You know, yesterday I was like a million bucks. You know? But today I just stumbled across one of those online net worth calculators. I’m asked to enter my liabilities, home mortgage, car loan, credit card debt, student loans. And then next, what are your assets? Like the value of my home. Jewelry or collectibles. Cash, mutual funds, stocks, bonds, and after the immensely complicated calculation of subtracting my liabilities from my assets, I magically discover I am worth about $2 and a bus pass. Funny thing though, the only solace that really made a dent in nursing my worthlessness with a meager offering of encouragement was the old adage, “I still got my health.” Now maybe your net worth is not the best test of your self worth. Perhaps the more critical query is, what is your net health? Tell us a little bit about your assets. Bonds. Rate the strength of your connection with others. Collectibles. Are you living a life of memories worth saving? Vehicle. How powerful or painful is your mobility? Life insurance. What is the total value of your contribution to humanity? And, of course, retirement. How much good will have you paid into your IRA? And can’t forget our liabilities. Student loans, alright? How much has it cost you to learn the hard lessons about stressing? Oh, credit card debit. Are you still really just paying the minimum balance due to your behavior? Personal debt. How much do you owe it to yourself still to get fixed? Your net health score will not be a number. It will be many numbers. Cholesterol levels to reach, numbers of hours to sleep, amount of veggies to eat, a number of relaxation techniques. It will be fitness suggestions, and most importantly, a personalized navigation to get behavior from location to destination. And that map, the map that tells you what to do, that is the critical component to the assessment. Our odds for success that changing our unhealthy behavior are only as good as our directions. To simply approach change as though it is difficult, it only scratches the surface. Change is emotional. Change is emotional. And our ability to override our emotional resistance to change depends on our intrinsic motivation to give a damn. The irritating know-it-all in my mirror is right, I do know better, right? I know what medical research says about my unhealthy behaviors. I know how it feels when I make unhealthy decisions. And I know that I’m supposed to do the right thing, and I finally learned what the right thing is to do, and I finally give a damn enough to get it done. Stop forgetting your pills. You know better. Stop postponing treatment. You know better. Stop being afraid to change. You know better. Stop staying you’ll start tomorrow. You know better. To know what to do, you must have the right answers. To have the right answers, you must have the right questions, the right plan, and the right reasons to help change your direction. So grab those reigns of responsibility for navigating your path. Embrace your power to enhance the luster of your laugh. Accept your duty as sovereign leader of your wellness. Of your vision. Your net health potential is limitless. This is the VOA Special English Economics Report, from http://voaspecialenglish.com The United States has a new health care reform law. The main goal is to insure about thirty-two million additional people. That is about ninety-five percent of Americans who are not already covered by Medicare, the government insurance program for older people. About sixteen million people will be added to Medicaid, the government health plan for the poor. The law will require Americans to have health insurance, with a few exceptions, or pay a fine starting in four years. Also, in four years, employers with more than fifty workers will have to offer their employees a health insurance plan. Employers will pay a fine for each uninsured worker. Smaller businesses will receive tax credits to help pay for health plans. People not covered by employer plans, Medicaid or Medicare could buy health insurance in marketplaces called exchanges. The idea is that competition among plans will drive down costs. States will provide these exchanges by twenty fourteen. The law is the biggest change in American health care since nineteen sixty-five. But it is not a government operated health care system like the ones in other countries. President Obama said it provides limited reform. He said: So this is not radical reform. But it is major reform. This legislation will not fix everything that ails our health care system. But it moves us decisively in the right direction. Barry Arbuckle is chief executive of the non-profit MemorialCare Health System in Fountain Valley, California. He says the new law gets the issue of health reform moving. But he notes it is mainly health insurance reform. He says lawmakers will have to reform the way health care is provided. That means finding ways for hospitals, doctors and other providers to work together more effectively. Mister Arbuckle also would like to see more attention on prevention so fewer people need costly medical treatment. The law is expected to cost about nine hundred forty billion dollars over ten years. However, the Obama administration says the plan will cut the nations budget deficit by more than one hundred billion dollars during that period. Last year, the United States spent two and a half trillion dollars on health care. This was an increase of almost six percent from the year before. And thats the VOA Special English Economics Report. Obstructive Sleep Apnoea (OSA) occurs when narrowing of the throat during sleep causes partial or total blockages that reduce airflow, preventing sufficient oxygen from reaching the brain. OSA causes tiredness and exhaustion, which can be severe enough to make activities such as driving unsafe. Existing methods of diagnosis are relatively impractical for the large number of undiagnosed OSA sufferers, as they involve overnight stays in a sleep clinic. This project sought to produce and evaluate an accessible, low cost method for self-recording and remotely diagnosing OSA using a smartphone microphone, wireless accelerometer, and wireless pulse oximeter. Over the course of five weeks, a smartphone application for data collection using these methods was developed and a small group of healthy and apnoeic subjects sampled. Using this data and the output of a snoring classifier developed previously within the University of Sheffield Department of Computer Science, experiments in training decision tree classifiers to predict whether a subject had OSA were conducted. Candidates for data collection were limited, resulting in insufficient evidence from which to thoroughly evaluate the effectiveness of combined audio and pulse oximetry against these methods as