Bryan Williams, Professor and Chair of Medicine, University College London, UK, talks to Cardio Debate about the guidelines of high blood pressure treatment, how they compare in different countries, and the recent debate about high pressure goals. For more interviews please visit www.cardio-debate.com *TRANSCRIPT* Guidelines and goal blood pressure in patients with cardiovascular disease: What is your recommendation regarding goals to achieve? There has been a lot of debate about how low we should lower blood pressure. I think the thing that’s clear is the conventional target across most guidelines in the world is less than 140/90 mm Hg. So we try and get everyone’s blood pressure on treatment below 140/90 mm Hg. The only caveat to that is people who are over the age of 80 years, who have recently started treatment and in whom it might be difficult to get it down to those levels. And the best evidence we have is that we should target those to be below 150 systolic. So 150 systolic, get below that over the age of 80 years. Less than 140 systolic over 90 for everybody else. Now recently there was a publication, a big study from the United States called SPRINT, which targeted half the population in the study to a blood pressure below 120. And they showed a very significant reduction in mortality associated with cardiovascular disease, and even total mortality. So this has really fuelled the debate over whether we should be thinking about going even lower than 140 as our target for the treatment of hypertension. Now before we jump to that conclusion we have to look at the SPRINT trial, and the kind of patients that went into that trial, and whether or not the result is generally applicable to the entire population or whether it really reflects a recommendation for a sub-population. So the people in SPRINT had a high risk, they either had cardiovascular disease or chronic kidney disease, or they were over the age of 75 years. They did exclude people with diabetes and patients with a previous stroke, so that’s the first thing. And secondly, many of the patients who went into the trial didn’t have particularly high blood pressure, so it wasn’t particularly difficult to get down to those targets. So you can imagine if you see a patient in your typical clinical with a blood pressure of around 170, the idea, if they were 70 years old, trying to get them to below 120 – we all know that practically that would be quite challenging and technically quite difficult. The other thing in SPRINT is that there was a significant increased risk of developing some side effects, hypotension, dizziness, renal impairment associated with the lower targets. So it didn’t come free – there was an offset in this trial. So I think the data has stimulated debate, I don’t think it’s a definitive outcome. How do I interpret it in my practice? What I tend to do is I get everyone below 140. Then if I’m dealing with a patient who is quite high risk for cardiovascular disease – previously had a stroke, previously had a myocardial infarction, I will say to that patient ‘If you’re feeling okay at 135, we could try and go a bit lower. And it’s possible, if you tolerate that treatment, that you’ll get some added value from that.” So I think what SPRINT has done is given justification to doctors to think about being more aggressive in patients who tolerate lower blood pressures than the conventional target, but we need to monitor them closely to make sure that they are tolerating this, and they are not running into any problems. (CNN)A low-carb or high-carb diet raises your risk of death, a new study suggests, with people eating the food staple in moderation seeing the greatest benefits to their health.Less than 40% or more than 70% of your energy -- or calories -- coming from carbohydrates was associated with the greatest risk of mortality. Eating moderate levels between that range offered the best options for a healthy lifespan.The lowest risk of an early death was seen where carbs made up 50-55% of a person's diet, according to the study published Thursday.However, the definition of a low-carb diet had some caveats as not all diets were equal. Although previous studies have shown such diets can be beneficial for short-term weight loss and lower heart risk, the longer-term impact is proving to have more negative consequences, according to the study."Our data suggests that animal-based low carbohydrate diets, which are prevalent in North America and Europe, might be associated with shorter overall life span and should be discouraged," Seidelmann said."On an 'average' 2,000 kcal-a-day intake, a diet of 30% calories from carbs equates to only 150g a day, with sugars (natural or 'added') contributing around 50g of that total. With a mere 100g of complex carb a day to play with, a lower intake of cereals, grains, and starchy vegetables is inevitable," said Catherine Collins, a dietitian with the UK's National Health Service, who was not involved in the study. Mitch and I will soon be leaving to spend my summer break in Australia, New Zealand, and possibly Singapore doing food challenges. While we are "down under," we will finish posting the videos from our 2016 