Whenever a person has got jaundice we do a liver function test the nature of elevation of bilirubin and other enzymes gives us a clue that it could be bile duct obstruction. For an obstructive jaundice like this you do a simple investigation like ultrasound of the abdomen. The ultrasound will show whether there is obstruction or not. There are ducts within the liver that are dilated on ultrasound. It suggests that there is biliary obstruction. The ultrasound also detects if there are any stones in the gallbladder and we suspect the stone might be the cause of obstruction. The ultrasound also shows the level of the obstruction, whether it is above the level of the gall bladder and the cystic duct or is it below that level. When you have an obstruction below the level of cystic duct you usually do a further investigation with CT scan of the abdomen. If you suspect obstruction at the upper abdomen at the level of the liver hilum we usually do MRCP.

The tube and the medical procedure to implant it go by a number of names and acronyms. For simplicity, we're going to call it a biliary drain. So what is a biliary drain and why would you need one? A normally functioning digestive system is responsible for breaking down and absorbing nutrients into your bloodstream. Your liver produces bile salts that break down the fats entering your small intestine. When your bile ducts have blockages from cancer, the bile salts begin to build up in your liver and blood. You may experience pain and fever, and symptoms of jaundice: yellowing of the eyes and skin,dark urine and pale stools. Fluids may accumulate in your abdomen. This is a very serious condition, and if the blockage is not corrected, it can lead to a life threatening infection, or even liver failure. One way to remove the bile or other secretions from your body is to insert a biliary drainage tube. An Interventional Radiologist will perform the procedure using local anaesthetic. Guided by x-ray or ultrasound, the radiologist inserts a needle through the liver, directly into your bile duct. A guide wire placed through the needle will help to direct and position the drainage tube to the desired location. When the procedure is complete, the tube will allow bile to drain externally to a bile bag. If good internal drainage can be achieved to your small intestine, a bile bag may not be necessary. In this case, a low profile device can be applied to the external end, making the tube comfortable and discreet. If the biliary drainage tube is successful and the obstruction is relieved, your liver function will improve and you will feel better. Your oncologist will be able to inform you if you have improved enough to proceed with further treatments such as chemotherapy, radiation, and occasionally, surgery. This type of drainage tube can also be very helpful in relieving obstruction arising from cancers of the pancreas or small bowel. You, your home care nurses and the Interventional Radiology Team will work together to keep your biliary drain working well and free from infection. There are a few things you'll need to look out for. Occasionally fluid can leak out around the tube. This is generally not serious and can be addressed with gauze or dressing. Occasionally the external drainage tube becomes blocked and flow to the bile bag stops. This may result in internal pressure or pain near the liver. You must consult your doctor immediately. If the site looks or feels infected, or if you develop a fever, you must also consult your doctor immediately. Affected individuals may have no symptoms or may experience signs and symptoms of liver disease such as yellow discoloration of the skin and eyes, itching, and abdominal pain. The bile duct scarring which occurs in PSC narrows the ducts of the biliary tree and impedes the flow of bile to the intestines. Eventually, this can lead to cirrhosis of the liver and liver failure. PSC increases the risk of various cancers including liver cancer, gallbladder carcinoma, colorectal cancer, and cholangiocarcinoma. The underlying cause of PSC is unknown. Genetic susceptibility, immune system dysfunction, and abnormal composition of the gut flora may play a role. This is further suggested by the observation that approximately 75% of individuals with PSC also have inflammatory bowel disease (IBD), most often ulcerative colitis. There is no effective medical treatment for primary sclerosing cholangitis. The most definitive treatment for PSC is a liver transplant but it can recur after transplantation. Few people affected by PSC require a liver transplant. PSC is a rare disease and most commonly affects people with IBD. Approximately 3–7.5% of people with ulcerative colitis have PSC and 80% of people with PSC have some form of IBD. Diagnosis usually occurs in younger people in their 30s or 40s. Individuals of Northern European ancestry are affected more often than people of Southern European or Asian descent. Men are affected more often than women. The disease was initially described in the mid-1800s but was not fully characterized until the 1970s with the advent of improved medical imaging techniques such as endoscopic retrograde cholangiopancreatography (ERCP). Historically, a cholangiogram would be obtained via endoscopic retrograde cholangiopancreatography (ERCP), which typically reveals "beading" (alternating strictures and dilation) of the bile ducts inside and/or outside the liver. Currently, the preferred option for diagnostic cholangiography, given its non-invasive yet highly accurate nature, is magnetic resonance cholangiopancreatography (MRCP), a magnetic resonance imaging technique. MRCP has unique strengths, including high spatial resolution, and can even be used to visualize the biliary tract of small animal models of PSC. Most people with PSC have evidence of autoantibodies and abnormal immunoglobulin levels. For example, approximately 80% of people with PSC have perinuclear anti-neutrophil cytoplasmic antibodies; however, this and other immunoglobulin findings are not specific to those with PSC and are of unclear clinical significance/consequence. Many people can relate to making some type of sacrifice for the one they love. For some, its a move far away from family and friends. For others, its changing jobs or schools to be closer to their beloved. For 46 year-old Don Schwabe from Boise, Idaho, it was giving of himself in a way he had never imagined. Three years ago at the age of 37, Dons wife Heather Schwabe was diagnosed with primary sclerosing cholangitis, a chronic liver disease caused by progressive inflammation and scarring of the bile ducts in the liver. The cause is unknown but often the diagnosis is linked to a prior diagnosis of inflammatory bowel disease. Primary sclerosing cholangitis (PSC) leads to liver damage and, eventually, liver failure. Liver transplant is the only known cure. The time from first diagnosis to liver transplant is 12 to 18 years, on average. Known for her eternal optimism, Heather tried not to think about the eventual and perhaps inevitable need for a liver transplant. That need came much sooner than anyone would have expected when Heather was diagnosed with bile duct cancer, a known complication of PSC, in July 2009. Her doctors in Seattle delivered the news, along with a recommendation and referral to Mayo Clinic in Rochester, Minnesota. Within two weeks, Heather and Don arrived at Mayo Clinic and began learning the implications and treatment options for Heathers disease. It was quickly determined that, in addition to undergoing chemotherapy treatment for her bile duct cancer, Heather would soon need a liver transplant. What neither of them would have imagined at that time was that Don would end up being Heathers donor, and their surgeries were scheduled on the date they met 14 years ago.
