Which says that my high CPK levels are likely to be related to another autoimmune condition "Polymyositis" and I have been scheduled for an EMG. A hospital visit later and I've got some stronger pain killers but still waiting to start on the hydroxychloroquine aka plaquenil. In certain studies, Americans has been found to be the #1 population in vitamin D deficiency. Some of the main symptoms of vitamin D deficiency are a weakened immune system, candida or bacterial overgrowth, emotional ups and downs, hormone imbalance, trouble putting on muscle or losing weight, weakened bones or teeth, cancer, heart disease, or diabetes. Vitamin D supports having a healthy immune response, digestive health, hormone balance, and more. The biggest causes of vitamin D deficiency are not getting enough sunlight, certain chemicals from plastic bottles (bpa), and toxicity. Try to get outside everyday for about 20-30 minutes of natural sunlight. Certain foods that are sources of vitamin D are wild caught fish, raw fermented milk, mushrooms, and eggs. Test Description Aldolase is a glycolytic enzyme that is present in all body cells. The highest concentrations of aldolase are found in the cells of skeletal muscles, the heart, and liver tissue, although the test is considered most specific for muscle tissue destruction. When damage to muscle tissue occurs, cells are destroyed, resulting in the release of aldolase into the blood. Thus, testing for aldolase is useful in monitoring the progress of muscle damage in such disorders as muscular dystrophy. Normal Values Adult: 0–7 U/L (0–117 nkat/L SI units) Child: Two times adult norms Newborn: Four times adult norms Possible Meanings of Abnormal Values Increased Decreased Burns Late muscular dystrophy Dermatomyositis Gangrene Hepatitis Liver cancer Muscle inflammation Muscle necrosis Muscle trauma Myocardial infarction Myositis Polymyositis Progressive muscular dystrophy Pulmonary infarction Contributing Factors to Abnormal Values • Hemolysis of the blood sample falsely increases the test results. • Recent minor trauma, including intramuscular injections, may increase the aldolase level. • Drugs that may increase aldolase levels: corticotropin, cortisone acetate, hepatotoxic drugs. • Drugs that may decrease aldolase levels: phenothiazines. Interventions/Implications Pretest • Explain to the patient the purpose of the test and the need for a blood sample to be drawn. • Although fasting is not required before the test, some institutions require a short fasting period to improve the accuracy of the test results. Procedure • A 5-mL blood sample is drawn in a collection tube containing a silicone gel. • Gloves are worn throughout the procedure. Posttest • Apply pressure at venipuncture site. Apply dressing, periodically assessing for continued bleeding. • Label the specimen and transport it to the laboratory. • Report abnormal findings to the primary care provider.

In primary hyperthyroidism TSH are low and TRH administration induces little or no change in TSH levels. • In hypothyroidism due to end organ failure, administration of TRH produces a prompt increase in TSH. • In hypothyroidism due to pituitary disease (secondary hypothyroidism)administration of TRH does not produce an increase in TSH. • In hypothyroidism due to hypothalamic disease (tertiary hypothyroidism), administration of TRH produces a delayed (60–120 minutes, rather than 15–30 minutes) increase in TSH, The TRH test involves administration of a small amount of TRH intravenously, following which levels of TSH will be measured at several subsequent time points using samples of blood taken from a peripheral vein. The test is used in the differential diagnosis of secondary and tertiary hypothyroidism. First, blood is drawn and a baseline TSH level is measured. Then, TRH is administered via a vein. After 30 minutes blood is drawn again and the levels of TSH are measured and compared to the baseline. Some authors recommend additional blood sampling at 15 minutes. In children, late blood sampling at 60 to 120 minutes is necessary. An increase in the serum TSH level following TRH administration means that the cause of the hypothyroidism is in the hypothalamus (tertiary hypothyroidism), i.e. the hypothalamus is not producing TRH. Therefore, when TRH is given exogenously, TSH levels increase. If the increase in serum TSH level following TRH administration is absent or very slight, then the cause of the hypothyroidism is in the anterior pituitary gland, i.e. the pituitary is not secreting TSH. Therefore, even when TRH is given exogenously, TSH levels do not rise as the pituitary is diseased. TRH may cause nausea, vomiting and some patients experience an urge to urinate. Rarely, TRH may cause blood vessel constriction leading to hemorrhage in patients with pre-existing pituitary tumors. Accordingly, patients should be advised about the risks, albeit rare, of TRH testing.