European Tour, and will then move on to posting all our 2016-2017 Northwest USA Winter Tour videos. I got really BIG during the second half of our 2016 European Tour, and many people are commenting about my current weight and health, not knowing the videos were filmed in 2016. Therefore, I wanted to get all my blood levels and everything tested and publish the results before we leave, confirming for our subscribers that I am a professional and I am not just some idiot with a death wish and no real regard for my physique and personal health. All three pages from the blood test and urinalysis are posted in this video. To analyze them more closely, just pause the video where needed. There are about 60 videos waiting to be posted, which will upload over the course of the next 25 to 30 weeks. They all took place and were filmed prior to the blood test that I did on May 11, 2017. My own family, my neighbour's family, everyone in the community knows somebody who has been affected by heart disease. It's very common, it's too common. We know that there are ways of reducing the levels of heart disease. We also know that it causes untold stress and difficulty and sadness for families and that we have the capacity to act on it. So it's everywhere, it's a huge problem, many die from it, others suffer long-term illness because of it. So, in a sense, we see it, we know it, we have to do something about it. On the good news side, I think we're more aware of cardiovascular problems and heart disease generally. We know that smoking, for example, is a real problem. That's one of the things that's causing this issue. We know that our lifestyle, what we eat, how active or inactive we are - we know that there's genetic issues as well there - but there are many things about how we live, the activities we undertake as human beings that affect our heart and our cardiovascular system and that we can actually take action on. What can be done to address genetic diseases like familial hypercholesterolemia? What's been fascinating for me is to learn about the fact that cholesterol and high levels of it can be genetic. And that even if you try very hard to live the perfect life in terms of diet and exercise, that there is a precondition within your genes that make you prone to high cholesterol, which ultimately could kill you. So I was shocked to learn that one in two hundred Europeans are impacted by this. And I guess that that one in two hundred, many of them do not know because they have not been tested. And I really do think that my awareness now of this fact pushes me to look back on my own familial gene code, if you like, to see, could I be one of those? Because at the moment, I don't know. So I need to be tested, I think other people who know that family members have had heart disease in the past probably need to go to their GP and say, "look, could you check me out?". Because if it is a genetic precondition, there is medication that can help. And therefore we need medical interventions. But the first thing to know is, are you one of the many who are affected? As I look around here, there could be umpteen people walking around innocently thinking that they have the perfect heart, because they haven't checked themselves out for this genetic precondition to high cholesterol levels, dangerously high, which ultimately will kill you. What roles does the EU have in improving cardiovascular health? I think of issues around research. I think we need and are coordinating better the research that's happening in our member states around cardiovascular health. A key issue, because we don't want duplication, we want good results. I think we need to press the Commission for action on issues around what's in our foods. For example, industrial produced trans fatty acids is a really big issue. Some individual countries are doing things, but perhaps we need the European Union, collectively, to take action on it. You know, lobbying and pressure on the food industry does work, because we have seen that many, many big companies will react to the pressure. They want to engage with their customers and they know it makes good business sense for them to do that. But there are some areas where we may need the Commission to propose legislation so that there's no ambiguity and that when we go out to shop, we can see on a label, or hopefully not see in a food stuff, these ingredients which actually damage our health. So I think on those issues, yes Europe can be more proactive. Because it's really important that we understand that not just because it's good that people live longer and better quality lives, but for the European Union, with an ageing population, the costs of our ill health, if you like, the high cost of poor cardiovascular health is enormous for our hospitals and for the wider health services. And I think we owe it both, as a society, to our well-being and to our economic well-being, to take action. And I think those two parallel issues actually meet up very closely and ultimately will mean, I hope, that my children will have a better knowledge of better hearts, and live longer, better lives than perhaps previous generations or indeed myself will live.

