Basically a care plan is used to communicate with the healthcare team what the plan of care is for a particular client is. It outlines specific aspects of care so that important elements are not missed or neglected. Some people seem to think that they are a waste of time or even try to ignore them, but that is dangerous! Without a written care plan communication of client needs would suffer. With all the different people working in the healthcare setting something would almost certainly be missed and there could be legal consequences. The amount of detail you put into your care plan will depend on where you are working. In school, for example, we make you put more detail into a care plan than you will see in the hospital because you are still learning the nursing process and how to care for clients. In the work setting different organizations will abbreviate how the care plan is presented but nurses still need to think through the entire nursing process when they are developing care plans. Usually they are presented in a table format. The care planning process parallels the nursing process. In fact, most care plans in school will use the nursing process as the headings in the table you are using for your care plan. They may or may not add in sections for rationales. Before you put anything in writing you need to do an assessment of the client. Familiarize yourself with the client’s previous assessments and records to gather information that will inform you as you think critically about this person’s plan of care. Then make sure you actually assess the person yourself. The type of assessment you complete will depend on where you are working. Your organisation will have protocols for what types of initial and ongoing assessments are needed. If you are in school you may be given a case study to work with instead of an actual person. After assessing the client you will need to formulate nursing diagnoses. Unless you are very experienced and up to date on current best practices you will probably need to do some reading to help you create complete and accurate nursing diagnoses. Diagnostic handbooks can help you formulate nursing diagnoses. I also have a few videos on my channel that can help you with this. Medical-surgical textbooks can help you learn about the client’s disease and typical nursing diagnoses as well as care measures that you can include later in your care plan. In some settings you will see standardised care plans. These care plans are used as a starting point for common situations that you will encounter on a particular unit or in a certain setting. In order to be most effective these care plans should be individualised. If you are starting from a standardised care plan now is where you would select one, but keep in mind it is just a starting point. Be sure to read whatever information accompanies it as it may help to guide some of your decision making. If you are starting with a blank table now is when you would want to fill in the information you have collected so far including the client’s name, medical diagnoses and your assessment data. Typically instructors will ask you to separate subjective and objective assessment data. Remember that Subjective information is Reported but not objectively measureable. You cannot see it. It includes verbal descriptions of health. Subjective data are often referred to as symptoms and represent things a client tells you about that you cannot observe or measure. Most subjective data will be collected during a health history. Objective Data on the other hand is measureable. Think about things like temperature, blood pressure, and weight. Objective data are often referred to as signs. These signs can be seen heard, felt, or smelled and are often collected during a physical exam. If you are using a standardised care plan make sure you modify what they have so everything in it applies to your client. Also be careful that you have included everything that you need to in it.

A common question we hear in our customer care center is, "What happens to my current health insurance plan when 2014 arrives?" That would depend on when the current plan first went into effect, what we call the effective date of your plan. If your plan has an effective date before March 23, 2010, you may have what is called a "grandfathered" health insurance plan, and you may have the option to stay on that plan or change to one of the new metallic plans. But your plan will lose its grandfathered status if your insurance company makes significant changes to your plan that reduces benefits or increases costs. If your plan has an effective date of between March 24, 2010 and January 1, 2014, it's a non-grandfathered plan, and it may have to be converted to a new metallic plan in 2014. Even if your plan's renewal date is later in the year, it may need to be converted to a new metallic plan by no later than March 31, 2014 or earlier. But in some instances, your insurer may move you to a new plan as early as January 1, 2014. If your plan is purchased with an effective date of coverage that is after January 1, 2014, that plan will need to have one of the new metallic benefit levels.

