DVA skin cancer management  There are many types of skin cancers including BCCs or basal cell cancers, SCC or squamous cell cancers as well as melanomas. Todays review is on Bowen disease or Intraepidermal Squamous Cell Carcinoma. This is a common non-invasive cancer of the skin affecting the epidermal layers of skin. IECs most frequently occur on sun-exposed areas and present as red, scaly and persistent patches. IECs may arise de-novo, or from lesions of solar keratosis. If left untreated, Bowen disease may progress to an invasive Squamous Cell Carcinoma. This is one of the most common skin cancers I see in DVA or Department of Defence patients in Australia, and is due to the high UV radiation we have in cities like Brisbane.  Clinical Features Bowen disease most frequently presents as a persistent red scaly patch that slow grows over time. The most commonly affect areas include the legs, arms, and face- areas of sun exposure. Bowen disease or IECs can also affect the lips, oral mucosa, or even genital areas. As Bowen Disease is asymptomatic there is often a delay in the diagnosis, as it may mimic conditions such as dermatitis, psoriasis, tinea, and sun-spots.  Associations Most cases of Bowen disease arise from sun-damaged skin, including areas of actinic keratosis. The most common associations include- UV radiation. This plays a major contributing factor in the development of IECs. Immunosuppression. Patients on certain immunosuppressive medications are more likely to develop IECs.  Arsenic exposure. This was a common cause of Bowen Disease in the past, as arsenic was frequently used in medicines during the 60s and 70s including Bells Asthma Mixture, sheep dip, and as pesticides. Arsenic induced IECs-Bowen disease is a rare event today, as occupational safety has increased. A time lag of 10-20 years is seen between exposure and cancer.  Human Papilloma virus infections. Other associations include genetic diseases, radiotherapy and skin lesions such as porokeratosis, and solar keratosis. Investigations Bowen’s Disease can be suspected clinically, however in the majority of cases, a biopsy should be performed to confirm the diagnosis.  Treatment  There are multiple ways to manage Bowens/IECs. Treatment modalities will depend on the location and size of the cancer, the number of lesions, the degree of functional impairment.  Each modality has both advantages and disadvantages.  Cryotherapy or liquid nitrogen  Liquid nitrogen can be an excellent method of treating IECs. Most cases will require a double –freeze cycle of cryotherapy. This treatment is not recommended for IECs below the knee, as poor healing may occur.  Creams  Imiquimod, or 5-fluorouracil are effective in the treatment of Bowen disease. Clinical studies have shown cure rates approaching 80-90%. Treatment appears to be most successful when these creams are applied for 6-16 weeks.   Curettage and Electrocautery When small patches of IECs are present they may be treated via curettage and cautery. A local anaesthetic is given prior to the procedure to prevent pain and discomfort.  Photodynamic therapy  Photodynamic therapy or PDT can be an excellent non-surgical method of treating skin cancers such as Bowen’s/ IEC’s. PDT uses  a photosensitive cream called 5-aminolevulinic acid or methyl aminolevulinate, that is applied to the skin cancer. The cancerous cells of Bowens/ IEC take up this chemical over a period of 3 hours. A low-level laser light then activates the chemical, and destroys the skin cancer cells. Patients will require two treatments spaced a week or two apart. DVA or the Department of Veteran Affairs in Brisbane, covers PDT for both sunspots on the face and also IECs on all areas. PDT can also be used for multiple or large IECs. The benefit of PDT is that it selectively targets neoplastic cells without affecting the surrounding epidermis. This results in improved cosmetic outcomes, and faster healing times.  Surgery  Surgery is the ideal method of treating IECs in certain areas including recurrent IEC or ones that dwell in hair follicles. Surgery has the highest cure rate for Bowens/ IECs. For the majority of patients, especially DVA patients in Australia, non surgical methods, including the use of photodynamic therapy, is the future. Dr Davin Lim, Dermatologist. So this episode is going to be about a trick for doing better self skin cancer exams. Generally speaking, in most medical issues, symmetry is a reliable indicator that something isn't dangerous. So, in doing a self skin exam, if you find a mole or growth that is symmetric, then it is probably normal and not cancerous or precancerous. And just to remind you, a growth is symmetrical if you can fold it down the middle and the two halves are identical. Just like when I fold this circle in half. Yet in all of medicine there is only one situation that I know of where symmetry actually is a bad sign and usually indicates that something is abnormal and in fact at very least, precancerous. That situation occurs when two moles occur symmetrically to each other on your body. So imagine a line drawn down the center of your body, which of course goes right through your belly button. If you have two moles on your stomach, both about equal distance to the left and right of your bellybutton, then there is a very high probability that both of those moles are precancerous and each have a 20 to 25% life time probability of becoming melanomas. They should be shown to a dermatologist and probably be removed. The bottom line is that moles which are symmetric to each other with respect to the midline of your body are usually very dangerous, precancerous moles which if