So, what is Metabolic Syndrome, why is it so serious and why haven’t you heard about it? Here is what you need to know. The Metabolic Syndrome is a cluster of the most dangerous heart attack risk factors: raised blood glucose; central adiposity (often called beer belly); abnormal cholesterol levels; abnormal blood fats (also called triglycerides); and high blood pressure. It is estimated that 1 in 4 adults have Metabolic Syndrome and the condition becomes more common as we get older; so that by the age of 50 nearly a third of adults have Metabolic Syndrome. So why is Metabolic Syndrome so important? Well if you have Metabolic Syndrome, you are much more likely to suffer a heart attack or stroke, more likely to develop full blown diabetes, more likely to develop cancer and more likely to develop Alzheimer’s disease. All of these conditions are serious and they are getting far more common. Dementia and Alzheimer’s disease have overtaken cancer and heart disease as the leading cause of death in England and Wales in 2016 and the Metabolic Syndrome and diabetes are thought to be important risk factors for these conditions. So, how can you check to see if you have Metabolic Syndrome? Well, first of all, get out a tape measure. If you are a man and your waist circumference is more than 94 cm or if you are a woman and your waist is more than 80 cm you have one of the main features of Metabolic Syndrome. If in addition you have any 2 of the following you will be classified as having Metabolic Syndrome: raised blood triglycerides; reduced HDL cholesterol-sometimes known as good cholesterol; raised blood pressure; or raised fasting plasma glucose. Note if you are a man of South Asian or Indian heritage, the important threshold for waist circumference is 90 cm. Metabolic Syndrome has not yet been classified as a disease – it is still regarded as a collection of signs and symptoms which together are serious risk factors for our most deadly and disabling medical conditions. Several medical organisations such as the World Health Organisation (WHO), the Independent Diabetes Federation (IDF) and the European Group for the Study of Insulin Resistance (EGIR) define the Metabolic Syndrome in very slightly different ways and this adds even more to the confusion about what exactly Metabolic Syndrome is. However, many experts now believe that the underlying problem in Metabolic Syndrome is insulin resistance. Insulin resistance develops when this hormone cannot work properly, when blood insulin levels are abnormally high and still relatively ineffective in keeping blood glucose levels regulated properly. Metabolic specialists now use the term “Insulin Resistance Syndrome” instead of Metabolic Syndrome. Insulin is a hormone produced by the pancreas and it is important in the metabolism of carbohydrates and fats. When sugar and carbohydrates are eaten, insulin promotes the uptake of glucose from the blood into muscles and fat cells. Hence, it is often said that insulin allows cells to use glucose for energy and also to store excess glucose and sugar as glycogen and fat. When blood glucose levels fall, stored glucose is released for energy from the breakdown of glycogen stores in the liver and in the muscles. In the Metabolic Syndrome, insulin levels are chronically high. Fasting blood levels of insulin in the morning are often abnormally high and insulin levels rise to very high levels when food is eaten. Continuously high levels of insulin cause the liver cells to store fat, resulting in a fatty liver which cannot function normally. As the Insulin Resistance worsens, the pancreas may not be able to produce enough insulin in response to food and then glucose levels in the blood rise above the threshold for the patient to be called diabetic. So why haven’t you heard of Metabolic Syndrome? Well, until quite recently, the connection between Metabolic Syndrome and a host of chronic diseases was not fully understood. We now know that insulin resistance is the key. Until recently, it was difficult to measure blood insulin levels, but, the technology has improved and we can now measure blood insulin levels accurately and we have good tests to diagnose Insulin Resistance as well as good strategies to reverse insulin resistance and the Metabolic Syndrome. Once you can measure Insulin Resistance, you are well on your way to fixing it and monitoring improvement with interventions.

Bio Dr. Graham S. Downing is a consultant in Neuro-musculoskeletal & Functional Medicine. Living in a Motorhome Fulltime, Steve, Wendy, and Pippin enjoy travelling around the UK and soon to be Europe when time allows. We took a year off and travelled around the UK seeing places that we had no idea existed until we stumbled upon them. Our first adventures took shape in a 10 year old Caravan; a Coachman Pastiche 540/4 which we towed behind a trusty Audi Allroad 3.0TDI Quattro. A beautiful combination of tow car and living space. We purchased the car in January 2016 and the Caravan in March 2016 and set off on a journey to Essex in memory of Wendy’s mum who’d passed away in January 2016. In July 2017 we decided that we’d buy a new motorhome and so we now travel around in a Hymer T678CL Golden Limited Motorhome. We work at a caravan part in the North West of England part time and so get at least 4 days a week when we can be free to travel. The lack of physical activity and unbalanced diets can lead to obesity, diabetes, high cholesterol levels, and chronic diseases such as cardiovascular disease and stroke. Today, more than half of Europeans are overweight or obese.  And according to a new Eurobarometer survey on sport and physical activity, more than half of European citizens seldom do any activity. Maintaining a daily energy balance at a healthy weight, balancing the energy consumed through the diet with the energy expended through physical activity,  can help prevent chronic diseases such as diabetes and CVDs.​  What should be the next steps for decision makers and other stakeholders to help citizens achieve energy balance? Can small changes have a big impact on obesity? What to do first? What does science tell us?
