Vitamin D is important for proper maturation and development of the bone. Vitamin D is naturally found in Tuna, Salmon and Cod liver oil. Vitamin D and K are not present in sufficient quantity in breast milk. Production in the body: Precursor for vitamin D is found in the skin. Cholesterol derived precursor (pro-vitamin D3) is formed in the liver and stored in the skin. UV light converts it to pre-vitamin D3. The liver will then convert it to 25 hydroxyvitamin D3. Then the kidney will convert it to 1,25-dihydroxyvitamin D3, which is the active form of vitamin D3. PTH regulates the active form of vitamin D3. Active vitamin D3 stimulates the absorption of calcium and phosphate from the intestine. Low vitamin D levels lead to rickets. Low vitamin D rickets is the most common form of rickets worldwide. In adults, it leads to osteomalacia. Looser zones are stress fractures in patients with osteomalacia. Darker skin individuals should have more sun exposure. Vitamin D resistant rickets: the most common form of rickets in the United States. Also known as familial hypophosphatemic rickets. It is an X-linked inherited disease (dominant). The defective mineralization of bone in osteomalacia leads to a qualitative defect of bone with a high amount of unmineralized osteoid matrix. Osteoporosis, on the other hand, is a quantitative defect of bone. Vitamin D deficiency is considered when blood levels of 25 hydroxyvitamin D3 fall below 20 ng/ml. vitamin D levels are usually found to be low in patients with low energy fractures. Vitamin D deficiency is found in the majority of patients with endocrine or metabolic problems presenting with fracture non-unions. Dilantin (phenytoin) inhibits vitamin D formation in the liver. Recent research has also demonstrated a link between low vitamin D and vitamin B12 levels and the development of Alzheimer’s disease. Daily requirements of vitamin D are 400-800 international units. Vitamin D and calcium are given for female athletic triad. Vitamin E Vitamin E affects peripheral nerves and is a major antioxidant as it stabilizes free radicals and reduces the potential for oxidative damage. Vitamin E prevents peroxidation of fatty acids in cell membranes, helping to maintain their normal fluidity. Vitamin E also has some enzymatic activity. Deficiency of Vitamin E can lead to hemolysis, neurological problems, and retinitis pigmentosa. Vitamin E is found in milk, nuts, corn oil, sunflower oil, avocados, asparagus, and broccoli. Vitamin K Vitamin K is needed to introduce calcium binding sites on Vitamin K dependent clotting factors in order to activate them. Without Vitamin K the blood will not clot. This process is called gamma-carboxylation of glutamic acid. Vitamin K dependent factors are 1972 (factors: 10, 9, 7, 2,). Vitamin K is found in green leafy vegetables. Causes of Vitamin K deficiency include fat malabsorption, prolonged use of broad-spectrum antibiotics, breast-fed newborns, and infants of mothers taking Phenytoin (Dilantin). How to differentiate between Vitamin K and Vitamin C deficiencies? The importance of vitamin D is also discussed in immunity, cancer and other conditions. Some people claim vitamin D to be a miracle hormone. vitamin D is essential for strong bones, because it helps the body use calcium from the diet. The man action of vitamin D is absorption of calcium and phosphate from the intestine. Where does vitamin D come from? vitamin D comes from diet and exposure to the sun. vitamin D is naturally found in fish. Both vitamin D from diet and converted by sunlight are not biologically active and needs to be activated before achieving all of its functions. What foods are rich in vitamin D? 1-cod liver oil, best source but it tastes bad. 2-Fish such as salmon, tuna, swordfish, and oysters (need to eat a lot to meet the required amount) 3-Ham, salami, sausages 4-Fortified cereals 5-Mushrooms 6-Eggs 7-Dairy products such as milk and cheese (not a lot of vitamin D) 8-Supplements •Cod liver oil (3.5 oz): 8500 international units (very good source) •Raw herring (3.5 oz ): 900 international units •Salmon (3.5 oz) : 600 international units •Multivitamin: 400 international units •Milk ( 1cup): 100 international units (not a very good source) How about the sun? known as the sunshine vitamin, vitamin D is produced by the body in response to skin exposed to sunlight. Cholesterol derived precursor ( pro-vitamin D3) is formed in the liver and stored in the skin. Provitamin D is activated by ultraviolet light yet it still requires more steps for activation. 