On March 23, 2010, President Obama signed into law the Health Care Reform. And, of course, it did many, many things. And one thing it did is it drew a line on the calendar that basically said that any plans that are in force as of March 23, 2010 are considered to be Grandfathered Plans. New plans after that date, for the most part, are considered Non-Grandfathered Plans. What's the difference? The Grandfathered Plans, because they were in force before the Health Care Reform, do not have to abide by the Health Care Reform, whereas the Non-Grandfathered Plans do have to abide by the Health Care Reform. And all of the Non-Grandfathered Plans that are currently in force as of this date in October of 2013, they are either being terminated or they are being migrated into new Health Care Compliant Plans effective January 01, 2014. So, people who have Non-Grandfathered Plans, they have no choice, they have to leave their plan and move into a new plan effective the first of the year. People who have Grandfathered Plans: you have a choice. You have the choice of keeping your plan or you have the choice of switching to any open plan effective January 01. 2014. So you have a choice. What is the value of having Grandfathered Plan? Well, it doesn't have to abide by the Health Care Reform so many, if not most, of the Grandfathered Plans do not cover preventative care at 100%. Some don't cover preventative care at all, so that's a downside. There are some downsides to the Grandfathered Plans where, because they don't have to abide by the Health Care Reform they may not have as much or as rich coverage as the new plans. However, because they don't have to abide by the Health Care Reform, going forward into the future, they may end up a better value because they may just be less expensive going forward. And all of the new plans, as you're probably aware, are now Guarantee Issue. Anybody can apply to any plan, there are no health questions, so anybody, no matter what their health situation, can enroll on any open plan now and we anticipate that going forward will drive rates and maybe drive rates higher. We might not see it immediately, but going into the future that may be a reality in this new Health Care Reform environment, and being on a Grandfathered Plan, where no new people, for the most part, can be added onto Grandfathered Plans or enroll on Grandfathered Plans, you're insulated on that. So there's a lot of reasons to keep a Grandfathered Plan because of that potential value in the future. All this is theory. We don't know for sure this is not exactly what is going to happen, but in my opinion, it makes sense that there's going to be value in hanging on to a Grandfathered Plan, for most people. Some people, people who may be eligible for a subsidy from the government to help pay for their health insurance they may find that it's better to go on a new plan through the state's marketplace, Covered California. This is the Health Insurance Exchange you've probably been hearing a lot about. This is where you apply for new health insurance and this is where you will be able to obtain subsidy from the government to help pay for your health insurance. If you felt that you were eligible for a subsidy from the government maybe it might be worthwhile to replace your grandfathered plan with a plan that you are getting subsidized by the government and basically only paying a portion out of your pocket for that. It's a little complex question that's not cut and dry for everybody. We're here to help you with that. In order to enroll in a new plan, you have to apply by December 15th for a January 01st effective date, whether it's through the state's exchange, Covered California, or if it's through any outside health insurance source outside Covered California. Again, we can help you with obtaining the insurance through Covered California or outside of Covered California or just discussing keeping your Grandfathered Plan. Please keep in touch. We don't have all of the rates and plans yet because they have not been approved by the Department of Insurance as of the beginning of October 2013. Jarvis The Affordable Care Act (ACA) has brought transformational changes to the healthcare system, including, in some ACA programs, movement away from a pay-for-volume system to pay-for-performance or outcome. Three programs exemplify this approach: readmission penalties, no payment for selected hospital-acquired conditions (HACs), and value-based purchasing. To date, the HAC nonpayment program has targeted prevention of central-line-associated bloodstream infections, catheter-associated urinary tract infections, selected surgical site infections, and methicillin-resistant Staphylococcus aureus (MRSA) or Clostridium difficile infections. With better understanding, improved procedural practices, and closer monitoring, more of these infections are proving preventable; infection rates, including for MRSA, have dramatically decreased. When to sign up for Medicare - Bonus Information Bonus # 1 - Automatic Medicare Enrollment Most people who receive Social Security benefits, before turning 65, are automatically enrolled