If you dig through a little bit of fat loss research you'll see right away that insulin is very closely connected with burning fat and losing weight. In fact without reducing insulin there is almost no way we can burn a substantial amount of fat no matter how hard we exercise and diet. And even though most people have heard about insulin spikes not being good for weight loss the average person doesn't truly understand why. Well my hope is that today I can explain this in a way that anybody can understand. It all starts with you eating food. When you eat food the chewed up food particles get broken down further in your stomach into very small parts and then they pass through your intestines at which point your body absorbs and breaks down those nutrients and calories again and dumps them into your bloodstream. These broken down nutrients can't just jump into the cells that need them for their daily functions without some help. That's where insulin comes in. Insulin is a hormone secreted by your pancreas and it gets released into your bloodstream when your body senses that blood sugar has increased. Very high blood sugar levels are not good for you and your body knows this. So insulin acts as a bridge between your cells and the sugar found floating around in your blood. Without insulin your blood sugar would be very high because your body wouldn't be able to get that sugar into the cells. This is actually pretty much what type 2 diabetes is. When people are insulin resistant which means that insulin isn't doing its job they usually wind up being labeled as a type 2 diabetic which means that they need an external source of insulin for example either a shot or a pill to get their blood sugar down. So what does all of this have to do with fat loss and weight loss. Well whether you eat pasta brown rice or drink a can of soda all of it will eventually be broken down and it will be blood sugar floating around in your bloodstream. The only difference is that whole high-fiber carbs like brown rice will take a lot longer to digest which means that the sugar will be released into your bloodstream slowly while the can of soda will pretty much be released all at once. When it's released into your bloodstream slowly you give your body a chance to actually use it overtime so you won't require quite as much insulin which leads to less of it being stored as fat. Before insulin starts storing blood glucose into fat cells it will first store as much of it as possible into your liver for later use. The liver can only hold a limited amount of blood sugar and then the excess starts to get dumped off into your muscle cells. Now your muscle cells can hold more of this glucose then your liver can however it also has its limit. By building more muscle you're going to be able to increase this limit and use more of the blood sugar for your muscles rather than storing it as fat, however again even the most muscular person on the planet is going to have a limit of how much of this blood glucose can actually be used by the muscles. Once that limit is reached we hit a point called spillover where the sugar that came from mostly carbohydrates in your diet will begin to be stored in fat cells in order to lower the sugar found in your blood stream to normal levels. So insulin is an anabolic hormone that causes weight gain and growth. The opposite hormone to insulin is a catabolic hormone known as glucagon which is released when blood sugar drops too low. Glucagon just like insulin goes in and acts as a bridge between the stored energy in your cells and your bloodstream. Except it has the inverse effect. So it pulls energy from your cells and dumps it into the bloodstream. Before your body will start pulling fat from fat cells for energy it will first pull that stored glucose from your liver and your muscle cells. This is why so many low carb diets work. When you don't have that many carbs it's easy for your body to use up the carbs stored in your muscles and your liver and then proceed straight to burning fat for energy. This is the same reason why ketogenic diets work. Except an added benefit to ketogenic diets is that your body becomes more efficient at using fat for energy which means more fat will burnt.

Diabetes refers to a group of conditions characterized by a high level of blood glucose, commonly referred to as blood sugar. Too much sugar in the blood can cause serious, sometimes life-threatening health problems. There are two types of chronic diabetic conditions: type 1 diabetes and type 2 diabetes. Pregnant women may acquire a transient form of the disease called “gestational diabetes” which usually resolves after the birth of baby. Pre-diabetes is when the blood sugar level is at the borderline: higher than normal, but lower than in diabetics. Prediabetes may or may not progress to diabetes. During food digestion, carbohydrates - or carb - break down into glucose which is carried by the bloodstream to various organs of the body. Here, it is either consumed as an energy source - in muscles for example - or is stored for later use in the liver. Insulin is a hormone produced by beta cells of the pancreas and is necessary for glucose intake by target cells. In other words, when insulin is deficient, muscle or liver cells are unable to use or store glucose, and as a result, glucose accumulates in the blood. In healthy people, beta cells of the pancreas produce insulin; insulin binds to its receptor on target cells and induces glucose intake. In type 1 diabetes, beta cells of the pancreas are destroyed by the immune system by mistake. The reason why this happens is unclear, but genetic factors are believed to play a major role. Insulin production is reduced; less insulin binds to its receptor on target cells; less glucose is taken into the cells, more glucose stays in the blood. Type 1 is characterized by early onset, symptoms commonly start suddenly and before the age of 20. Type 1 diabetes is normally managed with insulin injection. Type 1 diabetics are therefore “insulin dependent”. In type 2 diabetes, the pancreas produces enough insulin but something goes wrong either with receptor binding or insulin signaling inside the target cells. The cells are not responsive to insulin and therefore cannot import glucose; glucose stays in the blood. In other words, type 2 diabetics are “insulin resistant”. Here again, genetic factors predispose susceptibility to the disease, but it is believed that lifestyle plays a very important role in type 2. Typically, obesity, inactive lifestyle, and unhealthy diet are associated with higher risk of type 2 diabetes. Type 2 is characterized by adult onset; symptoms usually appear gradually and start after the age of 30. Type 2 diabetes accounts for about 80 to 90% of all diabetics. Management focuses on weight loss and includes a low-carb diet.