not removed have an unacceptably high probability of becoming deadly melanomas, a cancer that sadly claims the life of one American every hour. This is a truly rare medical example of symmetry being very dangerous, so if you come across it, show it to your doctor. Age – Many people get angiomas at the age of 30 or more. As a rule, in this case it looks like a small convex cherry mole and is mainly situated on the arms, legs or chest and shoulders. Such angiomas are caused by simple age changes of skin and blood vessels; 2. Health problems or the wrong lifestyle – If the angioma looks like a spot and begins vascular branching, it may indicate problems with the liver, vascular diseases, or a hormonal failure. What to do about it? While angiomas are not a cause for concern, you should watch them regularly to see if there is a change in them. If you suddenly notice that a familiar red mole unexpectedly changes size, shape, or has started bleeding, you’d better consult a doctor as soon as possible. Furthermore, just to be on the safe side, you should mention them at your next scheduled doctor’s appointment. The best angioma prevention is a healthy lifestyle and a balanced diet. Drink juices and plenty of water, eat fresh vegetables, and don’t forget about healthy fats such as avocados and olive oil. These simple rules will help you keep your skin clean and young, and your body healthy. If you already have angiomas and want to remove them, it’s a routine visit to the dermatologist. Your dermatologist can “burn” them with safe electric current. However, don’t try to burn an undiagnosed angioma on your own – you can cause further complications.

It often appears on skin that got lots of sun, such as an ear, face, bald scalp, neck, or arm. But it can appear elsewhere — even inside the mouth, on the lip, or on the genitals. Too much sun is often the cause, but it is not the only cause. SCC can appear on skin that was badly burned, had lots of radiation (such as x-rays), or was exposed to strong chemicals. SCC often has a reddish color. Without treatment, it can grow deeply. If this happens, the cancer can spread to other parts of the body. This can be deadly. SCC often looks like a: Hard (scaly or crusty) reddish bump, patch, or pearl-shaped growth Open sore that itches and bleeds; it can heal and return Scaly patch on the lip; skin on the lip can get thick Melanoma Know where you have moles on your skin! Melanoma often appears in an existing mole or looks like a new mole. By knowing where you have moles on your skin, you can find melanoma when it first appears. When treated early, melanoma is often curable. Recently, a viewer wrote in, "I have a mole that's uniform in color and shape but it itches. Can a non cancerous mole itch? Or does that mean it's surely cancerous?" Well. What do you think? Does the way a mole or growth feels really help you determines if it's normal or abnormal? The answer is usually not. The way your moles or growths feel offers very little reliable info about whether it's normal or not because both normal moles and growths, and precancerous and cancerous ones usually don't have any symptoms you can feel. They don't itch, burn, hurt or bother you.They don't feel like anything! But occasionally, regardless of whether they're normal or abnormal, if they become injured or inflamed, then they can itch, hurt or burn. Actually, the symptoms of itching, hurting or burning are usually typical of inflammation in general. That can occur if it gets scratched, rubbed, hit or just bumped into. When that happens, it may temporarily, rapidly swell, get red or even become warm, somewhat like a twisted ankle, but again, has nothing to do with normalcy. Because your body repairs the inflammation or injury that causes these symptoms, they are usually temporary, lasting only a few days or weeks. If they persist longer, then it's time to get it checked out, just to be sure. These harmless temporary symptoms of inflammation should never be confused with the actual dangerous warning signs of abnormal growths. How to spot if a mole is cancerous: Animation graphic reveals the ABCDE of skin cancer warning signs Skin cancer is the second most common form of the disease in people under 50 And yet, experts warn, the rate of early detection could be much higher Here, Dr David Fisher of Massachusetts General Hospital explains the benefits of early detection - and we present an animation graphic on how to check yourself As the years go by, our skin loses it springy freshness - and mysterious lumps and bumps start to appear. These blemishes can come in all shapes and sizes, and many are harmless. But slight changes could be a red flag: skin cancer is the second most common form of the disease in people under 50. Melanoma, a type of skin cancer, is one of the most treatable cancers. If caught early, the entire cancer could be removed simply by removing the mole in question. Basal cell carcinoma is even more manageable - only rare cases spread beyond the site. And yet, experts warn, the rate of early detection could be much higher. 'Many people are afraid of checking,' Dr David Fisher, director of the Melanoma Program at Massachusetts General Hospital, told Daily Mail Online. 'It's one of those fear and denial-type things. People would rather not know than find out something scary or devastating. 'But the thing is, six out of seven melanoma cases are cured just by catching it early and removing it. In other words, early detection could be life-saving.' He added: 'That statistic should help motivate people to be really proactive, to realize that there's a benefit to this. 