Today we want to talk about something that's a big concern for a lot of women - we've been getting a lot of questions about this recently - which is, the connection between hCG and cancer. So, pretty simply, hCG for women looks a lot like another hormone, lutenizing hormone, which stimulates the ovaries. So far, that's not a problem for cancer. The effect of the ovarian stimulation is an increase in progesterone levels. Where this can become a problem is if people have pre-existing, let's say, hormone-sensitive breast cancer, absolutely not recommended to do hCG, because the increase in hormones could accelerate the other condition. So hCG, because of its effect in increasing progesterone, can accelerate progesterone-sensitive health conditions, and primarily with our cases, we're looking at things like breast cancer - that's the concern. If people have had their most recent health screening - pelvic exam, breast exam, mammograms based on their age and history, it is generally not a concern. We do require that people have that before being approved. If people are on the diet and they have any type of suspicious lesion or anything unknown that comes up, we also recommend stopping, because we don't know that if there is a risk or not until it's fully evaluated. In summary, hCG does increase progesterone for women, if you have a progesterone-sensitive health condition, this is a problem. Breast cancer, also things like endometriosis, sometimes PCOS, and some other hormonal conditions. Hopefully that answers that question, and I will be answering more next week. What level of blood Beta HCG confirms pregnancy? - Dr. Phani Madhuri
This is the most anticipated tests in women who are undergoing fertility treatment month on month. Blood beta HCG is a test done to determine whether the woman is pregnant or not or to detect various other conditions like cancers etc. But beta HCG is a hormone produced from the corpus luteal cells which supports the fertilized egg in the first few weeks of pregnancy. So beta HCG is detected as early as 10 days that is after the implantation has happened and it becomes very obvious after 14 to 16 days after the implantation process that is roughly about 1 month from your last menstrual period. Usually a value of above 5 could be a determinant factor based on the length of the duration of the cycle or the day of the cycle of the period. But mostly a value of 25 or 50 milli international units is considered as a positive sign in pregnancy. Now single value of beta HCG does not give you any information whether the pregnancy is there or it is going or it is failing. Beta HCG is one of the methods to assess the quality of the pregnancy or the ongoing pregnancy. If the quality of the pregnancy is good and the beta HCG levels double every 48 hours, or there is about 80% increase in beta HCG levels every 48 hours. When this is not happening, then it is nor a very healthy pregnancy, it could be an early miscarriage; it can be an ablighted womb, rarely an ectopic pregnancy. If the beta HCG value is more than double in 48 hours, then it could be wrong dates or multiple pregnancies or it can be molar pregnancy. Now a value of 8 milli international units per ml, if I is pregnancy positive or not. It is a very tricky question. A value of 8 seen on day 30, 35 or day 40 of the cycle, it may not be a very good sign but if the same value is seen probably on day 8 or 9 or day 10 after the implantation, it will be assign of positive pregnancy and you can chart it upwards and see if the value is doubling or not to assess if the pregnancy is going on well or not.

Developed in the late 1950s, by the renowned oncologist, the late Dr. Manuel D. Navarro, the test detects the presence of cancer cells even before signs or symptoms develop. Dr. Navarro found HCG to be present in all types of cancers. The test is based on a theory proposed by Howard Beard and other researchers who contend that cancer is related to a misplaced trophoblastic cell that become malignant in a manner similar to pregnancy in that they both secrete HCG. As a consequence, a measure of the amount of HCG found in the blood or urine is also a measure of the degree of malignancy. The higher the number, the greater is the severity of cancer. Urine, as opposed to blood or serum, is the preferred specimen for the test. In 1980, Papapetrou and co-authors reported the correctness of the urine specimen to be used in HCG Immunoassay. In 32 proven cancer cases, the immunoassay test gave 31 positive results using urine while only 12 positive results were reported using blood. HCG has been found to undergo glycosylation in the liver as it travels in the hepatic circulation. Thus, the HCG molecule cannot be detected. The molecule does not undergo this process in the kidney and therefore the molecule remains intact in the urine. The test detects the presence of brain cancer as early as early as 29 months before symptoms appear; 27 months for fibro sarcoma of the abdomen; 24 months for skin cancer; 12 months