The answer is yes, because in both situations your sugar is high and your insulin will also be high. This is because in type 1, you're taking insulin and fluctuating be a higher high sugar state and a low blood sugar state and taking some carb to raise it. The big point is rarely does one focus on the triggers to blood sugars in the first place. This is a deeper root cause and if you could take the least amount of insulin to control blood sugars then you will receive the least amount of side effects. High insulin is a bit more dangerous than high sugars, but both are equally bad for your eyes (retina), your kidneys, your heart and blood vessels and your nerves and brain. So the top two things you need to do is reduce your carbs and do intermittent fasting. Both of these actions will greatly take the stress of the cells of the pancreas and liver and allow you to take less insulin. Get with your doctor as you manage your medications on this. Someone had a question, can I do Keto and Intermittent Fasting if I am diabetic, on Metformin and Insulin at the same time. I don’t know if this person is type 1 or type 2, chances are here type 2 going into type 1 eventually. But here’s the difference, type 1 is the problem of pancreas not producing insulin. Type 2 is more of a problem with the receptor and not receiving insulin and the feedback goes back and causes the pancreas to produce more insulin to compensate. It is a compensatory type of thing and you have high amounts of insulin. But both of these situations type 1 and type 2 have high blood sugars. Metformin makes the receptor more receptive to insulin and the insulin gives you what the pancreas is not providing in the body. But the problems is, if you are type 1, they are giving you insulin, your insulin is going to be high now because you are going to be taking insulin to control this out of control blood sugar. So in both of this conditions you have high sugar and high insulin at the same time. Now it’s more dangerous to have high insulin than it is to have high sugar, there is more problems. Sustain high levels of insulin destroys the retina, kidney, arteries and the brain. Of course high sugars will do it too but high insulin creates even more destruction. The goal is to take the least amount of insulin possible. If you are type 1 and maybe your cells are completely destroyed of any chance of recovery but we still want you to take the least amount of insulin possible so it would make sense to avoid the triggers of insulin. Unfortunately, the American Diabetes Association does not recommend what i'm gonna recommend but the needs to be more focus on the diet and the frequency of eating because reducing your carbohydrates and doing the intermittent fasting are the two things they are gonna allow the person to take the least amount of insulin. And it's also gonna help improve insulin resistance which will take the stress off the liver and the cells. DR. Do you kind of HATE healthy foods like vegetables and fruit? You're not alone. Jamie, up until a few years ago, ate nothing but junk food - all the time - chips, soda, candy, anything she could find that had lots and lots of sugar, or was fried, or processed, or just plain bad for you. It was all she craved. Even though her Mom told her not to eat all that junk food, and tried to get her to eat better healthier foods, she just ignored her and went on eating whatever she wanted. And this went on for a few years, until, Jamie started having trouble hearing. She didn't think much of it, but it got bad enough so that her Mom ended up taking her to the hospital. After the nurse tested her blood sugar - which Jamie hated since she hates blood and needles - she came back with bad news. Jamie had diabetes! It was a diagnosis she did not expect nor a disease she wanted. Her Mom was concerned, but Jamie was just totally confused - she had to learn all about it fast, AND, she had to take medicine - insulin - every single day. So, with a serious fear of needles and shots, Jamie found out that she had to give herself the shots, and monitor and test her blood sugar levels on her own. It was scary, and people would look at her when she had to do it and make her feel different and uncomfortable. But the hardest part of all was that she had to stop eating all of the junk food and sugar and start eating healthy foods. As a diabetic, She had to totally change her diet. And eventually, she got used to her new diet and at making healthier choices for eating and snacking. She even got used to the shots and is really living a normal life - but she does have to take care of herself a little more carefully than most kids her age and manage her diabetes. That raises the question about expectations. Metformin appears to change the gut biome over about a month. The bacteria in the biome impact the appetite after about a month. But decreased appetite alone will not create weight loss. In fact, even with metformin, weight loss requires total lifestyle change. And patience. I recommend a low carb, plant-based diet for those that can use it. It has a positive impact on glucose fluctuation, hunger, and the hunger