That’s the consensus of a new study published in the International Journal of Rheumatic Diseases. The randomized, open-label study involved 150 adults who with rheumatoid arthritis who had been taking disease-modifying anti-rheumatic drugs (DMARDs) for a mean of more than three years. At the beginning of the study, nearly half (49%, 73 individuals) were found to be deficient in vitamin D. The participants were given 60,000 IU vitamin D per week for six weeks, followed by 60,000 IU per month for a total duration of three months. Investigators evaluated vitamin D levels and disease activity among 59 (80.8%) of the vitamin D deficient patients who completed the 12 weeks of treatment and observed the following: • Significant improvement in disease activity score • Significant improvement in serum vitamin D levels from a mean of 10.05 ng/mL to 57.21 ng/mL. The role of vitamin D for health promotion has undergone a paradigm shift. While traditionally thought to only be important for development and maintenance of strong bones, an impressive body of scientific research has accumulated over the past decade, showing that adequate vitamin D levels are necessary to prevent many diseases, especially cardiovascular disease, high blood pressure, endothelial dysfunction, diabetes (both type-1 and type-2), the metabolic syndrome, chronic inflammation, cancer, osteoporosis (including falls and fractures), muscle weakness, cognitive dysfunction and mental illness, autoimmune diseases (e.g. multiple sclerosis, rheumatoid arthritis), infectious diseases, as well as infertility and adverse pregnancy and birth outcomes [1-24]. The joints most likely to be affected are the hips and knees and those of the fingers, neck and lower back. In RA, the body's immune system—which normally defends us against invaders such as viruses and bacteria—attacks the membrane lining the joints. The joints usually affected are those of the hands, wrists, shoulders, elbows, knees, ankles and feet. If not stopped, this attack can cause joint damage and deformity. In recent years, research has led to great advances in treating RA. For many people, drugs that modify the immune system improve symptoms and prevent joint damage. But for OA, treatment does little to help the disease's progression. For most people, treatment consists of medicine to control pain followed by surgery when pain is no longer controllable. But there's hope on the horizon. Many scientists, including David T. Felson, M.D., MPH, chief of the Boston University Clinical Epidemiology Research Training Unit, believe research holds clues for better treating the disease and perhaps more importantly, preventing its development. Costochondritis is an inflammation of the cartilage connecting your ribs to your breastbone (also known as your sternum). The area where the sternum meets a rib is known as the costosternal joint. Costochondritis is the most common cause of chest pain originating in the chest wall. It can be especially frightening because the pain resembles the pain of a heart attack. Causes of costochondritis Not all costochondritis cases have a clear cause. But sometimes it's relatively easy to figure out. The most common cause is injury from a hard blow of some kind. Lifting, pushing, or pulling something that's too heavy is also a possible cause. Fibromyalgia is another possibility. Fibromyalgia is a condition that causes pain and tenderness in various areas of the body. The upper part of the breastbone is one of these. Fibromyalgia causes pain that feels similar to arthritis. Infection can develop in the costosternal joint. An upper respiratory infection that produces sneezing or a cough may also be a cause. Symptoms Pain is generally specific to the areas where the ribs meet the breastbone. Most of the time, the pain is sharp, although some costochondritis sufferers report an aching or gnawing type of pain. For some reason, the pain often happens on the left side of the breastbone. But it can occur on either side. When you have costochondritis, you'll probably feel pain just from breathing. Coughing or sneezing can be especially uncomfortable. As mentioned above, people often think they're having a heart attack when they first experience costochondritis. But angina, or chest pain associated with a heart attack, tends to be more widespread across the chest. Angina also feels as if it's coming from inside your chest. Costochondritis discomfort occurs more toward the outside where the ribs meet the breastbone. Costochondritis pain also remains constant. Angina grows more intense with activity. However, if your chest hurts, you're better being safe and seeking medical attention right away. Let the doctors figure out if your pain is caused by costochondritis or angina. Treatment Very often, costochondritis needs no treatment. The body heals itself in a few days. However, there have been cases where rib pain from costochondritis continued for several months. You'll probably want to ease your pain until the condition heals. Doctors generally recommend NSAIDs, which are nonsteroidal anti-inflammatory drugs. If pain is keeping you awake at night, your doctor may prescribe a type of medicine called tricyclic antidepressants. He or she may also prescribe muscle relaxants, which should help ease your discomfort. While you're healing, you should also try to rest. A heating pad may give you some relief. Take it easy and don't push your recovery. There's little you or your doctor can do except let the healing process take place in its own good time.