in Medicare. In most cases, you'll automatically get Part A and Part B starting the first day of the month you turn 65 or the month prior if your birthday is the 1st day of the month. If you receive your Medicare card but don't need (or want) Part B because you’re still covered under a group employer health plan, then you can return the card to Medicare. It lasts for 6 months from your Part B effective date. Once the Medigap Enrollment period ends, in most cases, you’ll have to medically qualify for a Medigap plan. The Affordable Care Act (ACA) or Obamacare, includes numerous provisions that took effect between 2010 and 2020. Policies issued before 2010 are exempted by a grandfather clause from many of the changes to insurance standards, but they were affected by other provisions. Significant reforms, most of which took effect on January 1, 2014, include: Guaranteed issue prohibits insurers from denying coverage to individuals due to pre-existing conditions, and a partial community rating requires insurers to offer the same premium price to all applicants of the same age and geographical location without regard to gender or most pre-existing conditions (excluding tobacco use). Minimum standards for health insurance policies are established. An individual mandate requires all individuals not covered by an employer sponsored health plan, Medicaid, Medicare or other public insurance programs (such as Tricare) to secure an approved private-insurance policy or pay a penalty, unless the applicable individual has a financial hardship or is a member of a recognized religious sect exempted by the Internal Revenue Service. The law includes subsidies to help people with low incomes comply with the mandate. Health insurance exchanges operate as a new avenue by which individuals and small businesses in every state can compare policies and buy insurance (with a government subsidy if eligible). In the first year of operation, open enrollment on the exchanges ran from October 1, 2013 to March 31, 2014. The original purchase deadline date to be covered for January 1, 2014 was December 15, 2013, but the deadline was pushed back, first to December 23, 2013 and later to December 24, 2013. For plans starting in 2015, the proposed enrollment period is November 15, 2014 – February 15, 2015. Low-income individuals and families whose incomes are between 100% and 400% of the federal poverty level will receive federal subsidies on a sliding scale if they purchase insurance via an exchange. Those from 133% to 150% of the poverty level will be subsidized such that their premium costs will be 3% to 4% of income. In 2013, the subsidy would apply for incomes up to $45,960 for an individual or $94,200 for a family of four; consumers can choose to receive their tax credits in advance, and the exchange will send the money directly to the insurer every month. Small businesses will be eligible for subsidies. Medicaid eligibility expanded to include individuals and families with incomes up to 133% of the federal poverty level, including adults without disabilities and without dependent children. The law also provides for a 5% "income disregard", making the effective income eligibility limit for Medicaid 138% of the poverty level. Furthermore, the State Children's Health Insurance Program (CHIP) enrollment process is simplified. However, in National Federation of Independent Business v. Sebelius, the Supreme Court ruled that states may opt out of the Medicaid expansion, and several have done so. Reforms to the Medicare payment system are meant to promote greater efficiency in the healthcare delivery system by restructuring Medicare reimbursements from fee-for-service to bundled payments. Under the new payment system, a single payment is paid to a hospital and a physician group for a defined episode of care (such as a hip replacement) rather than individual payments to individual service providers. In addition, the Medicare Part D coverage gap (commonly called the "donut hole") will shrink incrementally, closing completely by January 1, 2020. Businesses which employ 50 or more people but do not offer health insurance to their full-time employees will pay a tax penalty if the government has subsidized a full-time employee's healthcare through tax deductions or other means. This is commonly known as the employer mandate. MassHealth, the Massachusetts Medicaid program, is undergoing a significant transformation intended to generate payment and care delivery innovations. Currently, MassHealth Members are being notified of these changes and any changes to their PCP and health plan. This webinar, presented by ABH’s Vice President for Mental Health, Lydia Conley, will provide an overview of MassHealth Systems Transformation as it relates to the MassHealth managed care structure, including Accountable Care Organizations (ACOs), Managed Care Organizations (MCOs), and the Primary Care Clinician Plan (PCC Plan). This has been a tumultuous time during this initial Open Enrollment Period. By one carrier's account, last year, in 2013, they had approximately 5,000 individual applications and, right now, during the Open Enrollment