used individually. However, the development of these techniques and surrounding classification framework provide a valuable system through which more data can be collected in the future. Future work could continue research on incorporating motion sensors for more robust classification of apnoeas, as well as experimenting with using heart rate data to determine sleep stage in order to provide more accurate classification. I have a bad history with this medication, it doesn't fit with my personal values, and I'm scared. But a lot of you showed me support, love and encouragement when I talked about it, so I feel more confident in progressing with my treatment. I know a few of you also said "Please don't!!" and honestly - I'd rather not take it. I have put this off a long time because I'd rather not take such a heavy, controversial medication. But the way I see it is this: My acne may subside and become less over time, but it may never fully go away. If this drug makes it go away for good, then I want to at least give it a go. I'm going to be taking notes of my progress, and talking openly about my emotions and side effects on my channel while taking it. If I have a bad episode, I'll know, and I will act. I will probably update my blog next week about my specific treatment plan and what skincare and makeup I'm using to combat my newly dried out skin. I know this can be hard and I put this as number 4 because some of those other items prior to this one can help get this back in order too. Start by creating a night time routine for yourself. It's amazing how adaptive we are, and we can use it to our advantage when it comes to getting enough sleep. 5. Take your medication! I hear all the time how someone hated the side effects of a medication or just decided that didn't need it anymore and stopped taking it. I cannot say this enough, but please do NOT go off a medication without talking to your doctor! Please talk to them first because stopping a medication can have harmful side effects (withdrawal for example). If you are taking your medication, try to get used to taking it around the same time each day. That can make sure we are getting a steady amount of the med and decide if it's working for us or not. Depending on what you had for dinner or what snacks you had during the evening. What time you took your medicine can play a factor as well. But if you have consistently high blood glucose readings every morning, it could be one of three reasons that we are going to discuss in this video. DAWN PHENOMENON Researchers feel the most common reason for high blood glucose levels in the morning is the dawn phenomenon. The glucose is going up from sources other than digested food. Some of it is produced by the liver from stored starch and fatty acids. Livers that produce too much glucose are one of the main ways diabetes causes high blood glucose levels. Other organs also produce small amounts of glucose. This is called “gluconeogenesis” for those of you who like the technical stuff. Organs produce glucose to keep blood glucose from going too low at night or other times of not eating. From about 2 AM to 8 AM, most people’s bodies produce hormones, including cortisol, glucagon, and epinephrine. All these hormones increase insulin resistance and tell the liver to make more glucose. The idea is to get you enough glucose to get out of bed and start the day. Everyone has a dawn phenomenon. Otherwise they’d be too weak to get breakfast. But in people without diabetes, insulin levels also increase to handle the extra glucose. People with diabetes can’t increase insulin levels that much, so their early morning blood glucose levels can rise dramatically. Experts disagree on how many people have a dawn phenomenon. Estimates range from 3% to 50% of Type 2s and from 25% to 50% of Type 1s. Is dawn phenomenon a serious problem? It can be serious. According to the American Diabetes Association, “Some people with dawn phenomenon find that their glucose continues to rise until they eat in the morning. For others, levels will settle down a few hours after waking, regardless of whether or not they eat.” According to columnist Wil Dubois, the higher your A1C, the more likely you are to have a significant dawn phenomenon. It could be that spending a number of hours each morning out of control is having a significant effect on your overall control. Some people have high glucose levels in the morning because their medicines wear off overnight. This could be true of medicines like insulin, and metformin. If you are taking any long-acting medicine, consider asking your doctor about changing meds, doses, or times THE SOMOGYI EFFECT In some cases, medicine can be too strong. If your glucose goes too low in the night, you could have a rebound high in the morning. This is called the Somogyi effect. If you are waking up high and are suffering pounding headaches, or find your sheets sweat-soaked, the odds are you are having lows in your sleep…You need to visit with your doctor about taking less meds. According to Dubois, the new insulins are much less likely to cause a Somogyi reaction. But because of cost, people are going back to NPH insulin. NPH is cheaper, shorter-acting, and more likely to cause a low, leading to a rebound high in the morning. WANING INSULIN If you take insulin and have been experiencing high blood sugar in the morning, your insulin may simply be wearing off too soon. If this is the case, your doctor can adjust your dosage or change what time you are taking the insulin to prevent high glucose levels. Pinpointing the Cause for Effective Treatment If your blood sugar is fairly even when you go to bed and at 3 a.m. but is higher in the morning, you are probably experiencing dawn phenomenon. If your blood sugar is low at 3 a.m., but high in the morning, you probably suffer from the Somogyi effect. If your blood sugar is elevated at 3 a.m. and then higher still in the morning, you probably have waning insulin. Even if you’ve identified the reason behind your high morning number, never attempt to correct it on your own. Instead, talk with your doctor. Together, you can find a treatment plan that gets you back on track in the morning. How can this situation be corrected? Once you and your doctor determine how your blood sugar levels are behaving at night, he or she can advise you about the changes you need to make to better control them. -Changing the time you take the long-acting insulin in the evening so that its peak action occurs when your blood sugars start rising -Changing the type of insulin you take in the evening -Taking extra insulin overnight -Eating a lighter breakfast -Increasing your morning dose of insulin -Switching to an insulin pump, which can be programmed to release additional insulin in the morning
