What exactly makes these two so dependent on each other? If you dig through a little bit of fat loss research you'll see right away that insulin is very closely connected with burning fat and losing weight. In fact without reducing insulin there is almost no way we can burn a substantial amount of fat no matter how hard we exercise and diet. And even though most people have heard about insulin spikes not being good for weight loss the average person doesn't truly understand why. Well my hope is that today I can explain this in a way that anybody can understand. It all starts with you eating food. When you eat food the chewed up food particles get broken down further in your stomach into very small parts and then they pass through your intestines at which point your body absorbs and breaks down those nutrients and calories again and dumps them into your bloodstream. These broken down nutrients can't just jump into the cells that need them for their daily functions without some help. That's where insulin comes in. Insulin is a hormone secreted by your pancreas and it gets released into your bloodstream when your body senses that blood sugar has increased. Very high blood sugar levels are not good for you and your body knows this. So insulin acts as a bridge between your cells and the sugar found floating around in your blood. Without insulin your blood sugar would be very high because your body wouldn't be able to get that sugar into the cells. This is actually pretty much what type 2 diabetes is. When people are insulin resistant which means that insulin isn't doing its job they usually wind up being labeled as a type 2 diabetic which means that they need an external source of insulin for example either a shot or a pill to get their blood sugar down. So what does all of this have to do with fat loss and weight loss. Well whether you eat pasta brown rice or drink a can of soda all of it will eventually be broken down and it will be blood sugar floating around in your bloodstream. The only difference is that whole high-fiber carbs like brown rice will take a lot longer to digest which means that the sugar will be released into your bloodstream slowly while the can of soda will pretty much be released all at once. When it's released into your bloodstream slowly you give your body a chance to actually use it overtime so you won't require quite as much insulin which leads to less of it being stored as fat. Before insulin starts storing blood glucose into fat cells it will first store as much of it as possible into your liver for later use. The liver can only hold a limited amount of blood sugar and then the excess starts to get dumped off into your muscle cells. Now your muscle cells can hold more of this glucose then your liver can however it also has its limit. By building more muscle you're going to be able to increase this limit and use more of the blood sugar for your muscles rather than storing it as fat, however again even the most muscular person on the planet is going to have a limit of how much of this blood glucose can actually be used by the muscles. Once that limit is reached we hit a point called spillover where the sugar that came from mostly carbohydrates in your diet will begin to be stored in fat cells in order to lower the sugar found in your blood stream to normal levels. So insulin is an anabolic hormone that causes weight gain and growth. The opposite hormone to insulin is a catabolic hormone known as glucagon which is released when blood sugar drops too low. Glucagon just like insulin goes in and acts as a bridge between the stored energy in your cells and your bloodstream. Except it has the inverse effect. So it pulls energy from your cells and dumps it into the bloodstream. Before your body will start pulling fat from fat cells for energy it will first pull that stored glucose from your liver and your muscle cells. This is why so many low carb diets work. When you don't have that many carbs it's easy for your body to use up the carbs stored in your muscles and your liver and then proceed straight to burning fat for energy. This is the same reason why ketogenic diets work. Except an added benefit to ketogenic diets is that your body becomes more efficient at using fat for energy which means more fat will burnt.