hormone ghrelin. Intermittent fasting and mimicked fasting change the addictive relationship we develop with food after eating 3 times/day for decades. Mimicked fasting also seems to have a positive impact on transcription and expression of genes critical to glucose metabolism. Even with the correct dietary changes, exercise is a critical part of weight loss. Resistance training and high intensity interval training (HIIT) are creating the best results in this area. But I wouldn't leave out endurance training for long-term energy burning capacity. There is a lot of lay press buzz about our gut biome (bacteria) has a big impact on hunger, and the amount we eat. The science behind this is growing quickly. It also happens that the science behind gut biome changes with Metformin is growing as well. In fact, there is a clear scientific case to be made that metformin actually has both a glucose metabolism effect and a weight loss effect by changing the gut biome. It decreases bacteroides species. And it increases e coli as well as Akkermansia species.This actually provide a helpful tip: if you:re not getting an impact from immediate release metformin, try the Extended Release. The Extended Release appears to have more of a gut-related impact than liver. About Dr. Brewer - Ford Brewer is a physician that started as an Emergency Doctor. After seeing too many patients coming in dead from early heart attacks, he went to Johns Hopkins to learn Preventive Medicine. DO some detective work. Tracking everything you eat and drink for at least a week is the best way to spot patterns. "You might find that you graze a lot more throughout the day than you realized, or that you often forget to eat breakfast," Brown says. You can use an app or pen and paper, whichever you prefer. 4. DON'T blow off breakfast. Or lunch. Or dinner! It backfires. "When you skip meals, you're setting yourself up for a poor eating pattern for the day, as you'll probably be hungrier later on," says Jaclyn London, RD, senior clinical dietitian at The Mount Sinai Hospital in New York. 5. DO work with your emotions. Many people overeat when they're worried or depressed. "Stress is a huge factor. It actually raises your blood sugar levels," Brown says. She often tells her clients to meet with a therapist to learn other ways to handle stress. 6. DON'T keep your goal a secret. Having a strong support system can make all the difference. That can include friends, family, co-workers, or people who are working toward the same goal. You can also team up with experts. "I'm a big believer in working with a lifestyle coach, whether it's in person, over the telephone, or via the Internet," Dansinger says. You'll get the advice, structure, and people to hold you accountable. That can make you five times more likely to lose 10% of your body weight. Recent guidelines (issued jointly by the American Heart Association, American College of Cardiology, and The Obesity Society) urge doctors to refer overweight and obese people to a comprehensive lifestyle program that lasts at least 6 months. 7. DO more than diet. Watching what you eat is a good start. Exercise also matters a lot. Besides cardio, you should also do strength training. Lifting weights or working with resistance bands will help you build muscle and, in turn, curb insulin resistance -- when your body doesn’t respond to the insulin it makes. "Your muscles play a large role in using and storing sugar, so keeping them strong is really important for maintaining healthy blood sugar levels," says Wayne Westcott, PhD, an instructor of exercise science at Quincy College. Aim to do some strength training at least twice a week. Be active as much as possible throughout the day. Research links long periods of sitting to a bigger chance of getting certain diseases, including diabetes. Brown suggests small bursts of activity every hour. Get up and refill your water bottle, walk to the farthest bathroom, or go chat with someone in person instead of sending an email or a text. 8. DO keep eating carbs. You can, and should, keep carbs in your diet. "Our brains run on carbs!" Brown says. The key is to watch portion sizes. A serving is about the size of your fist. You should also aim to cut back on the refined stuff (like white bread and pasta) in favor of healthier, less processed options. Whole-grain pasta, brown rice, quinoa, and sweet potatoes are good choices. 