A new study from the Indiana University School of medicine has found that anticholinergic drugs, or medicines that block the neurotransmitter acetylcholine in the nervous system, could increase the likelihood of cognitive decline in older adults. Common anticholinergic drugs include the heartburn medication Zantac, allergy medications such as Benadryl and Claritin, depression medications like Effexor and anxiety drug Xanax. A list of anticholinergic drugs, along with scores (1 for a possible anticholinergic; 2 and 3 for definite anticholinergic) can be found here. The researchers further confirmed other studies from the past decade that have linked anticholinergic drugs to cognitive impairment and conditions like dementia or cognitive decline that can interfere with daily life. "These findings provide us with a much better understanding of how this class of drugs may act upon the brain in ways that might raise the risk of cognitive impairment and dementia," lead study author Shannon Risacher, an assistant professor of radiology and imaging sciences at Indiana University, said in a press release.

G: there has been quite a bit of media coverage of an article in the February edition of the Journal of the American medical Association internal medicine. It is a study which suggests that a common drug to both of our practices can cause injury to patients’ kidneys. The class of drug is called proton pump inhibitors. Most of you will recognize the names Prilosec and Nexium as popular name brands from this class. Their function is to turn off the acid pump in the stomach to decrease injury from acid inside the digestive tract. These medicines are usually prescribed for patients who have either short-term or long-term overproduction of acid resulting in symptoms ranging from heartburn to stomach ulcers. Now that they are available over-the-counter without a prescription they are used by many patients without any consultation with a physician. M: The article reports on a small study done on data collected during the atherosclerosis risk in communities study which studied over 10 thousand subjects. The results were then replicated by analyzing records of over 240 thousand individuals. They found that patients who used proton pump inhibitor drugs had a significantly higher incidence of chronic kidney disease on a blood test compared to people who did not use these drugs. They also found that there was a significant increase in kidney disease when compared to people who used H2 antagonist drugs. The cutoff they used to delineate between people with and without chronic kidney disease was based on a lab value found on a blood test. There was no requirement that the patients have any symptoms or other noticeable effect on their health to be labeled as having kidney disease. It is expected that the vast majority of these patients will never suffer any significant ill effects apart from this number found on a lab test. G: but that does not give us license to take these drugs willy-nilly. They should still only be used when there is significant benefit over alternative treatments. The simplest is through diet. Many of the problems which respond to this type of medication can be better treated by reducing intake Of fatty foods, alcohol, spicy foods, and acidic foods. Symptoms can also be decreased by eating smaller quantities during meals and not lying down within two hours of eating. If symptoms only occur occasionally, say less than three times per week, occasional use of an over-the-counter antacid can resolve the symptoms. More persistent symptoms may be treated by a doctor using an over the counter H2 antagonist like Zantac. M: without physician supervision, these medicines really should only be taken for a short period, not exceeding two weeks. I will often use these medicines for a 30 day period followed by a recheck. Patients continuing to have symptoms after this treatment are referred to a gastroenterologist for endoscopy. The gastroenterologist will check for serious disorders which can even include cancer of the digestive tract. Most patients do not have a serious disorder and many end up doing best continuing on these medicines long-term. G: we hope this gives you a better understanding of the concerns surrounding the use of proton pump inhibitors and kidney disease. Original release date: July 20, 2015 Show Notes Stomach Acid Basics Stomach acid causes heartburn. Long-term reflux problems leads to a GERD diagnosis. Stomach acid has a pH of 2. Low pH = acid; High pH = base Stomach acid is Hydrochloric acid (HCl) Some Biochemistry The molecules of the acid like to spend their time joining together and breaking apart. So by attaching to something else instead of each other is how it can be dangerous but also how it helps digest food quickly. Your stomach is designed to hold this strong acid safely. There are pumps in the cells of the lining of your stomach that produce the acid. Proton pumps work kind of like a water wheel - they move protons from inside the cell to outside to the stomach cavity. Hydrogen atoms are made up of 1 proton (positive charge) and 1 electron (negative charge). So if you take the electron away from hydrogen, you are left with a proton with a positive charge. Another process that helps create acid in the stomach relies on histamines. They are not quite the same as the histamines that you hear about in relation to allergies. But there is a particular type of histamine that is only in your stomach. Acid-reducing Medications There is a class of medications for reflux called Proton Pump Inhibitors (PPI). These medications turn off the pumps so they pump less protons into the stomach. The medications for the histamine process are called Histamine 2 Receptor Blocker (H2RB). H2RB’s work faster than PPI’s. Just like you can take an antihistamine and it block histamines causing allergies in just a dose or two, H2RB’s can work as fast as one or two doses. That is why they are advertised to treat heartburn after you eat or to help prevent heartburn before you eat. PPI’s take up to 2 weeks to reach maximum effect. The third option for heartburn are your antacids. They are bases that go into your stomach acid and help neutralize it. OTC Medications (by class) Antacids: Tums, Rolaids, Maalox, Mylanta - fastest H2RB’s: Pepcid, Zantac, Tagamet - all have generics, all OTC Tagamet can have drug-drug interactions with other prescription medications, so caution is advised. PPI’s: Prilosec, Prevacid, Nexium - some generic, newly OTC - slowest Cautions The downside to having reflux medications available OTC and people having the opportunity to self-treat is if there are any cellular changes in your esophagus. Your esophagus is not designed to be in contact with that level of acid. As those cells are injured, they eventually change and can become cancer. Fun Tidbit Just like the cells of your skin are epithelial cells and their job is to keep the inside things in and the outside things out. Your digestive tract is also lined with epithelial cells. So technically the food you eat doesn’t go inside your body, it just moves through this tract that is “outside turned in”. Holla @steve_tessler's question: After he eats, he coughs for 30 minutes, and sometimes sneezes. Is this considered GERD? Cannot eat nuts or seeds due to diverticulitis. If he sits and rests it doesn’t get so bad, but if he has to be active right after a meal, it is. Recommendation: try a H2RB morning and night and see if it contains the acid after meals. The next option could be a slight food allergy, possibly gluten, so cutting out certain foods would be necessary. The advice is to go to your GP (who gave you the cocktail in the first place) so that they can review your medication. Now consider the GP's bonus for each drug prescribed and ask yourself if your GP is the right person to do this re-evaluation. Combinations of common prescription drugs used to treat routine illnesses conditions such as heart disease, inflammation, pain, depression and allergies are speeding up dementia and even death in many elderly patients. The report, from Dundee University, analysed the records of almost 140,000 patients who were at risk of suffering bad reactions to drugs. They found about one in seven, more than 19,000 people, had been prescribed drugs which could harm them. Well-known brands of hay fever tablets, painkillers and sleeping pills pose a previously unknown threat to people's health when taken together, British scientists claim. The drugs, including common allergy treatments Piriton and Zantac, as well as Seroxat, an anti-depressant, are thought to be used by half of the 10 million over-65s in Britain. Many of the drugs, when taken in combination, were found to more than treble an elderly patient's chance of dying within two years. Common bladder medications, heart drugs, eye drops and asthma treatments were also among those found to pose a risk. The most dangerous included the antihistamines chlorphenamine (used in the brand Piriton) and promethazine (used in Phenergan), the anti-depressant paroxetine (used in Seroxat) and the incontinence drug oxybutynin (used in Ditropan). The heartburn drug ranitidine (used in Zantac), beta blocker Atenolol, the painkiller codeine and some eye drops were among the drugs in the mildest category. Low-risk drugs were graded one point while high-risk drugs were graded three.
