Cesarean delivery — also known as a C-section — is a surgical procedure used to deliver a baby through incisions in the mother's abdomen and uterus. A C-section might be planned ahead of time. Often, however, the need for a first-time C-section doesn't become obvious until you are in labor. Sometimes a C-section is safer for you or your baby than is a vaginal delivery. Your doctor might recommend a C-section if: Your labor isn't progressing. When your cervix isn't opening enough despite strong contractions over several hours. Your baby isn't getting enough oxygen. For example when There's a problem with the umbilical cord. Your baby or babies are in an abnormal position. You're carrying multiples. There's a problem with your placenta. For example if the placenta covers the opening of your cervix (placenta previa) You have a health codition. complex heart problems, high blood pressure requiring urgent delivery or an infection that could be passed to your baby during vaginal delivery — such as genital herpes or HIV. Mechanical obstruction. You might need a C-section if you have a large fibroid obstructing the birth canal, a severely displaced pelvic fracture or your baby has a very large head. You've had a previous C-section. Depending on the type of uterine incision and other factors, it's often possible to attempt a vaginal birth after a previous C-section. In some cases, however, your doctor might recommend a repeat C-section. Some women request C-sections with their first babies — to avoid labor or the possible complications of vaginal birth or to take advantage of the convenience of a planned delivery. However, this is discouraged if you plan on having several children. Recovery from a C-section takes longer than does recovery from a vaginal birth. And like other types of major surgery, C-sections also carry risks to you and your baby. Risks to your baby include: Breathing problems. C-sections done before 39 weeks of pregnancy or without proof of the baby's lung maturity might increase the risk of breathing problems for babies. Surgical injury. Although rare, accidental cuts to the baby's skin can occur during surgery. Although rare, surgical injuries to nearby organs — such as the bladder can occur during a C-section. If there is a surgical injury during your C-section additional surgery might be needed. Increased risks during future pregnancies. After a C-section, you face a higher risk of potentially serious complications in a subsequent pregnancy — including problems with the placenta — than you would after a vaginal delivery. The risk of uterine rupture, when the uterus tears open along the scar line from a prior C-section, is also higher if you attempt vaginal birth after C-section (VBAC). If your C-section is scheduled in advance, certain blood tests will provide information about your blood type and your level of hemoglobin. These details will be helpful to your health care team in the unlikely event that you need a blood transfusion during the C-section. If your C-section is planned before 39 weeks for a non-emergency reason, your baby's lung maturity might be tested before the C-section. This is done with amniocentesis — a procedure in which a sample of the amniotic fluid is removed from the uterus for testing. Maturity amniocentesis can offer assurance that the baby is ready for birth. Even if you're planning a vaginal birth, it's important to prepare for the unexpected. Discuss the possibility of a C-section with your health care provider well before your due date. After a C-section, you'll need time to rest and recover. This has been the hardest video for me to post. Firstly, because I can't control my emotions AT ALL in it, and secondly because I didn't want to hurt anyone's feelings. Alex and I, and Maryellen and I, talked for a long time about wether or not I should post it. On one hand, I didn't want to hurt anyone. So for so long I have just suffered on my own with these feelings so as not to make anyone else feel that they did anything 'wrong'. On the other hand though, I wanted to tell women what it COULD be like, so that they can really consider the possibilities of what will happen after the birth if it doesn't go exactly as planned. I think, had i considered all of this before James' birth, it may have been easier for me after. Not because I would know and understand the whole emotions of it, but because I would at least TRY to prepare myself for them all. Maybe I'm not making sense, but I really hope this video helps even one person out there. And if anyone has any suggestions for my next birth to help prevent these feelings (if I can't have the VBAC i want) it is greatly appreciated. Please do not attack my family below. Nobody intended on hurting me or anyone else. Why? Because I didn't want to make one video where I talk for an hour or more and you have to sit through information you might not want to watch. So these will give you some options to skip information or parts of the video you don't want to see (if you don't have a