Rheumatoid Arthritis is often hard to diagnose, particularly in early stages when symptoms seem like normal aches and pains. There are many diseases for which RA can be mistaken, so how does your doctor accurately diagnose RA? Symptoms of RA include: swelling, redness, or warmth in 3 or more joints. Experiencing pain or stiffness within the first 30 minutes of waking up also is a consideration when diagnosing RA. With RA, joint pain appears to be symmetrical, affecting the same joint in both sides of the body. Talk to your doctor about where specifically you are experiencing pain, stiffness, or swelling, and be prepared for questions on other symptoms, so as to rule out other possible diseases. During a full physical examination, your physician will conduct a gentle squeeze test on your hands and feet to test unusual discomfort. They will also feel for rheumatoid nodules under the skin near affected joints, and may conduct further testing such as MRIs, X-rays, ultrasounds, and draw blood or synovial fluid for lab testing. RA can take up to a few weeks or months to diagnose. Physicians will identify whether you are experiencing benign or aggressive RA to discuss the most appropriate methods of treatment. Learn more at http://www.thomasdelauer.com Inflammation is the body’s response to injury and infections and is essential in healing. This is how the immune system responds and delivers white blood cells and other things essential for getting well. When we have chronic inflammation, however, this becomes a problem. Chronic inflammation can lead to other diseases and can also be a signal of conditions in the body. But how can you know if you have chronic inflammation in the body? It is not always as obvious as bloating, and even when it is, the cause and treatment may not be clear. When a part of your body is inflamed that inflamed tissue will release extra proteins into your blood. These extra proteins can be found in blood tests and are known as inflammation markers. 3 common tests for increased protein: ESR, CRP and PV- ESR (erythrocyte sedimentation rate): A blood sample is taken and treated with a chemical so that the red blood cells will fall away from the clear liquid plasma. The rate at which the red blood cells fall is measured. When certain proteins are present they will cause the red blood cells to stick together and fall more quickly. Thus a high ESR rate shows that there is inflammation somewhere in the body. CRP (C-reactive protein): CRP is a substance that is produced in the liver. More CRP is produced when there is inflammation in the body. Increased CRP is tied to the increase of one specific protein and is thus a bit more specific than an ESR, but still does not reveal what condition specifically is causing the inflammation. PV (plasma viscosity): Similar to ESR, but used specifically for rheumatoid arthritis Evaluation of these tests over time acts as a signal to a doctor, and provides additional information about inflammation. ESR and PV change more slowly than CRP. CRP is affected by fewer factors than PV and ESR, making it a better marker for certain types of inflammation. Raised ESR, CRP and PV are signs of many infections, including: -bacterial infections -some cancers -Crohn’s Disease -rheumatoid arthritis -muscular and connective tissue disorders -infections post-surgery These are non-specific tests and further tests are needed to narrow down the specific cause of the inflammation. Doctors will often recommend these tests twice or more spread out over weeks to months to see how the levels change over time. As CRP changes more quickly it is often used to see how well a treatment is going. Lower CRP signals an improvement. Quantitative Immunoglobulins Test: Another test that is often run to find inflammation markers, is one that measures the levels of the three main immunoglobulins (IgG, IgA, IgM). Both high and low levels are markers of different conditions and diseases. Immunoglobulins are antibodies that are produced during your body’s response and play a role against bacteria, viruses and other microorganisms. This test is run when chronic inflammation is suspected or continuous infections occur. This helps to diagnose and monitor conditions. Used together and with other tests depending on symptoms and results, it is possible to diagnose and measure progress to treatment for many diseases, such as inflammatory bowel disease and rheumatoid arthritis. Understanding Treatment: When an area in the body is inflamed, COX-1 and COX-2 enzymes produce more prostaglandins (PGs), which play a key role in the inflammatory response. When you notice redness, swelling and pain, this is due to the prostaglandin response. Nonsteroidal anti-inflammatory drugs work to block the synthesis of PGs and thus the pain associated with inflammation. Future therapeutic measures for chronic inflammation may look at PG signaling. PGs may play a role in both the formation and resolution of inflammation. Lifestyle changes such as healthy eating and exercise are key in reducing chronic inflammation. References: 1. Unfortunately there is no screening tool for arthritis. Arthritis is a clinical diagnosis wherein people have symptoms and exanimation findings and in them we do testing. The reason being there are some of these tests being done for arthritis like the rheumatoid factor and anti CCP antibodies. These tests can be positive in normal people also. So the tests by themselves do not give the diagnosis. So if is the test used in the context of the symptoms. So somebody if they have symptoms or findings pertaining to arthritis, we recommend these tests. So there is no screening tests for arthritis or no screening tests for arthritis is required.

