So, what is Metabolic Syndrome, why is it so serious and why haven’t you heard about it? Here is what you need to know. The Metabolic Syndrome is a cluster of the most dangerous heart attack risk factors: raised blood glucose; central adiposity (often called beer belly); abnormal cholesterol levels; abnormal blood fats (also called triglycerides); and high blood pressure. It is estimated that 1 in 4 adults have Metabolic Syndrome and the condition becomes more common as we get older; so that by the age of 50 nearly a third of adults have Metabolic Syndrome. So why is Metabolic Syndrome so important? Well if you have Metabolic Syndrome, you are much more likely to suffer a heart attack or stroke, more likely to develop full blown diabetes, more likely to develop cancer and more likely to develop Alzheimer’s disease. All of these conditions are serious and they are getting far more common. Dementia and Alzheimer’s disease have overtaken cancer and heart disease as the leading cause of death in England and Wales in 2016 and the Metabolic Syndrome and diabetes are thought to be important risk factors for these conditions. So, how can you check to see if you have Metabolic Syndrome? Well, first of all, get out a tape measure. If you are a man and your waist circumference is more than 94 cm or if you are a woman and your waist is more than 80 cm you have one of the main features of Metabolic Syndrome. If in addition you have any 2 of the following you will be classified as having Metabolic Syndrome: raised blood triglycerides; reduced HDL cholesterol-sometimes known as good cholesterol; raised blood pressure; or raised fasting plasma glucose. Note if you are a man of South Asian or Indian heritage, the important threshold for waist circumference is 90 cm. Metabolic Syndrome has not yet been classified as a disease – it is still regarded as a collection of signs and symptoms which together are serious risk factors for our most deadly and disabling medical conditions. Several medical organisations such as the World Health Organisation (WHO), the Independent Diabetes Federation (IDF) and the European Group for the Study of Insulin Resistance (EGIR) define the Metabolic Syndrome in very slightly different ways and this adds even more to the confusion about what exactly Metabolic Syndrome is. However, many experts now believe that the underlying problem in Metabolic Syndrome is insulin resistance. Insulin resistance develops when this hormone cannot work properly, when blood insulin levels are abnormally high and still relatively ineffective in keeping blood glucose levels regulated properly. Metabolic specialists now use the term “Insulin Resistance Syndrome” instead of Metabolic Syndrome. Insulin is a hormone produced by the pancreas and it is important in the metabolism of carbohydrates and fats. When sugar and carbohydrates are eaten, insulin promotes the uptake of glucose from the blood into muscles and fat cells. Hence, it is often said that insulin allows cells to use glucose for energy and also to store excess glucose and sugar as glycogen and fat. When blood glucose levels fall, stored glucose is released for energy from the breakdown of glycogen stores in the liver and in the muscles. In the Metabolic Syndrome, insulin levels are chronically high. Fasting blood levels of insulin in the morning are often abnormally high and insulin levels rise to very high levels when food is eaten. Continuously high levels of insulin cause the liver cells to store fat, resulting in a fatty liver which cannot function normally. As the Insulin Resistance worsens, the pancreas may not be able to produce enough insulin in response to food and then glucose levels in the blood rise above the threshold for the patient to be called diabetic. So why haven’t you heard of Metabolic Syndrome? Well, until quite recently, the connection between Metabolic Syndrome and a host of chronic diseases was not fully understood. We now know that insulin resistance is the key. Until recently, it was difficult to measure blood insulin levels, but, the technology has improved and we can now measure blood insulin levels accurately and we have good tests to diagnose Insulin Resistance as well as good strategies to reverse insulin resistance and the Metabolic Syndrome. Once you can measure Insulin Resistance, you are well on your way to fixing it and monitoring improvement with interventions.