Period, they've had over 200,000 applications. With this tremendous volume of business, the insurance carriers have been overwhelmed and are having a difficult time enrolling people efficiently and accurately, as well as making changes for subscribers, billing appropriately, and just simply doing normal service issues for everyone. We've seen the wait times for assistance on their dedicated service lines balloon to as long as two hours or more. Obviously, this is very, very frustrating for everybody. What does this mean to you? I think fist, you need to be alert. You need to be alert with all the materials you get in the mail, communications you get in the mail from your carrier to make sure that everything you receive is accurate. And, if it's not, then you have to move to make sure it is accurate and make sure the corrections are made. Also, I think it's very, very important to find that well of patience within you, because everybody is overwhelmed with this tremendous workload and no one is able to do the work that they normally do as effectively as they had in the past so we just all have to kind of work together, and just be patient. It's easy to say, it is harder to do. The other thing that's very important are the deadlines that are approaching. Here are deadlines for open enrollment this Open Enrollment Period: You must apply by March 15 for an April 1st effective date for your individual insurance. This on exchange, meaning through Covered California, as well as directly through the insurance carrier. If you apply after March 15th, by March 31st, you will get a May 01, 2014 effective date on your health insurance. The last day to apply during this open enrollment period is March 31st, 2014. Unless there is a special qualifying event, there are no new enrollments or plan changes until the next open enrollment period for 2015. The 2015 Open Enrollment Period begins on October 15th, 2014. Please call us at 877.789.5831 Hello, this is George Geldin with a California Individual Health Insurance Update and Medicare Update for September 25th, 2015. Sadly, the weather is changing; maybe the last day can wear a Hawaiian shirt and because autumn is here, it also means that Open Enrollments are just around the corner. What's cooking? Well, the first thing most importantly, is for those people who work, are on Covered California, who are getting subsidies: you must give Covered California consent to verify your income and your family size for 2016. If you haven't already given them permission to do so, you must call them or log into your Covered California account and give them permission to do so by September 30th, that's 5 days from now, or you will have to reapply for your subsidy for 2016. Secondly, people who were covered by Assurant plans must replace their coverage in 2016 as Assurant is leaving the California market. These plans remain viable to the end of the year, but they will end on December 31st, 2015 so they must be replaced. Number 3: In LA, Orange and San Diego counties, Anthem Blue Cross will now be offering PPO plans to replace their individual EPO plans. This is a good thing as insurance on these plans will have greater flexibility with providers. If you are currently on one of these plans, you don't have to do anything as you'll be migrated to the closest PPO plan that matches your EPO plan. If you want to switch to a different plan, you'll be able to do so during the Open Enrollment and if you want to switch to one of these plans during the Open Enrollment from another carrier, of course you can do that also. There will also be a couple of new carriers in the marketplace depending on what county you live in. UnitedHealthcare which you know for individual plans, will now be marketed under United Health One and also there will be a new carrier called Oscar which will have some plans. Our office should have all of these new individual options, rates and benefits toward the end of October and we can share that with you when we have that which will give you time to review them for the beginning of the Open Enrollment, Individual Open Enrollment, which begins on November 1st. Now Medicare, those of you who are on Medicare, Medicare Open Enrollment begins on October 15th. This Open Enrollment is for people who are switching between or enrolling in Medicare Advantage Plans or for people who are enrolling in or switching between Medicare Part D Prescription Plans. It is not for people who are enrolling in or switching between Medicare Supplement Plans. I think that's about it, but let me review: so Medicare Open Enrollment (this is my props) Medicare Open Enrollment is October 15th to December 7th for effective dates for January 1st 2016. The Open Enrollment in Covered California, and direct with the carriers, starts November 1st and will extend to January 31st, 2016. To obtain a January 1st, 2016 Effective Date on an Individual Plan you must apply between November 1st and December 15th to get that January 1st Effective Date. Also if you're on Covered California and you're getting a subsidy: to give Covered California consent to verify your income and family size.
