Complete Excision of A sebaceous cyst on the scalp by Halim Grace. 1. Local anesthesia infiltration surrounding the cyst and beneath the cyst but not in the cyst . Lidocaine 2% with adrenaline is preferred as the scalp is very vascular . Sedation was by a tiny dose Of fentanyl. 2. A linear incision near the punctum was done with a number 15 scalpel. Although an elliptical incision enclosing the punctum is the most popular method. 3. The skin edges were raised by allis forceps ( skin hooks are less traumatic , but they weren't available) 4. Sharp dissection was done by the scalpel , in a circumferential manner , to achieve an adequate plane for blunt dissection . 5. Blunt dissection was done using curved scissors. Keeping the convex side up , and dissecting the cyst from the skin and surrounding tissue. 6. Small bleeders are cauterized by diathermy . 7. Suturing was done with silk 2-0. Dead space is minimized by taking a deeper bite . Edges of the skin should be everted . 8. Patient is instructed not to get water on the wound , and a follow up visit to replace the dressing was done On the 4th day following the surgery . Sutures are removed after 6 days , as the scalp is very vascular and heals very fast. N.B . If the cyst was inflamed , then I wouldn't have suggested excision until the inflammation subsides. Also if the cyst has been inflamed previously then excision without rupturing the cyst wall is not easy due to adhesions to the surrounding tissues. If an abcess forms then treatment is only incision and drainage , and if possible excising as much as the sac as possible , (anatomy would be distorted) This has an increase in recurrence rate . I dont advocate the minimal Excision technique ; were a tiny incision is done , and the contents of the cyst are squeezed out , then the sac removed . Simply because how will you know that you've excised the whole wall of the cyst. Surgeons that like this technique say that the incision is smaller , and operative time is not as much and stitches aren't required .

Patient postponed from different anaesthetist and surgeons during last year and all them told him about the risk and highly mortality rate when receive GA in prone position with his systemic disease ASA3….so patient came to me and he told me he is suffering from this mass and even the sleep begun difficult and no way just to operate and finish this disasters….he deadly refuse GA and need regional anaesthesia and awake surgery as he know the risk of general anaesthesia….patient scheduled to Tuesday list and the plan now give regional anaesthesia…. Her weight was 52 Kg (around) For LPB = 2% Lignocaine adrenaline 5ml(test dose) plus 15 ml 0.33% Levobupivacaine. For classical sciatic nerve = 0.33% Levobupivacaine 15ml plus 2% Lignocaine adrenaline 5ml=20ml 5ml 1% lignocaine adrenaline for skin infiltration by surgeon. So Total Levobupivacaine = 30ml of 0.33%=99mg Lignocaine with adrenaline=10ml of 2%=200mg Lignocaine with adrenaline=5ml of 1%=50mg Within SAFE limits of LA in 50kg patient.

Warts are viruses that live in the skin and can be very difficult to cure. Removal of warts on the foot can be performed in the office in only a few minutes. In this video, Dr. Leo Krawetz performs an excision of a wart on a patient's foot. Hi, I’m Dr. Leo Krawetz of Healthy Feet Podiatry. Today, I have a patient who has a wart on the bottom of her heel, and it’s actually mosaic wart, which means there is more than a few warts there. The mosaic warts usually grow in clusters and you can see a cluster of warts here, cluster of warts here, and a cluster there. So what I am gonna do today is cut this out and cut a little bit of the area out around the warts basically to kill viral particles. So first, I am gonna numb it up with 1% lidocaine with epinephrine and I’m injecting under the wart area and this gets it real numb real fast. Usually this is a little bit painful but not too bad and the good thing is you don’t hear any screaming right now in the background. The next thing I’m gonna do is cut around the area where the warts are including some of the skin that does not look infected with a wart. There reason I do that is because the viral particles are so small you can’t see with a naked eye. Now, if you have any pain up there you let me know so I am basically circumscribing around the wart and the wart itself or the viral particles only live in the skin. There are 5 layers of skin and the wart lives in the bottom layer of skin in the stratum germinativum that is what it’s called and we are only going down to that layer so I am removing all the stuff around the wart which is good skin and viral infected tissue and you could see this is the chunk of warty particles. This is the inside, this is the stuff which is actually in the foot itself and those are millions of viral particles. Now, in the skin itself you can see this part here is the bottom layer of the skin. As you can see, there is a little of bit of fat coming out which is a little bit deeper than where the wart is actually growing, but at times when you take this out, you get a little bit of the fat which could scar when it is a weightbearing area. So what I’m doing right now is just scraping the base to try to get any remaining particles out of there and then I am gonna cauterize the base with Lumicain so it doesn’t bleed very much afterwards. These tend to be very vascular in nature and they do bleed a lot after the procedure so we put a compression dressing on them. This is just some Lumicain so it doesn’t bleed as much and then we put a dressing on this. It will take approximately four weeks for this to heal and we will follow up in a couple of weeks, and in the meantime, we will just be putting Neosporin in the hole after cleaning this out with peroxide and soap and water. Carpal Tunnel Injection - Everything You Need To Know - Dr. Nabil Ebraheim