10-15 minutes per day is enough exposure for light skinned individuals and darker skinned people need more exposure. If you are in the sun for 15 minutes, you may get about 10,000 units of vitamin D. How does metabolic activation occur? Metabolic activation occurs by adding two hydroxyl groups. One is at he carbon 25 in the liver and one at the carbon 1 in the kidney. Hydroxylation to 25 vitamin D3 occurs in the liver. The big organ takes the big number-25 25 (OH)- vitamin D3. Another hydroxylation occurs in the kidneys. The small organ takes the small number -1. The result will be 1,25 (OH)2- vitamin D3. This is the active form vitamin D and works mainly on the intestines and bones. Activation of vitamin D is controlled by the PTH hormone (Parathyroid hormone) any problem with this activation process of vitamin D will lead to deficiency due to an insufficient diet, lack of sun exposure, liver or kidney disease. Dilantin impairs the metabolism of vitamin D in the liver. Once the vitamin D is activated in the kidneys, it is then carried by a carrier with vitamin D binding protein to its target tissue. The target cells have receptors and vitamin D is bound to these receptors which lead to conformational changes in the nucleus. The receptors are found in the intestines, the bones and maybe the kidneys. The primary function of vitamin D is absorption of calcium and phosphates from the intestines. vitamin D influences activity of the cells that remodel and build the bone. 1,25 dihydroxy vitamin D stimulates RANKL expression. RANKL (ligand) is secreted by osteoblasts and binds to the RANK receptor on osteoclast precursor and mature osteoclast cells. Osteoclast activation stimulates bone reabsorption. In the kidneys, vitamin D helps in regulation of calcium absorption in the tubules. The parathyroid glands are on the back side of the thyroid gland. Elevated vitamin D decreases the formation of parathyroid hormone. Parathyroid hormone stimulates the conversion of 25 vitamin D3 to 1,25 hydroxyvitamin D in the proximal tubules of the kidneys by stimulating 1 alpha hydroxylase enzymes. Receptors are also found in the cardiovascular and immune systems. What are the stimuli for the release of 1,25 vitamin D? •Low serum calcium concentration •Low serum phosphate concentration •Elevated PTH levels. What are the factors that inhibit the production of vitamin D3? •Decreased parathyroid hormones •Elevated serum calcium and elevated serum phosphate. They are designated by Roman Numerals I through XIII. When a blood vessel is damaged, these clotting factors are switched on in a certain order (Blood Clotting Cascade) [link to coagulation page] and work to form a clot. Specifically, these 13 factors normally combine to a clot. If one factor is missing or present at low levels, this causes hemophilia and other blood clotting problems and a proper clot will not form. The two most common factor deficiencies are: factor 8 (or factor VIII) deficiency and factor 9 (or factor IX) deficiency. The most common, affecting 80% of the hemophilia population - those with hemophilia A - is factor VIII. When these blood clotting proteins aren't present is not easily stopped. This factor is so important to the treatment of hemophilia, that instead of saying they have "hemophilia A or B," most people say they are "Factor VIII" or "Factor IX" to identify their condition. Various substances are required for the proper functioning of the coagulation cascade: Calcium and phospholipid (a platelet membrane constituent) are required for the tenase and prothrombinase complexes to function. Calcium mediates the binding of the complexes via the terminal gamma-carboxy residues on FXa and FIXa to the phospholipid surfaces expressed by platelets, as well as procoagulant microparticles or microvesicles shed from them. Calcium is also required at other points in the coagulation cascade. Vitamin K is an essential factor to a hepatic gamma-glutamyl carboxylase that adds a carboxyl group to glutamic acid residues on factors II, VII, IX and X, as well as Protein S, Protein C and Protein Z. In adding the gamma-carboxyl group to glutamate residues on the immature clotting factors Vitamin K is itself oxidized. Another enzyme, Vitamin K epoxide reductase, (VKORC) reduces vitamin K back to its active form. Vitamin K epoxide reductase is pharmacologically important as a target of anticoagulant drugs warfarin and related coumarins such as acenocoumarol, phenprocoumon, and dicumarol. These drugs create a deficiency of reduced vitamin K by blocking VKORC, thereby inhibiting maturation of clotting factors. Vitamin K deficiency from other causes (e.g., in malabsorption) or impaired vitamin K metabolism in disease (e.g., in liver failure) lead to the formation of PIVKAs (proteins formed in vitamin K absence) which are partially or totally non-gamma carboxylated, affecting the coagulation factors' ability to bind to phospholipid. There’s a fair amount of research out there that is compelling and growing, so I took some time to summarize and condense it for your benefit. How does vitamin D regulate testosterone? Primarily this occurs because vitamin D inhibits the process of aromatization in which testosterone is changed into estrogen. It also appears that vitamin D may enhance the sensitivity of cell receptors in the testicular glands that produce testosterone. The best source of vitamin D is natural sunlight. However, that’s not always an option and dietary sources include fatty fish (salmon, tuna, mackerel), fish liver, beef liver, cheese, egg yolks, and fortified foods (namely, milk and breakfast cereals). Many of these studies are looking at individuals who have a deficiency in vitamin D for one reason or another. Like most vitamins, minerals and supplements, there is almost certainly a saturation point at which the benefits subside. Regarding testosterone, it seems relatively clear that men with low vitamin D will have an increase in testosterone when they begin supplementing. That benefit is not as obvious or well studied in men who have normal levels of vitamin D. And on a final note, in the grand scheme of things, vitamin D supplementation has relatively few, if any major side effects when taken at the recommended daily allowance and should be part of a baseline supplement regimen for weight lifters. A1C is an average of the patients blood sugar (glucose) readings over the past three months and is the single most important test that can predict the future for the diabetic patient. Low Calcium Levels Contribute to Disease There are many medical conditions linked to calcium deficiency. The following list are some of the more common symptoms and side effects: Early bone loss (osteopenia) Muscle spasms and cramps (especially night time calf cramping) Heart palpitations Depression Fatigue Joint pain High Blood pressure Hormone abnormalities Blood clotting problems Gum disease Headaches The above problems are often times medicated before a doctor performs the proper nutritional evaluation. Keep in mind that serum calcium levels are not an accurate representation of calcium stores within the body. Too Much Calcium Can Also Be a Problem Calcium is a very commonly used nutritional supplement. Many doctors instruct their female patients to take calcium as a preventative for bone loss. Without monitoring and checking calcium, taking too much can contribute to many problems. Therefore; it is not recommended to take excessive calcium without need. Some symptoms of too much calcium include: Confusion Fatigue Irritability Constipation Magnesium and iron loss Heart Arrhythmia Kidney damage Drugs That Interfere with Calcium There are a number of prescription medications that can interfere with calcium absorption or metabolism. It is not the single or short term use of medication that one has to worry about, but taking some of the following medications long term (6 months or greater) can interfere with this mineral. Examples include: Corticosteroids -- these are commonly used for control of pain and inflammation. Some steroids are used to treat chronic fatigue, chronic muscle pain, and inflammatory joint pain. Additionally, some steroids are used in the treatment of asthma. Antacid medications -- these drugs are commonly used to treat gastric reflux, heartburn, and indigestion. Examples include Nexium, Tagamet, Tums, and Rolaids. These prescription and over the counter medications suppress and interfere with stomach acid production. Calcium requires acid to be absorbed properly. Sometimes people take Tums with calcium to as a calcium supplement. I have had patients whose doctors actually recommended this! Bad advice -- Don't take antacids for a source of dietary calcium. Foods High In Calcium Despite popular belief, milk and dairy products are not a necessity for calcium in the diet. Foods rich in calcium are kale, broccoli, turnip greens, mustard greens, collards, figs, salmon, sardines, arugula, spinach, kelp, bone marrow and broth, and citrus fruits. High Quality Gluten Free Calcium When you have the need for calcium supplementation, you have to be careful not to take excessive calcium without magnesium, because doing so can contribute to magnesium deficiency. Additionally, it is important to ensure that your vitamin D levels are normal as calcium absorption is low when vitamin D is deficient. Many supplements have been found to contain gluten cross contamination.

