That's because of bargaining power. Government programs, like Medicare and Medicaid, can ask for a lower price from health service providers because they have the numbers: the hospital has to comply or else risk losing the business of millions of Americans. There are dozens of private health insurance providers in the United States and they each need to bargain for prices with hospitals and doctors. The numbers of people private insurances represent are much less than the government programs. That means a higher price when you go to the doctor or fill a prescription. Uninsured individuals have the least bargaining power. Without any insurance, you will pay the highest price. It uses an incredible amount of resources, emits toxic pollutants into the air, and pumps the ground full of formaldehyde, which is known to cause cancer. It's also prohibitively expensive. The average cost of a modern funeral costs between $10,000 to $12,000. There are a number of greener options available though. Cremation uses less resources and requires less space than a traditional burial, but isn't perfect. There are more experimental methods on the horizon such as promession and alkaline hydrolysis. The importance of health care benefits to employers and employees has led to the creation of a variety of federal and state laws. Several laws have been enacted to provide protection for employees who leave their employers, either voluntarily or involuntarily. The Health Insurance Portability and Accountability Act (HIPAA) of 1996 allows employees to switch their health insurance plans when they change employers and to enroll in health coverage with the new company regardless of pre-existing health conditions. The statute also prohibits group insurance plans from dropping coverage for a sick employee and requires them to make individual coverage available to people who leave group plans. One of the greatest impacts of HIPAA comes from its provisions regarding the privacy of employee medical records. HIPAA regulations provide important health care related opportunities and protections for employees.

When I first heard about water fasting, I thought, “No way am I doing this!” But after doing some research and finding out about the benefits of this process, I changed my mind. And now I'm going to tell you what I experienced during my 20-day water fast. The first 2 days were going to be the most difficult. And they were indeed! I felt incredibly hungry — almost as if I had missed several breakfasts, lunches, and dinners. My energy level dropped, and my mood left much to be desired. I was increasingly irritated, and my patience was wearing thin. For more than 10 years, California consumers in most private health plans have had the right to obtain an independent external review of any health plan coverage denials, known as Independent Medical Review (IMR). The California IMR process is available to the more than 24 million Californians covered through health plans regulated by the California Department of Managed Health Care (DMHC) or health insurers regulated by the California Department of Insurance (CDI). This Sacramento briefing focused on a new Kelch Associates analysis of the California IMR program's history and impact, and prospects for change. The federal Affordable Care Act (ACA) mandates independent external review for all health coverage, including products for self-insured group plans. As coverage expands under health reform, the number of health plan enrollees with access to IMR appeal rights will increase. The federal Department of Health and Human Services determined that California need not make any statutory changes to its IMR process to comply with the ACA. Changes to California's IMR process, or the way California administers and evaluates it, will be state driven. The briefing highlighted several potential administrative improvements that position the state to more effectively deliver on the promise of a credible, transparent, and effective IMR program. Best Way To lose extra fat from body: Intermittent Fasting. For most people, the word fasting brings up the notion of prolonged periods without food or maybe even drink. Some people fast periodically for religious reasons or medical necessity (say, before a procedure). But a new type of diet has people fasting to lose weight, improve health, live longer (and healthier), boost mental clarity, and more. How Intermittent Fasting Affects Your Hormones Body fat is the body's way of storing energy (calories). When we don't eat anything, the body changes several things to make the stored energy more accessible. Here are some of the things that change in your metabolism when you fast: Insulin: Insulin increases when we eat. When we fast, insulin decreases dramatically (4). Lower levels of insulin facilitate fat burning. Human growth hormone (HGH): Levels of growth hormone may skyrocket during a fast, increasing as much as 5-fold (5, 6). Growth hormone is a hormone that can aid fat loss and muscle gain, among other things (7, 8, 9). Norepinephrine (noradrenaline): The nervous system sends norepinephrine to the fat cells, making them break down body fat into free fatty acids that can be burned for energy (10, 11). Interestingly, despite what the 5-6 meals a day proponents would have you believe, short-term fasting may actually increase fat burning. BOTTOM LINE: Short-term fasting leads to several changes in the body that make fat burning