Dr. Ebraheim’s educational animated video describes injection of the carpal tunnel - median nerve compression. As the median nerve travels through the carpal tunnel, it may become compressed. Carpal tunnel syndrome is the condition of pressure being placed on the median nerve. The transverse carpal ligament is usually thickened at the wrist which causes compression of the nerve, the condition usually occurs due to overuse injury such as repetitive handgrip movements. Carpal tunnel syndrome predisposing factors: •Trauma •Fractures •Pregnancy •Diabetes •Rheumatoid arthritis •Ganglion cyst. As a result of this compression, the patient may experience pain and paresthesia in the median nerve distribution, as well as weakness in the hand. Steroid injection is often used and may be successful to treat some patients who suffer from carpal tunnel syndrome. The median nerve is located between the palmaris longus and the flexor carpi radialis tendons. Surface anatomy for injection The course of the flexor carpi radialis, palmaris longus and the distal palmar crease should be identified and marked. The injection can be given either blind or with ultrasound guidance. The blind injection can be performed by the usual approach or by the proximal approach. Usual approach: •Mark the intersection of the palmaris longus tendon and the distal palmar crease. •Next go 1 cm proximal and 1 cm ulnar to that site, this will then be the point of injection. •Use a 25 gauge needle with desired steroid and 1 ml of 1 % lidocaine. •Warn the patient before injection that if any feeling of numbness, paresthesia or severe pain exists, to let the physician know about it. If these symptoms exist, the doctor should withdraw the needle or adjust it before injection. •May use local anesthesia or spray. •Use a sterile field. •Put the needle at a 45-degree angle to the skin of the wrist. •Direct the needle towards the base of the thumb and advance the needle distally and slowly. Proximal approach: •Mark the site 4-5 cm proximal to the distal palmar crease and between the palmaris longus and the flexor carpi radialis tendons. •Advance the needle distally towards the wrist at about a 20-degree angle from the skin. •Keep the needle between the two tendons. •Be aware that the nerve is in between the two tendons and adjust the needle as needed. •Inject the desired fluid. Syringomyelia and neuropathic pain: treatment.m4v

lthough pain is a prominent symptom in patients suffering from syringomyelia,and this central neuropathic pain is very difficult to treat. However, more than half of all patients with syringomyelia suffer from symptoms related to central neuropathic pain. Patients often present with complaints of radicular pain and/or central cord pain. Furthermore, nearly half of all patients with syringomyelia experience neuropathic dysesthetic pain, like burning sensation and/or pins and needles. But only small series of patients are available as a base for these figures. In patients suffering from this type of central neuropathic pain, it is often an overwhelming and disabling symptom that over-shadows many of the other complaints, such as decrease in temperature sensation and numbness. Similar central neuropathic pain can be found in other spinal cord pathological conditions such as intramedullary tumors, multiple sclerosis and infarctions. Surgical treatment of syringomyelia resulted in improvement of dysesthetic pain in nearly half of a small series of patients (37), but a significant number of patients (41%) reported no improvement or an intensification of pain despite the operation. Improvement in pain status following operation seems difficult to predict. Postoperative neuropathic pain is disabling and responds poorly to treatment with analgesics, sedatives and antiepileptics. To date, there is no consensus as to the best pharmacological treatment for pain in syringomyelia. No clinical trials have been conducted yet. In our centre we follow the normal steps for treating central neuropathic pain, and mostly prescribe two or more analgesics, such as low dose amitriptyline (10 - 50 mg per day) combined with either gabapentin (1800 - 2400 mg per day) or pregabalin (150 - 600 mg per day)and/or tramadol. However, there are patients non responding to these drugs. One of our patients did not respond to the drugs mentioned above, but did respond favourable to a topcial cream we developed in our centre, based isosorbidedinitrate(ISDN)0.4%, capsaicin 0.075%, and lidocaine 3%. A challenge-rechallenge paradigm was followed. The pain decreased by using this topical cream from score 8 to 5 (11 points Likert scale) and after stopping the cream, the pain was rebouding to score 8 in a few weeks. Starting again using this cream reduced the painscore again, and in a preriod of several weeks the pain decreased for more than 30% compared to baseline. Presented by Jan M. This nice gentleman comes into the clinic with a swollen left cheek. He has a hx of a "cracked" tooth around this area. No complaints of pain to teeth. no fever or chills. Upon exam, I notices the mandibular gum line was fluctuant (soft and bouncy). I numbed the area with q-tips soaked in viscous lidocaine. Then I locally injected lidocaine with epi. I then used an 18g needle to puncture the area twice. When I saw the pus, I realized I may need to open the area, so I used a 10 blade to perform a stab incision.