Mitch and I will soon be leaving to spend my summer break in Australia, New Zealand, and possibly Singapore doing food challenges. While we are "down under," we will finish posting the videos from our 2016 European Tour, and will then move on to posting all our 2016-2017 Northwest USA Winter Tour videos. I got really BIG during the second half of our 2016 European Tour, and many people are commenting about my current weight and health, not knowing the videos were filmed in 2016. Therefore, I wanted to get all my blood levels and everything tested and publish the results before we leave, confirming for our subscribers that I am a professional and I am not just some idiot with a death wish and no real regard for my physique and personal health. All three pages from the blood test and urinalysis are posted in this video. To analyze them more closely, just pause the video where needed. Mairead McGuinness is an Irish member of the European People's Party (EPP), Vice-President of the European Parliament, and co-chair of the MEP Heart Group. She answered questions by EurActiv's Evan Lamos. How important is the issue of cardiovascular disease in Europe? What family has not been affected? My own family, my neighbour's family, everyone in the community knows somebody who has been affected by heart disease. It's very common, it's too common. We know that there are ways of reducing the levels of heart disease. We also know that it causes untold stress and difficulty and sadness for families and that we have the capacity to act on it. So it's everywhere, it's a huge problem, many die from it, others suffer long-term illness because of it. So, in a sense, we see it, we know it, we have to do something about it. On the good news side, I think we're more aware of cardiovascular problems and heart disease generally. We know that smoking, for example, is a real problem. That's one of the things that's causing this issue. We know that our lifestyle, what we eat, how active or inactive we are - we know that there's genetic issues as well there - but there are many things about how we live, the activities we undertake as human beings that affect our heart and our cardiovascular system and that we can actually take action on. What can be done to address genetic diseases like familial hypercholesterolemia? What's been fascinating for me is to learn about the fact that cholesterol and high levels of it can be genetic. And that even if you try very hard to live the perfect life in terms of diet and exercise, that there is a precondition within your genes that make you prone to high cholesterol, which ultimately could kill you. So I was shocked to learn that one in two hundred Europeans are impacted by this. And I guess that that one in two hundred, many of them do not know because they have not been tested. And I really do think that my awareness now of this fact pushes me to look back on my own familial gene code, if you like, to see, could I be one of those? Because at the moment, I don't know. So I need to be tested, I think other people who know that family members have had heart disease in the past probably need to go to their GP and say, "look, could you check me out?". Because if it is a genetic precondition, there is medication that can help. And therefore we need medical interventions. But the first thing to know is, are you one of the many who are affected? As I look around here, there could be umpteen people walking around innocently thinking that they have the perfect heart, because they haven't checked themselves out for this genetic precondition to high cholesterol levels, dangerously high, which ultimately will kill you. What roles does the EU have in improving cardiovascular health? I think of issues around research. I think we need and are coordinating better the research that's happening in our member states around cardiovascular health. A key issue, because we don't want duplication, we want good results. I think we need to press the Commission for action on issues around what's in our foods. For example, industrial produced trans fatty acids is a really big issue. Some individual countries are doing things, but perhaps we need the European Union, collectively, to take action on it. You know, lobbying and pressure on the food industry does work, because we have seen that many, many big companies will react to the pressure. They want to engage with their customers and they know it makes good business sense for them to do that. But there are some areas where we may need the Commission to propose legislation so that there's no ambiguity and that when we go out to shop, we can see on a label, or hopefully not see in a food stuff, these ingredients which actually damage our health. So I think on those issues, yes Europe can be more proactive. Because it's really important that we understand that not just because it's good that people live longer and better quality lives, but for the European Union, with an ageing population, the costs of our ill health, if you like, the high cost of poor cardiovascular health is enormous for our hospitals and for the wider health services. And I think we owe it both, as a society, to our well-being and to our economic well-being, to take action. And I think those two parallel issues actually meet up very closely and ultimately will mean, I hope, that my children will have a better knowledge of better hearts, and live longer, better lives than perhaps previous generations or indeed myself will live.