'Not every cancer has that type of opportunity to catch it so early. This type of cancer does.' Moles are clusters of cells that produce a pigment in your skin. The more moles someone has on their body, the higher the risk of melanoma. Risk also increases if you have fair skin and red hair, if you sunbathe and if you use sunbeds. Of course, not all moles are dangerous: it's normal to have them. However, it can be easy to spot ones that are worth worrying about. Dermatologists use a simple list of five warning signs, using the rule ABCDE. This animation (above) explains how they work. A – asymmetry This means spotting an irregular shaped mole. B – borders Look to see if there are ragged edges to the mole. C – color change If a mole changes in color or is a different color in one part than in another. D – diameter Any increase in size should be checked out, but particularly more than about 6mm across. E – elevation In this graphic, the E section is classed as 'elevation', telling you to watch out for the mole raising from the surface, especially if it is irregularly raised. However, Dr Fisher explains, many dermatologists have different classifications for this. His preferred word is 'evolving'. 'Is it changing? Do you notice anything suspicious or concerning? That is key,' he says. 'You need to have a very low threshold for what counts as "concerning". I have yet to find a patient who is mad because it wasn't melanoma. It's always worth checking.' This is a real life story about a woman who was diagnosed with the deadliest disease nowadays – cancer. The woman was shocked and devastated when she found out that she has cancer. So, she has turned to mass media and social media to increase and raise the awareness of cancer and to show others how important is to have their growths and marks on their bodies checked out as soon as possible. Her name is Melanie Williams and she is 36 years old. Well, just few weeks before her cancer diagnosis, she noticed a strange dark spot appearing under her thumbnail. Melanie actually thought that it was just a fungal infection or a war. But, she was very wrong. The actual truth was much worse than that. Melanie posted on her Facebook wall when she found that it wasn’t a wart, but a cancer. “Sadly not”, that’s what she said. Melanie is a mother of 3 kids and she was diagnosed with melanoma, a form of skin cancer that can be removed with surgical procedure. Melanie said that she is so lucky and that the cancer hasn’t spread yet. The doctors will remove it next week, along with half of Melanie’s thumb. “Well, this may haven’t been the case if I didn’t went to a doctor and not got it checked out”, she said. Melanie also mentioned that her skin cancer appeared quickly, in just a matter of few weeks. When she posted her photo on Facebook, she said that she don’t want people’s sympathies. “I want more awareness. So, please go and get any changed or growths on your skin checked out. And, please tell other people around you to get checked as well”, Melanie posted on her wall. Well, it seems that Melanie has accomplished her mission. Unexplained weight loss An early sign of colon and other digestive cancers. Also a sign of cancer that has spread to the liver, affecting your appetite and the body’s ability to get rid of waste. 12. A red, sore, or swollen breast Indicates inflammatory breast cancer, unexplained changes should be told to a doctor immediately. A nipple that appears flattened, inverted or turned sideways had also been noticed by patients diagnosed with breast cancer. 13. Swelling of facial features Lung cancer patients have reported noticing puffiness, swelling or redness of the face. Small cell lung tumors commonly block blood vessels in the chest preventing blood from flowing freely to the head. 14. Sore/lump on the skin that bleeds, becomes crusty or doesn’t heal Different types of skin cancer (melanoma, basal cell carcinoma and squamous cell carcinoma) can cause these, so be aware of any odd-looking growths or spots. 15. Changes in nails A brown/black dot under the nail can indicate skin cancer. Pale or white nails can be a sign of liver cancer. “Clubbing”, which is the enlargement of the fingers with nails that curve down over the tips, can be a sign of lung cancer. Pain 16. Unusually heavy or painful periods, bleeding between periods A common sign of endometrial or uterine cancer. A transvaginal ultrasound should be considered if the problem persists. 17. Merkel cell carcinomas may appear as red or flesh-colored moles that are raised and grow quickly. These small tumors usually appear on sun-exposed areas of the skin, such as the face, neck or scalp.   Checking for skin cancer symptoms Regular examination of the skin for any new or unusual growths, or changes in the size, shape or color of an existing spot, is key to finding and treating skin cancers early. If you find anything suspicious, you should discuss it with your primary care physician or a dermatologist. While many skin cancers develop in areas exposed to the sun, they may also develop in areas that are usually hidden from the sun. It is important to examine all of these areas. Key Points *Most common form of skin cancer *Consists of bumps or growths which can be pearly or waxy, white, light pink, flesh-colored, or brown, sometimes slightly raised. Commonly, BCC will scab or bleed intermittently *Frequently occurs in sun-damaged skin *Does not usually metastasize. These are locally destructive cancers Basal cell carcinoma (BCC) is the most common form of skin cancer. It usually presents in areas of the skin exposed to sun, including the face, neck, head, chest and upper back, though it can appear anywhere. There