The test was developed by researchers from the University of Warwick, in England. According to UPI, scientists involved with the project say their new procedure will be able to provide an early diagnosis for osteoarthritis. While RA is notorious for causing joint pain and inflammation, it can also cause other symptoms throughout the body. Read on to learn more about the possible symptoms of RA and its overall effects on the body. The effects of rheumatoid arthritis on the body RA is a progressive autoimmune disease that mainly affects your joints. According to Arthritis Foundation, about 1.5 million U.S. people live with RA. Anyone can get RA, but it generally begins between the ages of 30 and 60. It also tends to affect women nearly three times more than men. The exact cause of RA is unknown, but genetics, infections, or hormonal changes may play a role. Disease-modifying medications can help slow the progression of RA. Other medications, combined with lifestyle changes, can help manage the effects and in turn improve your overall quality of life. Skeletal system One of the first signs of RA is inflammation of the smaller joints in the hands and feet. Most of the time, symptoms affect both sides of the body at once. Common symptoms include pain, swelling, tenderness, and stiffness, which is more pronounced in the morning. Morning RA pain can last for 30 minutes or longer. RA can also cause tingling or burning sensations in the joints. Symptoms can come and go in “flares” followed by a period of remission, but the initial stages can last at least six weeks. Symptoms of RA can occur in any of the body’s joints, including your: fingers wrists shoulders elbows hips knees ankles toes RA can also result in: bunions claw toes hammer toes As the disease progresses, cartilage and bone are damaged and destroyed. Eventually, supporting tendons, ligaments, and muscles weaken. This can lead to a limited range of motion or difficulty moving the joints properly. In the long term, joints can become deformed. Having RA also puts you at greater risk of developing osteoporosis, a weakening of the bones. This in turn can increase your risk of bone fractures and breaks. Chronic inflammation of the wrists can lead to carpal tunnel syndrome, making it difficult to use your wrists and hands. Weakened or damaged bones in the neck or cervical spine can cause chronic pain. Your doctor may order X-rays to investigate the extent of joint and bone damage from RA. Circulatory system RA can affect the system responsible for making and transporting blood throughout your body, too. A simple blood test can reveal the presence of an antibody called the rheumatoid factor. Not all people with the antibody develop RA, but it’s one of many clues doctors use to diagnose this condition. RA increases your risk for anemia. This is due to a decreased production of red blood cells. You may also have a higher risk of blocked or hardened arteries. In rare cases, RA can lead to inflammation of the sac around the heart (pericarditis), the heart muscle (myocarditis), or even congestive heart failure. A rare but serious complication of RA is inflammation of the blood vessels (rheumatoid vasculitis, or RA rash). Inflamed blood vessels weaken and expand or narrow, interfering with blood flow. This can lead to problems with the nerves, skin, heart, and brain. Skin, eyes, and mouth Rheumatoid nodules are hard lumps caused by inflammation that appear under the skin, usually near joints. They can be bothersome, but usually aren’t painful. As many as 4 million U.S. people have an inflammatory disease called Sjogren’s syndrome, according to the Sjogren’s Syndrome Foundation. About half of these individuals also have RA or a similar autoimmune disease. When the two diseases are present, it’s called secondary Sjogren’s syndrome. Sjogren’s causes severe dryness — especially of the eyes. You may notice a burning or gritty feeling. Prolonged dry eyes increases the risk of eye infection or corneal damage. Though it’s rare, RA can also cause inflammation of the eye. Sjogren’s can also cause a dry mouth and throat, making it difficult to eat or swallow, especially dry foods. Chronic dry mouth can lead to: tooth decay gingivitis oral infections You may also experience swollen glands in the face and neck, dry nasal passages, and dry skin. Women may also feel vaginal dryness. Respiratory system RA increases the risk of inflammation or scarring of the linings of the lungs (pleurisy) and damage to lung tissue (rheumatoid lung). Other problems include: blocked airways (bronchiolitis obliterans) fluid in the chest (pleural effusion) high blood pressure in the lungs (pulmonary hypertension) scarring of the lungs (pulmonary fibrosis) rheumatoid nodules on the lungs

A new test to detect early arthritis… next Study Shows Potential for Early Diagnosis of Arthritis Pat Anson writing for Pain News Network reported a new study by British researchers has demonstrated the potential for an experimental blood test that can diagnose arthritis in its earliest stages. Such a test could lead to earlier treatment of osteoarthritis (OA) and rheumatoid arthritis (RA), years before joint damage and physical symptoms begin. Researchers at Warwick Medical School recruited 225 people with early or advanced OA, RA or another inflammatory joint disease, along with a control group of healthy volunteers with no joint problems. Their blood and fluid from affected knee joints were then analyzed with mass spectrometry. The test found patterns in blood plasma amino acids that were damaged by oxygen, nitrogen and sugar molecules. The damage was highest in the blood samples of patients with OA or RA, and markedly lower in the blood of healthy volunteers -- giving researchers identifiable biomarkers that could be used for an early diagnosis. “This is a big step forward for early-stage detection of arthritis that will help start treatment early and prevent painful and debilitating disease,” said Naila Rabbani, PhD, of Warwick Medical School. “Damage to proteins in the arthritic joint have been known for many years but this is the first time it has been exploited for early-stage diagnosis. “For the first time we measured small fragments from damaged proteins that leak from the joint into blood. The combination of changes in oxidized, nitrated and sugar-modified amino acids in blood enabled early stage detection and classification of arthritis – osteoarthritis, rheumatoid arthritis or other self-resolving inflammatory joint disease."