The Metabolic Syndrome is a cluster of the most dangerous heart attack risk factors: raised blood glucose; central adiposity (often called beer belly); abnormal cholesterol levels; abnormal blood fats (also called triglycerides); and high blood pressure. It is estimated that 1 in 4 adults have Metabolic Syndrome and the condition becomes more common as we get older; so that by the age of 50 nearly a third of adults have Metabolic Syndrome. So why is Metabolic Syndrome so important? Well if you have Metabolic Syndrome, you are much more likely to suffer a heart attack or stroke, more likely to develop full blown diabetes, more likely to develop cancer and more likely to develop Alzheimer’s disease. All of these conditions are serious and they are getting far more common. Dementia and Alzheimer’s disease have overtaken cancer and heart disease as the leading cause of death in England and Wales in 2016 and the Metabolic Syndrome and diabetes are thought to be important risk factors for these conditions. So, how can you check to see if you have Metabolic Syndrome? Well, first of all, get out a tape measure. If you are a man and your waist circumference is more than 94 cm or if you are a woman and your waist is more than 80 cm you have one of the main features of Metabolic Syndrome. If in addition you have any 2 of the following you will be classified as having Metabolic Syndrome: raised blood triglycerides; reduced HDL cholesterol-sometimes known as good cholesterol; raised blood pressure; or raised fasting plasma glucose. Note if you are a man of South Asian or Indian heritage, the important threshold for waist circumference is 90 cm. Metabolic Syndrome has not yet been classified as a disease – it is still regarded as a collection of signs and symptoms which together are serious risk factors for our most deadly and disabling medical conditions. Several medical organisations such as the World Health Organisation (WHO), the Independent Diabetes Federation (IDF) and the European Group for the Study of Insulin Resistance (EGIR) define the Metabolic Syndrome in very slightly different ways and this adds even more to the confusion about what exactly Metabolic Syndrome is. However, many experts now believe that the underlying problem in Metabolic Syndrome is insulin resistance. Insulin resistance develops when this hormone cannot work properly, when blood insulin levels are abnormally high and still relatively ineffective in keeping blood glucose levels regulated properly. Metabolic specialists now use the term “Insulin Resistance Syndrome” instead of Metabolic Syndrome. Insulin is a hormone produced by the pancreas and it is important in the metabolism of carbohydrates and fats. When sugar and carbohydrates are eaten, insulin promotes the uptake of glucose from the blood into muscles and fat cells. Hence, it is often said that insulin allows cells to use glucose for energy and also to store excess glucose and sugar as glycogen and fat. When blood glucose levels fall, stored glucose is released for energy from the breakdown of glycogen stores in the liver and in the muscles. In the Metabolic Syndrome, insulin levels are chronically high. Fasting blood levels of insulin in the morning are often abnormally high and insulin levels rise to very high levels when food is eaten. Continuously high levels of insulin cause the liver cells to store fat, resulting in a fatty liver which cannot function normally. As the Insulin Resistance worsens, the pancreas may not be able to produce enough insulin in response to food and then glucose levels in the blood rise above the threshold for the patient to be called diabetic. So why haven’t you heard of Metabolic Syndrome? Well, until quite recently, the connection between Metabolic Syndrome and a host of chronic diseases was not fully understood. We now know that insulin resistance is the key. Until recently, it was difficult to measure blood insulin levels, but, the technology has improved and we can now measure blood insulin levels accurately and we have good tests to diagnose Insulin Resistance as well as good strategies to reverse insulin resistance and the Metabolic Syndrome. Once you can measure Insulin Resistance, you are well on your way to fixing it and monitoring improvement with interventions.

High blood pressure: Guidelines for treatment & high pressure goals - Prof Bryan Williams

For more interviews please visit www.cardio-debate.com *TRANSCRIPT* Guidelines and goal blood pressure in patients with cardiovascular disease: What is your recommendation regarding goals to achieve? There has been a lot of debate about how low we should lower blood pressure. I think the thing that’s clear is the conventional target across most guidelines in the world is less than 140/90 mm Hg. So we try and get everyone’s blood pressure on treatment below 140/90 mm Hg. The only caveat to that is people who are over the age of 80 years, who have recently started treatment and in whom it might be difficult to get it down to those levels. And the best evidence we have is that we should target those to be below 150 systolic. So 150 systolic, get below that over the age of 80 years. Less than 140 systolic over 90 for everybody else. Now recently there was a publication, a big study from the United States called SPRINT, which targeted half the population in the study to a blood pressure below 120. And they showed a very significant reduction in mortality associated with cardiovascular disease, and even total mortality. So this has really fuelled the debate over whether we should be thinking about going even lower than 140 as our target for the treatment of hypertension. Now before we jump to that conclusion we have to look at the SPRINT trial, and the kind of patients that went into that trial, and whether or not the result is generally applicable to the entire population or whether it really reflects a recommendation for a sub-population. So the people in SPRINT had a high risk, they either had cardiovascular disease or chronic kidney disease, or they were over the age of 75 years. They did exclude people with diabetes and patients with a previous stroke, so