easier. This includes reduced insulin, increased growth hormone, enhanced epinephrine signalling and a small boost in metabolism. Intermittent Fasting Helps You Reduce Calories and Lose Weight The main reason that intermittent fasting works for weight loss, is that it helps you eat fewer calories. All that being said, the benefits of intermittent fasting go way beyond just weight loss. It also has numerous benefits for metabolic health, and may even help prevent chronic disease and expand lifespan (15, 16). BOTTOM LINE: Intermittent fasting is a convenient way to restrict calories without consciously trying to eat less. Many studies show that it can help you lose weight and belly fat. Intermittent Fasting May Help You Hold on to Muscle When Dieting One of the worst side effects of dieting, is that the body tends to burn muscle as well as fat (17). In my opinion, one of the main benefits of intermittent fasting is the simplicity of it. I personally do the 16/8 method, where I only eat during a certain "feeding window" each day. Instead of eating 3+ meals per day, I eat only 2, which makes it a lot easier and simpler to maintain my healthy lifestyle. The single best "diet" for you is the one you can stick to in the long run. If intermittent fasting makes it easier for you to stick to a healthy diet, then this has obvious benefits for long-term health and weight maintenance. How to Succeed With an Intermittent Fasting Protocol Food quality: The foods you eat are still important. Try to eat mostly whole, single ingredient foods. Calories: Calories still count. Try to eat "normally" during the non-fasting periods, not so much that you compensate for the calories you missed by fasting. Consistency: Same as with any other weight loss method, you need to stick with it for an extended period of time if you want it to work. Patience: It can take your body some time to adapt to an intermittent fasting protocol. Try to be consistent with your meal schedule and it will get easier. Most of the popular intermittent fasting protocols also recommend strength training. This is very important if you want to burn mostly body fat while holding on to muscle. In the beginning, calorie counting is generally not required with intermittent fasting. However, if your weight loss stalls then calorie counting can be a useful tool. BOTTOM LINE: With intermittent fasting, you still need to eat healthy and maintain a calorie deficit if you want to lose weight. Being consistent is absolutely crucial, and strength training is important. At the end of the day, intermittent fasting is a very useful tool to lose weight. WHO SHOULDN’T TRY INTERMITTENT FASTING? Do not attempt fasting if you’ve had an eating disorder or signs of disordered eating, which include (but aren’t limited to) binge eating, food obsession, misuse of laxatives and extreme food restriction. If you’re pregnant or breast feeding, you should not attempt intermittent fasting. Anyone being treated for diabetes (with medications) as well as anyone with cancer and those with a compromised immune system should also avoid intermittent fasting (or speak to their doctor before trying it).

Medicare Part A is that portion of your health insurance that pays for your healthcare whenever you are an inpatient in a medical facility. As soon as you become an inpatient in a hospital, Medicare Part A takes over. The same when you become an inpatient in a skilled nursing facility or hospice. The one exception is that if you are not able to physically get to a medical facility for healthcare, Medicare Part A also pays for home healthcare. Think of Medicare Part A is inpatient insurance. Medicare Part B is the part of Medicare that pays for outpatient and physician services. This is the portion Medicare insurance you use when you see a doctor or have lab tests or physical therapy and so on. Medicare Part B also includes some wellness coverage. Medicare Part B is the part of Medicare people use most often because most of our medical care is via outpatient services. Even many surgeries and hospital services are now done as an outpatient and are thus covered under Medicare Part B. Your Original Medicare (Part A & B) is good anywhere in the country. You can see any doctor, go to any medical facility in any state or territory, as long as they accept Medicare. You do not need to get permission from a primary care physician. You have both freedom to go where you need and control to see who you choose. Also it is important to understand that Medicare Advantage Plans are local coverage. If you are a snowbird, and RVer or just travel the country that should rule MAPD as an option. Next in our Medicare Explained video are your three choices. There's a bizarre reason why millions of Americans saw their health plans cancelled in 2013 and, as explained in a new video featuring Robert Graboyes of the Mercatus Center at George Mason University, millions more will lose their plans in years to come. Insurance coverage for Americans will remain in permanent turmoil because the Affordable Care Act (ACA) requires that all plans fit within cookie-cutter designs called "metallic tiers." (The tiers—bronze, silver, gold, and platinum—refer to the percentage of medical expenses a particular plan pays.) The video also explains that families may have to switch plans repeatedly because, as circumstances change, a plan that fits within a tier one year may not fit in any tier in a later year.