Blood carries various substances that must be brought to one part of the body or another. Red blood cells are an important element of blood. Their job is to transport oxygen to the body’s tissues in exchange for carbon dioxide, which is carried to and eliminated by the lungs. Red blood cells are formed in the red bone marrow of bones. Stem cells in the red bone marrow called hemocytoblasts give rise to all of the formed elements in blood. If a hemocytoblast commits to becoming a cell called a proerythroblast, it will develop into a new red blood cell. The formation of a red blood cell from hemocytoblast takes about 2 days. The body makes about two million red blood cells every second. Blood is made up of both cellular and liquid components. If a sample of blood is spun in a centrifuge, the formed elements and fluid matrix of blood can be separated from each other. Blood consists of 45% red blood cells, less than 1% white blood cells and platelets, and 55% plasma. Red blood cells (RBCs), also called erythrocytes, are the most common type of blood cell and the vertebrate organism's principal means of delivering oxygen (O2) to the body tissues—via blood flow through the circulatory system. RBCs take up oxygen in the lungs or gills and release it into tissues while squeezing through the body's capillaries. The cytoplasm of erythrocytes is rich in hemoglobin, an iron-containing biomolecule that can bind oxygen and is responsible for the red color of the cells. The cell membrane is composed of proteins and lipids, and this structure provides properties essential for physiological cell function such as deformability and stability while traversing the circulatory system and specifically the capillary network. In humans, mature red blood cells are flexible and oval biconcave disks. They lack a cell nucleus and most organelles, in order to accommodate maximum space for hemoglobin; they can be viewed as sacks of hemoglobin, with a plasma membrane as the sack. Approximately 2.4 million new erythrocytes are produced per second in human adults. The cells develop in the bone marrow and circulate for about 100–120 days in the body before their components are recycled by macrophages. Each circulation takes about 20 seconds. Approximately a quarter of the cells in the human body are red blood cells. Nearly half of the blood's volume (40% to 45%) is red blood cells. Red blood cells are also known as RBCs, red cells, red blood corpuscles, haematids, erythroid cells or erythrocytes (from Greek erythros for "red" and kytos for "hollow vessel", with -cyte translated as "cell" in modern usage). Packed red blood cells (pRBC) are red blood cells that have been donated, processed, and stored in a blood bank for blood transfusion. Erythropoiesis (from Greek 'erythro' meaning "red" and 'poiesis' meaning "to make") is the process which produces red blood cells (erythrocytes). In postnatal birds and mammals (including humans), this usually occurs within the red bone marrow. In the early fetus, erythropoiesis takes place in the mesodermal cells of the yolk sac. By the third or fourth month, erythropoiesis moves to the liver. After seven months, erythropoiesis occurs in the bone marrow. Increased level of physical activity can cause an increase in erythropoiesis. However, in humans with certain diseases and in some animals, erythropoiesis also occurs outside the bone marrow, within the spleen or liver. This is termed extramedullary erythropoiesis. Whole blood contains red cells, white cells, and platelets (~45% of volume) suspended in plasma (~55% of volume). Red cells Red cells, or erythrocytes, carry oxygen from the lungs to your body’s tissue and take carbon dioxide back to your lungs to be exhaled. Platelets Platelets, or thrombocytes, are small, colorless cell fragments in the blood whose main function is to interact with clotting proteins to stop or prevent bleeding. Plasma Plasma is a fluid, composed of about 92% water, 7% vital proteins such as albumin, gamma globulin, anti- hemophilic factor, and other clotting factors, and 1% mineral salts, sugars, fats, hormones and vitamins. In the process of red blood corpuscle maturation, a cell undergoes a series of differentiations. The following stages of development all occur within the bone marrow: A Hemocytoblast, a multipotent hematopoietic stem cell, becomes a common myeloid progenitor or a multipotent stem cell, and then a unipotent stem cell, then a pronormoblast, also commonly called an proerythroblast or a rubriblast. This becomes a basophilic or early normoblast, also commonly called an erythroblast, then a polychromatophilic or intermediate normoblast, then an orthochromatic or late normoblast. At this stage the nucleus is expelled before the cell becomes a reticulocyte.

What actually determines the RDW is the width of the red cell volume distribution curve. To calculate RDW, use the following formula: RDW = (standard deviation of MCV ÷ mean MCV) × 100. In case of morphologic anisocytosis, RDW, which is normally 11–14% will increase to about 15–18%. RDW is of great benefit in two clinical scenarios; (1) In patients with microcytic anemia, when the differential diagnosis is between iron deficiency and thalassemia. In thalassemia, the small red cells are generally of uniform size with a normal small RDW, Whereas in iron deficiency, the size variability (and therefore the RDW) is large. (2) High RDW may suggest a dimorphic anemia which occurs when a chronic atrophic gastritis can produce both vitamin B 12 malabsorption to produce macrocytic anemia and blood loss to produce iron deficiency.