that’s the first thing. And secondly, many of the patients who went into the trial didn’t have particularly high blood pressure, so it wasn’t particularly difficult to get down to those targets. So you can imagine if you see a patient in your typical clinical with a blood pressure of around 170, the idea, if they were 70 years old, trying to get them to below 120 – we all know that practically that would be quite challenging and technically quite difficult. The other thing in SPRINT is that there was a significant increased risk of developing some side effects, hypotension, dizziness, renal impairment associated with the lower targets. So it didn’t come free – there was an offset in this trial. So I think the data has stimulated debate, I don’t think it’s a definitive outcome. How do I interpret it in my practice? What I tend to do is I get everyone below 140. Then if I’m dealing with a patient who is quite high risk for cardiovascular disease – previously had a stroke, previously had a myocardial infarction, I will say to that patient ‘If you’re feeling okay at 135, we could try and go a bit lower. And it’s possible, if you tolerate that treatment, that you’ll get some added value from that.” So I think what SPRINT has done is given justification to doctors to think about being more aggressive in patients who tolerate lower blood pressures than the conventional target, but we need to monitor them closely to make sure that they are tolerating this, and they are not running into any problems. Researchers from Oxford University and Queen Mary University of London were involved in its study of risk factors for cardiovascular disease.The report shows 26.9 per cent of British men are obese, putting themselves in danger of a heart attack or stroke, compared to an average EU rate of 22.9 per cent.Even more British women are severely overweight, making up 29.2 per cent of the population, compared with an EU average of 23.1 per cent.Obesity is the second biggest cause of early death worldwide after smoking. The findings suggest the national failure to exercise and drinking culture may be to blame for rising numbers here.Just under 45 per cent of British women fail to get their recommended 150 minutes of moderate exercise or 75 minutes of vigorous activity every week.Men do slightly better but more than a third are still not active enough. Only Malta is worse when it comes to physical inactivity.On binge drinking, the UK is 10th worst among the 47 countries, which include Europe and former Soviet states like Georgia. More than a quarter of Britons admitted heavy drinking in the previous 30 days, compared to the average of 19.1 per cent.The silver lining of the report in the European Heart Journal is that Britain has the lowest proportion of people with high blood pressure and one of the lowest smoking rates.But the authors, also including academics at the universities of Leeds and East Anglia, warned that the current fall in deaths from heart disease could be reversed by our unhealthy lifestyles.Lead author Dr Adam Timmis from Barts Heart Centre, at Queen Mary’s, said: ‘This downward trend for high-income countries is being threatened by the emerging obesity epidemic.‘Interestingly, the figures show that heart disease is as much of a problem for women as for men.’This month, the Organisation for Economic Co-operation and Development warned it has become normal for Britons to be ‘enormously overweight’.

Health benefits of dark chocolate.1) Chocolate lowers cholesterol According to one study, published in The Journal of Nutrition, chocolate consumption might help reduce low-density lipoprotein (LDL) cholesterol levels, also known as "bad fats." The team of researchers set out to investigate whether chocolate bars containing plant sterols (PS) and cocoa flavanols (CF) have any effect on cholesterol levels. The study authors wrote, "results indicate that regular consumption of chocolate bars containing PS and CF, as part of a low-fat diet, may support cardiovascular health by lowering cholesterol and improving blood pressure." 2) Chocolate may prevent memory decline Scientists at Harvard Medical School suggest that drinking two cups of hot chocolate a day could help keep the brain healthy and reduce memory decline in older people. The researchers found that hot chocolate helped improve blood flow to parts of the brain where it was needed. The lead author, Farzaneh A. Sorond, said: "As different areas of the brain need more energy to complete their tasks, they also need greater blood flow. This relationship, called neurovascular coupling, may play an important role in diseases such as Alzheimer's." In a further study, published in 2014, researchers found that a cocoa extract - called lavado - could, perhaps, reduce or prevent damage to nerve pathways found in patients with Alzheimer's disease. This means that symptoms of the condition - such as cognitive decline - could be slowed. 3) Chocolate and heart disease risk Research, published in The BMJ, suggests that consuming chocolate could help lower the risk of developing heart disease by one-third. The report was presented at the European Society of Cardiology Congress in Paris, France. The authors concluded: "Based on observational evidence, levels of chocolate consumption seem to be associated with a substantial reduction in the risk of cardiometabolic disorders. Further experimental studies are required to confirm a potentially beneficial effect of chocolate consumption." 