for cancer of the bones (metastasis from the breast extirpated 2 years earlier). Since the late 1970s women have been home testing to find out if they're pregnant. The telltale marker speaks volumes. "Pregnancy tests work by picking up BETA HCG, which is a hormone made by women during pregnancy," says Dr. Frank Rodriguez, oncologist on Lee Memorial Health System medical staff. Meant for female use, the test has implications beyond what you'd expect for men. In fact, it's possible that a man can test positive- but the results have nothing to do with a baby. "BETA HCG is not normally made up by men, however with testicular cancer there can be a development of cells that are not normally growing or active in normal men. But as those cells become malignant they may revert back to some of the more primitive functions and they may make BETA HCG. So you can certainly have a positive pregnancy test as a man if you have testicular cancer that has HCG," says Dr. Rodriguez. There have been cases, as recent as last fall, when a man discovered his testicular cancer after taking a pregnancy test as a joke. "At the hospital a lot of times if I have a testicular cancer patient I'll order BETA HCG and they'll run a pregnancy test and it will come back positive," says Dr. Rodriguez. While it may be positive indicator, using a pregnancy test as a cancer screener is getting negative reviews. "If you're a man and you're between the ages of 17 and 35 it's probably not a bad idea to do self testicular exams and if you find an abnormality or nodule certainly bring it up with your primary care doctor. Which I think, is a much more effective way of screening for cancer then buying pregnancy tests," says Dr. Rodriguez. Obesity raises risk for incident breast cancer; moreover, high body-mass index (BMI) can worsen prognosis. In a report based on data from nationwide clinical trials of adjuvant treatment for early-stage breast cancer conducted since 1977, Danish investigators assessed obesity's effects on risk for breast cancer recurrence and death among almost 19,000 women who received treatment for early-stage breast cancer between 1977 and 2006. Participants with BMIs 30 kg/m2 were older and had more-advanced disease at diagnosis than those with BMIs less than 25. Cumulative incidence of distant metastasis at 10 years was 24% versus 20% (adjusted hazard ratio, 1.46; P=0.007), and cumulative risk for death from breast cancer at 30 years was 57% versus 46% (AHR, 1.38; P=0.003). However, high BMI was not associated with excess risk for locoregional recurrence. Chemotherapy as well as adjuvant endocrine therapy with tamoxifen or an aromatase inhibitor were less effective after 10 years of follow-up in women with BMIs 30. Comment: These findings lend further support to the conclusion that obesity is an independent risk factor for developing distant metastatic disease and for dying from breast cancer. Editorialists note that obese patients were less likely to have received chemotherapy than were lean women, possibly reflecting concerns that obese patients might experience excess toxicity when such medications are dosed by body weight. High systemic levels of estrogen, insulin, leptin, and proinflammatory molecules also might contribute to poor prognoses in obese patients. Such patients should be informed of obesity's adverse effects on breast cancer outcomes and should be encouraged to adopt lifestyle changes leading to weight loss. Citation(s): Ewertz M et al. To confirm the diagnosis of ovarian cancer, we have to do some scans and blood tests. The first step is to do a transabdominal or transvaginal ultrasound, to look for the ovarian mass and rest of the pelvic structures. CT scan of abdomen and pelvis or PET CT scan may be done if required. Next step is to do blood tests, which will depend upon the sub-type of ovarian cancer, as we have discussed previously. CA-125 is the tumor marker for epithelial ovarian cancer, which is elevated most commonly. But CA-125 testing has some limitations. It may be false negative in some cases, that is negative when there is cancer. This could be seen in subtypes other than epithelial ovarian cancer. Even in some cases of early stage disease, it could be false negative. Also, in the mucinous subtype of epithelial ovarian cancer, it may be negative, and CA 19.9 or CEA may be elevated. CA-125 may also be false positive, that is elevated in the absence of ovarian cancer. Other cancers in which it may be false positive are breast, lung and GI cancer. The non-cancerous conditions in which it may be false positive are PID, endometriosis, pregnancy, cirrhosis, peritonitis, pleuritis and pancreatitis. The tumor marker for germ cell tumor are AFP, beta HCG and LDH. Different sub-types of germ cell tumors have different combinations of these tumor marker elevated. Most commonly elevated in dysgerminoma is LDH, and beta HCG may also be elevated in some cases, but AFP is never elevated in dysgerminoma. In choriocarcinoma, beta HCG is very high and LDH may also be