Leptin and the neural circuit regulation food intake and glucose metabolism Air date: Wednesday, January 11, 2017, 3:00:00 PM Category: WALS - Wednesday Afternoon Lectures Runtime: 00:57:39 Description: NIH Director’s Wednesday Afternoon Lecture Series The discovery of leptin has led to the elucidation of a robust physiologic system that maintains fat stores at a relatively constant level. Leptin is a peptide hormone secreted by adipose tissue in proportion to its mass. This hormone circulates in blood and acts on the hypothalamus to regulate food intake and energy expenditure. When fat mass falls, plasma leptin levels fall stimulating appetite and suppressing energy expenditure until fat mass is restored. When fat mass increases, leptin levels increase, suppressing appetite until weight is lost. By such a mechanism, total energy stores are stably maintained within a relatively narrow range. Recessive mutations in the leptin gene are associated with massive obesity in mice and some humans. Treatment with recombinant leptin markedly reduces food intake and body weight. The low leptin levels in patients with leptin mutations are also associated with multiple abnormalities including infertility, diabetes, and immune abnormalities all of which are corrected by leptin treatment. These findings have established important links between energy stores and many other physiologic systems and led to the use of leptin as a treatment for an increasing number of other human conditions including a subset of obesity, some forms of diabetes including lipodystrophy and hypothalamic amenorrhea, the cessation of menstruation seen in extremely thin women. Identification of a physiologic system that controls energy balance establishes a biologic basis for obesity. Recent studies have explored the relationship between leptin and the reward value of food. In addition, new methods for identifying neurons activated by leptin and other stimuli have been developed as have methods for noninvasively activating cells using radio waves. These new approaches are being applied to studies of the neural processes that control feeding, a complex motivational behavior. This video will guide you through how to diagnose and treat both Leptin Resistance and Thyroid Resistance. Both of these conditions go hand in hand so its helpful to look for both if you suspect you have one or the other. Symptoms of Leptin resistance: - Weight loss resistance or the inability to lose weight - Ravenous appetite even after a large meal - Slow metabolism - Cold hands/feet/extremities - Crushing fatigue - Symptoms of hypothyroidism despite being on medication - Constant weight gain Thyroid resistance is defined as low free T3 relative to Reverse T3. In order to test for this condition you need to order both tests and look at the ratio between the two. The higher your reverse T3 the harder it is for free T3 to get into your cells which leads to thyroid hormone resistance at the cellular level. Treatment usually requires T3 medication in conjunction with the 8 tips below. 8 Steps to Reverse Leptin Resistance and Thyroid Resistance: 1. Get your thyroid evaluated and get on the right type and dose of thyroid medication - For most patients this means NDT or T3 containing medications like Cytomel or Liothyronine. 2. While this principle is true to an extent, there are a number of increasingly common hormonal shifts that can alter this relationship. Insulin resistance, the clinical condition that precedes type 2 diabetes, is one such diagnosis. Individuals with insulin resistance will struggle to lose weight via traditional weight loss prescriptions simply because their body is not burning fuel the way it should be. Insulin is a hormone secreted by the pancreas and used to digest carbohydrates. Carbohydrates are found in plant-based foods including bread, rice, breakfast cereal, pasta, fruits and sugars. When carbohydrate-rich foods are consumed, insulin is secreted by the pancreas to take glucose from the food to the muscles for energy. For a number of reasons, over time insulin may fail to work as well as it should. Weight gain, where fat clogs the cells, is one reason, as is a lack of physical activity. Genes can also predispose a person to insulin resistance and type 2 diabetes. The highly processed nature of our daily carbohydrate food choices, including breads, breakfast cereal and snack foods, which require much higher amounts of insulin than less processed, low-GI carbohydrates, is also thought to be a significant contributing factor to the increased incidence of insulin resistance. Resistance to insulin builds up over time, with the body gradually producing more and more insulin in an attempt to get it to work better at taking glucose to the body's cells for energy. As insulin is also a fat-storing hormone, the more of it that circulates in the body, the harder it becomes to burn body fat. High levels of insulin can also make you feel tired, bloated and crave sugar. Individuals with insulin resistance also tend to have distinct abdominal fat deposits, and carry much of their weight around their belly. However, once diagnosed, managing insulin resistance can prevent the development of type 2 diabetes. While some cases warrant medication, the diet and exercise prescription does not change. Individuals with insulin resistance need a reduced-carbohydrate, increased-protein diet developed by a specialist dietitian, as well as a training program that integrates high-intensity cardio sessions with a light resistance training program. Individuals with insulin resistance need to become fussy with their choice of carbohydrates. High-GI, refined sources of carbohydrates, including juice, white bread and refined cereals, need to be completely eliminated from the diet. Signs that you may have a degree of insulin resistance that may be worth investigating with your doctor include an inability to lose weight, distinct abdominal fat, feeling unusually fatigued, bloating and craving sugar. The benefits of identifying insulin resistance early, and committing to a six- to 12-month diet and exercise intervention, will ultimately help you avoid contracting diabetes.