9. DON'T let a setback sabotage you. Effect of Whey Protein on Insulin Whey is one of the two main proteins in milk - the other is casein. Whey, like all proteins, is made up of amino acids, the building blocks of protein. When we consume protein, we then break down these amino acids and use them for muscle growth and tissue repair. Whey contains all three BCAAs along with all of the essential amino acids. Whey is highly insulinogenic - consuming whey leads to a sharp increase in insulin levels. This response does not have to do with blood sugar levels - when a food has a high glycemic index, consuming it will cause a large increase in blood sugar levels. In response to this, the body produces insulin to combat the high blood sugar and allow glucose to enter our cells for energy. This is not what is happening when we consume whey - insulin release is responding to amino acids, not glucose levels. Insulin binds to cells to allow them to take up energy, which can be glucose or amino acids from proteins. The amino acids in whey cause an insulin release. Whey also leads to the release of GIP and GLP-1, two gastrointestinal hormones that also raise insulin levels. This increase in insulin is actually a good thing! Whey is a fast acting protein, and the spike in insulin allows amino acids into the muscle cells to encourage repair, helping us to gain muscle and heal quickly. Consuming whey protein has actually been shown to lower blood glucose levels when consumed during or directly before a meal. A 2005 study in the American Journal of Clinical Nutrition found that when whey was combined in a high-sugar meal, individuals with type-2 diabetes had a lower blood glucose level following the meal than if no whey was consumed. This is thought to be because the increased insulin aided the body in glucose digestion. Generally, the better the whey, the higher the insulin spike and thus the more effective at delivering building blocks into the cells for the purpose or repair and growth. Best to consume whey directly following exercise. It is quickly broken down and made available to muscle cells for repair post workout. Whey protein isolate contains 90% protein and thus has less lactose, fat and cholesterol than whey protein concentrate. To get the most out of your protein, go for the whey protein isolate. You will experience less bloating and get more of the benefits - lean, ripped muscles. Too much protein is dangerous, so be sure not to consume excess protein. Lower levels of insulin facilitate fat burning. Human growth hormone (HGH): Levels of growth hormone may skyrocket during a fast, increasing as much as 5-fold (5, 6). Growth hormone is a hormone that can aid fat loss and muscle gain, among other things (7, 8, 9). Norepinephrine (noradrenaline): The nervous system sends norepinephrine to the fat cells, making them break down body fat into free fatty acids that can be burned for energy (10, 11). Interestingly, despite what the 5-6 meals a day proponents would have you believe, short-term fasting may actually increase fat burning. BOTTOM LINE: Short-term fasting leads to several changes in the body that make fat burning easier. Intermittent Fasting Helps You Reduce Calories and Lose Weight The main reason that intermittent fasting works for weight loss, is that it helps you eat fewer calories. All that being said, the benefits of intermittent fasting go way beyond just weight loss. It also has numerous benefits for metabolic health, and may even help prevent chronic disease and expand lifespan (15, 16). BOTTOM LINE: Intermittent fasting is a convenient way to restrict calories without consciously trying to eat less. Many studies show that it can help you lose weight and belly fat. Intermittent Fasting May Help You Hold on to Muscle When Dieting One of the worst side effects of dieting, is that the body tends to burn muscle as well as fat (17). In my opinion, one of the main benefits of intermittent fasting is the simplicity of it. I personally do the 16/8 method, where I only eat during a certain "feeding window" each day. Instead of eating 3+ meals per day, I eat only 2, which makes it a lot easier and simpler to maintain my healthy lifestyle. The single best "diet" for you is the one you can stick to in the long run. If intermittent fasting makes it easier for you to stick to a healthy diet, then this has obvious benefits for long-term health and weight maintenance. How to Succeed With an Intermittent Fasting Protocol Food quality: The foods you eat are still important. Try to eat mostly whole, single ingredient foods. Calories: Calories still count. Try to eat "normally" during the non-fasting periods, not so much that you compensate for the calories you missed by fasting. Consistency: Same as with any other weight loss method, you need to stick with it for an extended period of time if you want it to work. Patience: It can take your body some time to adapt to an intermittent fasting protocol. Try to be consistent with your meal schedule and it will get easier. Most of the popular intermittent fasting protocols also recommend strength training. This is very important if you want to burn mostly body fat while holding on to muscle. In the beginning, calorie counting is generally not required with intermittent fasting. However, if your weight loss stalls then calorie counting can be a useful tool. BOTTOM LINE: With intermittent fasting, you still need to eat healthy and maintain a calorie deficit if you want to lose weight. Being consistent is absolutely crucial, and strength training is important. At the end of the day, intermittent fasting is a very useful tool to lose weight. WHO SHOULDN’T TRY INTERMITTENT FASTING? Do not attempt fasting if you’ve had an eating disorder or signs of disordered eating, which include (but aren’t limited to) binge eating, food obsession, misuse of laxatives and extreme food restriction.