elevated. In endodermal sinus or yolk sack tumors, AFP is high and LDH may also be elevated. Inhibin A and inhibin B are the tumor markers for sex cord stromal tumors, mainly granulosa cell subtype. A germinoma is a type of germ cell tumor, which is not differentiated upon examination. It may be benign or malignant. The term germinoma most often refers to a tumor in the brain that has a histology identical to two other tumors: dysgerminoma in the ovary and seminoma in the testis. Since 1994, MeSH has defined germinoma as "a malignant neoplasm of the germinal tissue of the gonads; mediastinum; or pineal region" and within its scope included both dysgerminoma and seminoma. Collectively, these are the seminomatous or germinomatous tumors. Germinomas are thought to originate from an error of development, when certain primordial germ cells fail to migrate properly. Germinomas lack histologic differentiation, whereas nongerminomatous germ cell tumors display a variety of differentiation. Like other germ cell tumors, germinomas can undergo malignant transformation. The tumor is uniform in appearance, consisting of large, round cells with vesicular nuclei and clear or finely granular cytoplasm that is eosinophilic. On gross examination, the external surface is smooth and bosselated (knobby), and the interior is soft, fleshy and either cream-coloured, gray, pink or tan. Microscopic examination typically reveals uniform cells that resemble primordial germ cells. Typically, the stroma contains lymphocytes and about 20% of patients have sarcoid-like granulomas. Metastasis has been noted in approximately 22% of cases at time of diagnosis. Males are approximately twice as commonly affected in developing germinomas. Germinomas are most commonly diagnosed between the ages of 10 and 21. Often serum and spinal fluid tumor markers of AFP and beta-HCG are tested. Pure germinomas are not associated with these markers. Nongerminomatous germ cell tumors may be associated with increased markers such as AFP with yolk sac tumors as well as embryonic cell carcinomas and immature teratomas and beta-HCG which occur in choriocarcinomas. In 1-15% of germinonas a low level of beta-HCG may be produced. Although controversial, there are some thoughts that HCG-secreting germinomas are more aggressive than nonsecreting ones. Dysgerminoma is the most common type of malignant germ cell ovarian cancer. Dysgerminoma usually occurs in adolescence and early adult life; about 5% occur in pre-pubertal children. Dysgerminoma is extremely rare after age 50. Dysgerminoma occurs in both ovaries in 10% of patients and, in a further 10%, there is microscopic tumor in the other ovary. Seminoma is the second most common testicular cancer; the most common is mixed, which may contain seminoma. Abnormal gonads (due to gonadal dysgenesis and androgen insensitivity syndrome) have a high risk of developing a dysgerminoma. Most dysgerminomas are associated with elevated serum lactic dehydrogenase (LDH), which is sometimes used as a tumor marker. Metastases are most often present in the lymph nodes. Intracranial germinoma occurs in 0.7 per million children. As with other germ cell tumors (GCTs) occurring outside the gonads, the most common location of intracranial germinoma is on or near the midline, often in the pineal or suprasellar areas; in 5-10% of patients with germinoma in either area, the tumor is in both areas. Like other (GCTs), germinomas can occur in other parts of the brain. Within the brain, this tumor is most common in the hypothalamic or hypophyseal regions. No, not necessarily, conditions other than testicular cancer may also be present similarly. For this, we have to further investigate the patient. The next step is testicular ultrasound. It help us to differentiate whether the mass is intra-testicular or extra-testicular, that is, whether it is inside or outside the testis. Then we have to see whether it is solid or cystic. A solid, intratesticular mass goes in the favor of testicular cancer. The next step is to do blood tests, to check for tumor markers. Before discussing the tumor markers in detail, we will first discuss the subtypes of testicular cancer. Testicular tumors may be broadly divided into seminomas and non-seminomas. The non-seminomatous germ cell tumors may be further divided into embryonal carcinoma, endodermal sinus or yolk sac tumor, choriocarcinoma or teratoma. Now, let's discuss the tumor markers for testicular germ cell tumors according to the subtypes. First we will discus about seminoma. In seminoma, LDH is the most commonly elevated tumor marker, and beta-HCG may be elevated in some cases. Next comes choriocarcinoma, in which beta HCG is significantly elevated, and LDH maybe elevated in some cases. In endodermal sinus or yolk sac tumors, AFP is significantly elevated and LDH maybe elevated in some cases. And in embryonal carcinoma all the three, that is, AFP, beta HCG and LDH maybe elevated. So after doing testicular ultrasound and tumor markers, the next step is systemic imaging. This