., Premarin®) and ethinyl estradiol are not recommended because studies of non-trans women have shown them to increase the risk of some types of health problems. 3 2. Anti-androgens (also known as androgen blockers or androgen antagonists) Anti-androgen drugs work by blocking the effect of testosterone. This reduces “male” physical traits and has a mildly “feminizing” effect. For example, they will help slow “male”-pattern baldness, reduce growth of facial hair, and stop spontaneous/morning erections. There are different types of anti-androgens. The ones most typically prescribed to MTFs are spironolactone (Aldactone®) and finasteride (Proscar®). Check for the hormone levels like follicle stimulating hormone, prolactin, thyroid, antimullerian hormone, complete blood count and fasting blood sugar and correction of hormonal imbalance if present. Hormonal treatment should be considered in female patients whose acne is refractory to conventional lines of treatment. Hyperandrogensim is a definitive indication for hormonal therapy, but in women who have clinical signs of hyperandrogenism but normal androgen levels in their blood reports still be considered for hormonal therapy. We basically have 4 major categories of drug in the treatment of hormonal acne. They are anti androgens, peripheral androgen metabolism blockers, insulin sensitizing against and combined oral contraceptive pills. Anti androgens will block the action of sebaceous gland and hence they will decrease the release of sebum and control the acne. So spironolactone is what we have is a very potent anti androgen. It can be given in a dose of 50 – 100 mg per day dosage and it is very effective compared to an OCP in an adult onset acne. He next drug which falls into anti androgen category is cyproteronacetate which is used in the dose of 2 – 100 mg per day and it is usually combined with an OCP formulation and which will exert anti androgenic effects. We also have a novel progestin which is termed drosperinone which is only US –FDA approved progestin so far to be combined with a low dose estrogen and it has again anti androgenic properties which will treat hormonal acne as well as hirsutism. Next category of drugs are combined oral contraceptives which is the combination of low dose estrogen with various progesterones. Here we have to very carefully chose the OCP for the hormonal acne because most of the progesterones, that is the synthetic progesterones are having inherent androgenic properties.SO we here have to carefully chose a progesterone which has very low inherent androgenic property like a drosperinone which is US-FDA approved would be very ideal here.. Next category of drugs are anti-inflammatory as well as anti androgenic antibiotics like limicycline, which is a second generation tetracycline and we sometimes do use roxithromycin which is a macrolide. Again it also has anti androgenic properties, though it is a antibiotic. Recently there is a report that Vitamin D deficiency has a role in menstrual irregularities. Hyperandrogenism, hirsuitism, as well as in hormonal acne. So a supplementation of Vitamin D would be ideal. Apart from the hormonal care, the good routine also helps in hormonal acne. So cut out on the processed food as much as possible. Avoid sugars, white flours and fats. Stick to the whole diet, also usage of broccoli and cabbage would help you and do not pick or pop or squeeze the acne. Be very gentle while cleansing your face. Use a very mild cleanser.