Are you having difficulty losing weight? This case study walk through shows you an example of a hypothyroid patient who was found to have multiple hormone imbalances that were leading to weight loss resistance. After treating her multiple hormone imbalances like thyroid resistance, insulin resistance and low testosterone she was finally able to lose almost 40 pounds over 3 months. This is her story: By the time she came to see me she was already on natural desiccated thyroid but still having symptoms. Despite that her TSH was still elevated. She was unable to lose weight, she had lots of fatigue and she was dealing with insomnia. She had tried everything to lose weight including a lap band surgery and was able to lose some weight initially but gained it all back over time. After evaluating her lab work she was found to have: 1. Hypothyroidism with hashimoto's 2. Insulin resistance 3. Active inflammation with a component of autoimmunity 4. Low testosterone After undergoing treatment with Victoza, Testosterone, WP thyroid hormone and LDN she was able to lose a significant amount of weight over 3 months. Insulin is secreted from the Islets Cells of Langerhand specifically from the beta cells. Glucose enters the beta cell via the GLUT2 and is activated by glucokinase. This increases ATP which deactivates KATP pump. This leads to an influx of calcium and release of vesicles with insulin. OSU and repaglinide inhibit the same KATP channel. Insulin is synthesized in Rough Endoplasmic Reitculum. Preproinsulin has a c,b,a pre and post section. When pre and post is removed then it is known as pro insulin. Pro insulin moves on to Golgi Apparatus where the c peptide curve. a peptide and b peptide forms bond and the c peptide is removed PATHOPHYSIOLOGY OF DIABETES MELLITUS TYPE 2 Genetic causes of diabetes mellitus focuses on MODY1, MODY3, MODY5 associated with HNF. MODY2 is associated iwth glucokinase mutation and MODY4 is associated with INsulin promotor factor (IPF-1). Environmental influences includes obesity which causes releases of adipokinse such as leptin, adiponetin, TNF, resistin. SYMPTOMS OF DIABETES MELLITUS TYPE 2 Diabetes is often times asymptomatic. Most common presentation is polyuria, polydypsia, and weight loss. Other patients with Diabetes Type 2 present with blurred vision, vulvovaginits because diabetes is an immunosuppressed state. Often times it may present Hyperosmolar Hyperglycemic state associated with severe glucose level and hyperosmolality. Diabetes type 2 may also be associated with Diabetic Ketoacidosis. DIAGNOSTIC CRITERIA FOR DIABETES MELLITUS TYPE 2 The American Diabetes Association has four factor for diagnosing diabetes. HbA1c is greater than 6.5 percent is diagnosed as having diabetes. If the fast plama glucose is greater than 125 then the patient is diagnosed as having diabetes. OGTT includes giving 75g of glucose and then checking blood sugar. If patient has greater than 200mgdL than patient is diagnosed as having diabetes. If random plasma glucose is greater than 200 and the patient is symptomatic than that is enough to diagnose the patient was diabetic. SCREENING PATIENTS FOR DIABETES MELLITUS TYPE 2 Screen patient for diabetes if patient is greater than 45 years old, sedentary lifestyle, primary family history, high LDL, Low HDL, signs of insulin resistance and certain ethnicities. Screen children for diabetes if they are greater than 10 years old and overweight, family history, gestational diabetes of mother and signs of hyperinsulinemia. MANGEMENT OF DIABETES MELLITUS TYPE 2 Primary focus of management is control of blood glucose. Initial therapy focuses on Diet, Exercise and Metformin. If after three months the HbA1c is greater than 7 than add another drug to control the diabetes. If after three months the HbA1c is still not below 7, than add another drug for triple therapy. The current diet for diabetics is medical nutrition therapy (MNT). This individualized diets has been seen to be the most effective for diabetic patients. Diabetic children should get at least 60 minutes a day, while adults should have 150 minutes a week. Diabetic patients should be screened for complications of diabetes. Nephropathy check for albuminuria and start treatment for ACE inhibitor if positive. Diabetic Retinopathy can be check with routine ophthalmologic exams. Neuropathy is also common in diabetic patients. Check for sensation of arms and legs. Foot Exam is also important and should be done in every visit. Cardovascular assessment is also important in diabetic patients because they are at risk. Check for blood pressure, dyslipidemia and consider aspirin