lot of time). This has been a LONG and emotional journey for me. Between reliving the whole experience, organizing it all in my head, going back through pictures and videos from the birth, editing it all and putting it all together, I've spent hours and hours crying it all out. I have to say though, as I finish up the videos and get them all uploaded, I actually feel much better that they're out there and I can talk freely about it all now. I KNOW that my experience ended well, and that it could have been a million times worse and more scarring than it was, but it is what happened, and i AM emotional about it. I want to tell the world so that they can maybe plan better, or make more informed decisions than I did that they won't look back sadly on later. The World Health Organization recommends that a cesarean section should be performed only when medically necessary. This can be due to obstructed labor, twin pregnancy, high blood pressure in the mother, breech birth, or problems with the placenta or umbilical cord. In the United States, the National Vital Statistics Reports published, in January 2018 showed that the cesarean delivery rate declined, for the fourth in a row to 31.9 percent of U.S. births in 2016, the lowest rate since 2007. The rate of cesarean delivery peaked in 2009 at 32.9 percent after increasing every year since 1996 (20.7%). If you have a C-section, there are many simple things you can do to help your body recover quickly. #1. Take Advised Painkillers. After a C-section, it’s normal to experience pain. You may have to bear with it for up to two weeks, although it will lessen each day. Pain management is very important for a nursing mother, as pain can interfere with the release of hormones needed to help milk flow. To relieve the pain, doctors often prescribe an anti-inflammatory medication, like ibuprofen and a narcotic pain medication, both of which are safe to take while breastfeeding. However, avoid aspirin or pills containing aspirin for the first 10 to 14 days following surgery, as aspirin can reduce blood clotting. #2. Start Slow Walking. If you consider yourself fit enough to get out of bed, follow the advice, to start moving around seriously. Walking will increase blood circulation, which will reduce your risk of developing blood clots. It will also help prevent typical C-section side effects like constipation and gas buildup in the abdomen, help with bowel function, and increase your body’s ability to heal. #3. Proper Care Of The Incision. Clean your incision daily with warm, soapy water and pat dry your skin. If there are strips of tape on your incision, let them fall off on their own or remove them at the time specified by your doctor. Do not use lotion or powder on your incision. Avoid rubbing, scrubbing, soaking or sunbathing, all of which can slow the healing of your incision. Also, it will increase the risk of reopening the wound. #4. Get Plenty of Sleep and Rest. It is recommended to give your body proper rest for up to six weeks to let it heal completely. Sleeping encourages tissue growth, which will help your wound heal. Sleep also helps to lower your stress levels and reduces your risk of developing postpartum depression. #5. Combat Constipation. As a C-section is a major abdominal surgery involving anesthesia, side effects, side effects like constipation are common. To ease constipation, include more high-fiber foods in your diet. Both soluble fiber from fruits and vegetables and insoluble fiber from whole-grain cereals and breads are beneficial. Along with fiber-rich foods, drink plenty of water and get moving. Amanda Garcia hoped her baby would come into the world peacefully. Knowing she was scheduled for a C-section, she had her doubts. "There was anxiety for that whole process and that whole sterile field and being in this big white room with bright lights and all these people around you, makes you a little nervous," says new mom Amanda Garcia. But Rylee was delivered through a new method - the gentle C-section. The baby is placed directly on the mother's chest right after birth. The surgical team completes the operation in silence. "It creates such a calming environment, you feel like you're the only person in the room when it happens," says Garcia. "We have really worked in the last year to make sure that women that have had a cesarean birth also feel like they've had a baby. A lot of times the baby was taken out and gone to another place before the mom got to hold the baby or see the baby," says Nancy Travis, director of labor and delivery at Cape Coral Hospital. The procedure's taken off since it was introduced at Cape Coral Hospital last year. Mom Melissa Murray had the first gentle cesarean. "They cut his umbilical cord and handed him to the nurse and then she handed him and put him right on my chest," says Melissa Murray, first to have gentle C-section. Approximating a natural setting requires adjustments in the operating room. There is a sterile surgical field to maintain and more work to be done once the