4) Chocolate and stroke Canadian scientists carried out a study involving 44,489 individuals and found that people eating chocolate were 22 percent less likely to suffer a stroke than those who didn't. Also, those who had a stroke but regularly consumed chocolate were 46 percent less likely to die as a result. In a further study, published in the journal Heart in 2015, researchers tracked the impact of diet on the long-term health of 25,000 men and women. The findings suggested that eating up to 100 grams of chocolate each day may be linked to a lower risk of heart disease and stroke. Fenugreek has had a lot of claims made about it from Testosterone boosts in men to increasing milk production in woman, and even fighting hair loss. What does the research say? What are the benefits to eating fenugreek herbs and seeds? In this video highlight some interesting studies which have been conducted on Fenugreek. *This is not medical advice, always consult a physician before taking any new herb or supplement* Fenugreek is a herb that appearance wise, is similar to clover and native to the Mediterranean region, southern Europe, and western Asia. The herb is used as a spice in various traditional foods, and also in traditional medicine. Many choose to make their own teas from the seeds, while some companies sell the herb in pill form as a dietary supplement. When people talk about the plant its important to note that there are different parts of the plant which are studied. There are many different chemical compounds in both the fenugreek herb, the fenugreek seed and also there are different extracts, some that contain the seed husk and others that take the husk out before extraction Tea made from the seeds of the herb has consistently proven to increase milk volume in breast-feeding women. A study published in the JOURNAL OF ALTERNATIVE AND COMPLEMENTARY MEDICINE had one group of new mothers ingesting tea brewed with fenugreek seeds at three cups per day, on the third day they measured mean breast milk volume, the women drinking the tea produced a mean 73ml vs the control group which produced just 31 ml, they also found the babies of the fenugreek tea mothers got back up to their birth weight faster, for the guys watching this, apparently after babies are born they actually lose weight before they gain it, don’t worry I didn’t know that either. Here’s where things get confusing though, in recent years manufacturers have been selling Fenugreek as a testosterone booster… Now I’m going to get this out of the way, based on the evidence it doesn’t look like pure unaltered fenugreek alone, in any way increases muscle and strength, studies have however shown increases in Libedo in men but testing for a rise in testosterone has been unreliable. When talking about Testosterone it’s also important to mention another even more potent hormone, Dihydrotestosterone, or DHT. This hormone is similar to testosterone but is far more potent in activating androgen receptors (the receptors that cause men to display manly features) In our body circulating testosterone is acted on by an enzyme which turns it into this DHT. DHT is also one of the active hormones responsible for male and female pattern hair loss. DHT is anywhere between 2.5 to 10 times more potent than testosterone at activating androgen receptors in your body. And while studies on fenugreek have occasionally shown a mild increase in testosterone, these usually go hand in hand with a mild decrease in DHT. In one study DHT levels in men dropped by 9%. It is thought that this occurs because something in Fenugreek is inhibiting the alpha reductase enzyme, leading to a backlog of testosterone which isn’t converted to DHT. Which in theory could mean a slight benefit if you are suffering from male or female pattern hair loss, but this is also the only thing fenugreek does which can be clearly explained by measurable hormone changes. Just using a tiny testosterone increase doesn’t explain why people report for their libedo increasing. First of all, the testosterone changes which have been detected are only minor, and secondly, the brain’s androgen receptors primarily only respond to DHT and not testosterone, so if fenugreek means less DHT, if anything shouldn’t libedo drop. Well it turns out as I said things are just more complex It has been found that ingesting between 2-5 grams of fenugreek seeds can help lower blood glucose levels, and improve glucose tolerance, something helpful for diabetics and pre diabetics. This could be in part due to the soluble fibre in the seeds slowing down digestion and absorption of carbohydrates, that’s the explanation given by www.diabeties.co.uk

Now, in order to tackle this problem, we have to deal with two aspects. The first one is the intake, which means the consumption of food. The second one is the physical activity. As far as the consumption of food is concerned, one main problem is the consumption of junk food and of food containing too much fat and sugar by children. And the idea is that, along with other policies, other actions, that children are protected from direct marketing and advertising convincing them and inducing them to consume too much of this product." 4. European Union flag flying outside of Commission office building STORYLINE: The European Union's health commissioner said on Thursday that the EU will be setting a policy of protecting children from junk food marketing and advertising in an effort to tackle the growing problem of obesity in European children. EU Health Commissioner Markos Kyprianou said that too much junk food was being consumed by children and obesity levels among children are rising in the EU. The Commissioner also has called for a special forum on obesity in March for the food and advertising industry, in an effort to keep junk food ads away from children. Kyprianou wants the advertising industry no longer to target youngsters with products that contain a high level of salt, fat or sugar. The Commissioner warned that if the levels of obesity were allowed to continue, it would eventually become a problem for the whole health care system in the EU. The European Food Safety Authority last year found that Europeans eat less of the most dangerous, cholesterol-raising fats than Americans do, and the amount is decreasing. However, European children are still getting fatter and the Commissioner feared the 25-nation European Union will be going the same way as the US. Authorities in the US have already ordered food companies to produce more detailed labelling to warn consumers of dangerous, fattening products. There is pressure for the EU to do the same with the presence of anti-obesity drives in several EU nations. Central and eastern European countries have the worst problem with obesity, according to the International Obesity Task Force.