helps us to diagnose the spread of the disease to other part of the body. For systemic imaging, we require the CT scan of abdomen, pelvis and thorax. Very rarely, we may require MRI brain or bone scan to look for the spread of the disease to brain or bones. So after doing testicular ultrasound, tumor markers and system imaging, we are very close to diagnosis, but to be 100% sure, we require a tissue histropathology. So for that, we do high inguinal orchiectomy, in which the involved testis is removed. The procedure is both diagnostic as well as therapeutic because it provides tissue for histopathological diagnosis, as well as it removes the involved testis. A pathology report after high inguinal orchiectomy confirms the diagnosis of germ cell tumor. It also tells us whether it is a seminoma or non-seminoma or whether it is a mixed germ cell tumor, having components of both. It also tells about the sub-type of non-seminoma, that is, whether it is embryonal carcinoma, endodermal sinus or yolk sac tumor, choriocarcinoma or teratoma. CancerBro, as you previously told, even the tumor markers can help to differentiate the various sub-types of germ cell tumors. The tumor markers may not br elevated in all the cases, and even if they are elevated they are highly overlapping in different sub-types, so 100% diagnosis is rarely possible with just the tumor markers. Therefor, for the confirmation of the diagnosis of testicular germ cell tumor, we require both, that is histopathology and tumor markers. Rarely, it is possible that histopathology shows seminoma, but AFP is elevated, such cases are related as non-seminoma. So, always remember that elevation of AFP strictly goes in the favor of non-seminoma even if the hispathological diagnosis is suggestive of a seminoma. Once the diagnosis of the testicular germ cell tumor is confirmed, the next step is staging and risk stratification of the disease. Tumor antigens are useful in identifying tumor cells and are potential candidates for use in cancer therapy. Normal proteins in the body are not antigenic because of self-tolerance, a process in which self-reacting cytotoxic T lymphocytes (CTLs) and autoantibody-producing B lymphocytes are culled "centrally" in primary lymphatic tissue (BM) and "peripherally" in secondary lymphatic tissue (mostly thymus for T-cells and spleen/lymph nodes for B cells). Thus any protein that is not exposed to the immune system triggers an immune response. This may include normal proteins that are well sequestered from the immune system, proteins that are normally produced in extremely small quantities, proteins that are normally produced only in certain stages of development, or proteins whose structure is modified due to mutation. Initially tumor antigens were broadly classified into two categories based on their pattern of expression: Tumor-Specific Antigens (TSA), which are present only on tumor cells and not on any other cell and Tumor-Associated Antigens (TAA), which are present on some tumor cells and also some normal cells. In molecular biology, human chorionic gonadotropin (hCG) is a hormone produced by the syncytiotrophoblast, a component of the fertilized egg, after conception. The pituitary analog of hCG, known as luteinizing hormone (LH), is produced in the pituitary gland of males and females of all ages.[1][3] As of December 6, 2011, the United States FDA has prohibited the sale of "homeopathic" and over-the-counter hCG diet products and declared them fraudulent and illegal.[4][5] Human chorionic gonadotropin interacts with the LHCG receptor of the blastocyst and promotes the maintenance of the corpus luteum during the beginning of pregnancy. This allows the corpus luteum to secrete the hormone progesterone during the first trimester. Progesterone enriches the uterus with a thick lining of blood vessels and capillaries so that it can sustain the growing fetus. Due to its highly negative charge, hCG may repel the immune cells of the mother, protecting the fetus during the first trimester. It has also been hypothesized that hCG may be a placental link for the development of local maternal immunotolerance. For example, hCG-treated endometrial cells induce an increase in T cell apoptosis (dissolution of T cells). These results suggest that hCG may be a link in the development of peritrophoblastic immune tolerance, and may facilitate the trophoblast invasion, which is known to expedite fetal development in the endometrium.[10] It has also been suggested that hCG levels are linked to the severity of morning sickness in pregnant women.[11] Because of its similarity to LH, hCG can also be used clinically to induce ovulation in the ovaries as well as testosterone production in the testes. As the most abundant biological source is women who are presently pregnant, some organizations collect urine from pregnant women to extract hCG for use in fertility treatment.[12][13] Human chorionic gonadotropin also plays a role in cellular differentiation/proliferation and may activate apoptosis.[14]