baby's born. "We have to adapt the operating room a little bit, where drapes are moved, they move electrodes down, mom has a cardiac monitor on. And we put things in different places so mom has assess to her baby," says Travis. The end justifies the means. Giving mothers something they can hold on to for a lifetime. "You can feel that warmth, you get that immediate eye contact with that child. It is a complete bonding experience," says Garcia. Clinical pathways aim to promote organised and efficient patient care based on evidence-based medicine, and aim to optimise outcomes in settings such as acute care and home care. Couple Changes Plans To Hold Wedding In Intensive Care Unit With Their Daughter. A couple from Alabama, Rubia Ferreira and Tyler Campbell decided to hold their wedding in the NICU unit so that they could share their ceremony in the presence of their tiny daughter, Kaelin Maria. The baby is recovering following an emergency C-section when Rubia developed a rare condition called HELLP syndrome. She was born on November 8, 2017, at 24 weeks gestation, and would need to stay in the hospital for many months to recover. HELLP syndrome is an extremely rare life-threatening pregnancy complication that causes a variety of symptoms including high blood pressure, severe headaches, and blurred vision. Ferreira and Campbell met in Okinawa, Japan when he was stationed there as a Marine. They had planned to return for a beachside wedding but decided to change plans so that they could be married in Kaelin’s presence. Ferreira was inspired by videos she had seen on Facebook of other couples who were married in hospitals. “WE ASKED OUR NURSE LIAISON, AND SHE WAS ALL ABOUT IT!” CAMPBELL RECALLS. “THAT WAS ABOUT TWO WEEKS BEFORE VALENTINE’S DAY, AND SHE THOUGHT IT WOULD BE PERFECT IF THAT WAS OUR WEDDING DAY.” The hospital staff enthusiastically accommodated the couple, and they performed a carefully planned ceremony in the NICU on Valentine’s Day. They walked down the hospital hallway as the doctors, nurses, and staff watched, and music played. The room where Kaelin had been calling home since birth was now a make-shift wedding chapel and the hospital chaplain shared the vows. “MAY YOUR LOVE, AND IT ALREADY HAS, INSPIRE US TO LOVE EACH OTHER AND LOVE PEOPLE IN THE WORLD,” SAID THE CHAPLAIN. The ceremony featured a very special live performance of “Can’t Help Falling in Love” by Elvis Presley. Everything went off without a hitch and the couple couldn’t have been happier. “WE WANT KAELIN TO LOOK BACK AT PICTURES FROM OUR WEDDING AND KNOW ABOUT OUR EXPERIENCE AS A FAMILY AT UAB,” CAMPBELL SAID. “WE HOPE SHE UNDERSTANDS HOW MUCH WE CARED FOR AND LOVED HER, AND THAT IT MEANT THE WORLD TO US TO HAVE OUR WEDDING TAKE PLACE RIGHT THERE WITH HER.” Little Kaelin is still facing a long road to recovery, facing complications from lung disease called bronchopulmonary dysplasia (BPD) that affects premature babies. Many people experience joint pain in their knees, elbows and hands. Many people relate this to aging. Because of this, they believe it to be a normal pain and do not seek treatment for it. However, when the body is in pain, it experiences stress. Also, untreated knee pain can cause damage to the knees and cause shifting of the knee. In this video, parents Erin and Luke share their experience about attempting homebirth and making decision to transfer to hospital, ultimately utilizing cesarean surgery. In this case, they were welcomed into the care of Midwife Lonnie Morris with the collaboration and back-up of Dr. Charles Haddad at Mountainside Hospital in Montclair, NJ. Midwife Lonnie shares about their approach of Gentle Cesarean, which they began after studying rates and concluding that mothers who had experienced cesarean had higher rates of postpartum depression and had difficulty breastfeeding. In the United States c-sections are overall rising. Over the last five years progressively there has been an increase in the c-section rate. Especially the primary c-section rate, meaning a patient who has come in to have her first baby and ends up having a cesarean. We have been fortunate here throughout Intermountain Healthcare and specifically Dixie Regional Medical Center that our c-section rates have remained relatively stable over the last several years despite the nation increase. Currently in our population here at Dixie Regional in a first time mom laboring the primary c-section rate is only 13% which last year was the lowest in the country. There are several programs that have been put in place to try and lower that primary c-section rate. For one we don't induce any mother one week before her due date, for elective reasons. And the second is for first time moms we often ask that they go into labor on their own and not be induced unless they get to one week beyond their due date. That's where we've made the biggest difference in hospitals that have a large percentage of inductions the c-section rate and first time moms in some institutions has climbed up to 40%.