are several subtypes, including nodular, infiltrating, morpheic, and superficial. In nodular BCC, the cancer presents itself as pearly, skin-colored or pink bumps, with tiny blood vessels often appearing on their surface. As the lesion grows, it may ulcerate or bleed with minor trauma. Infiltrating lesions often appear scar-like or depressed. They may often be shiny and have sores or scabs on their surface. Superficial BCC presents itself as pink or red scaly skin, often dry appearing. Lesions will slowly grow and the border of the lesion may become raised. This can be mistaken as a chronic rash such as ringworm or just dryness. This subtype is generally found on the trunk, arms, and legs. While BCC can affect any age or ethnic group, it most commonly appears in elderly fair complected caucasians. Chronic sun exposure is virtually always noted as a causative factor. There are also syndromes they convey a markedly elevated risk of BCC, such as nevoid Basal cell carcinoma Syndrome and Xeroderma Pigmentosa. Differential Diagnosis (Other conditions with similar appearance) Fibrous papule Sebaceous hyperplasia Nevus Seborrheic keratosis Melanoma Angiokeratoma Nummular Eczema Psoriasis Bowen disease Diagnosis Key Points *Diagnosis based on skin appearance *Skin biopsy performed to confirm diagnosis and rule out other conditions *Biopsy results indicate which treatment course is appropriate BCC is generally diagnosed based on appearance, then further classified by sub-type. Health care professionals will then perform a biopsy to confirm diagnosis and plot out treatment course. Treatment *Self-examination and evaluation by a dermatologist is key in identifying lesions early *Surgical removal is the most commonly employed treatment While self-examination is recommended to identify lesions, they must be treated by health care professionals. Once diagnosis is confirmed via biopsy, treatment will be depend on several factors. These may include size and location of the cancer, as well as the general health of the patient. In the case of nodular BCC, treatment options include electrodessication and curettage, surgical excision, Mohs Micrographic surgery, and radiation treatment of the affected area. Radiation is usually only used in cases where the affected person isn't a good candidate for surgery due to other health problems. With infiltrating BCC or morpheic BCC, treatment is more limited due to the aggressive and invasive nature of the disease. Mohs Micrographic surgery is usually the treatment of choice in this situation. Aggressive treatments are generally not required for instances of superficial BCC, as it is slow-growing and thin. In addition to conventional treatments such as cryosurgery, electrodessication and radiation treatment, topical options such as Imiquimod (which encourages the immune system to attack the BCC), Photodynamic therapy, and laser treatments can be used to combat the condition.

10 Signs That You May Have Leukemia When we hear the word cancer we are probably quick to think of things like skin cancer, lung cancer, or maybe even especially unforgiving forms of cancer such as pancreatic cancer. Leukemia is a type of cancer that’s quite different from most other forms since it is actually cancer of the blood. Like so many other forms of cancer, what causes is it is still a mystery. It usually starts in the bonee marrow and causes the formation of white blood cells that are abnormal. Leukemia is normally diagnosed with a blood test of a bonee marrow biopsy. Treatment usually consists of chemotherapy, radiation or bonee marrow transplant. The 5-year survival rate for people diagnosed with leukemia is about 57 percent, which may provide a measure of hope for those who are diagnosed. The following are signs and symptoms that could indicate leukemia. 1. Tiny Spots or Dots on the Skin These tiny spots are technically known as petechiae and may show up as patches of very small purple or red dots that are often observed on the arms, back, chest and face. Since leukemia can cause a decrease in the number of clot-forming platelets in the blood, smaller blood vessels are more likely to break causing petechiae, which can easily be mistaken for some type of rash. 2. bonee and Joint Pain This is one of the most common symptoms of people who have developed leukemia and is characterized by pain that feels like it originates deep in the bonees. Swollen Glands Since leukemia has a damaging effect on the blood, it is much more difficult for the body to fight off opportunistic infections and that can cause glands and lymph nodes to swell. This can manifest as small lumps or bumps that show up in places such as the neck, stomach, under the arms or in the groin area that may take on a color that has hints of blue or purple. Normally, these kinds of abnormalities form as a result of less serious infections and do not generally persist for longer than a week, or so. 5. Fatigue The blood is absolutely essential for delivering a constant supply of nutrients and oxygen to the cells of the body and when leukemia interferes with that process, feeling tired or worn out all the time is often the result. It will often become very difficult to complete routine physical tasks when suffering from these symptoms, which can also take an emotional toll on victims of this disease. 6. Bruising and Bleeding As mentioned previously, the blood is often unable to do its job properly when leukemia is a factor, and this often makes it much easier to suffer bruising just about anywhere on the body. It’s especially telling when bruises begin to show up in areas where bruising has never been a problem previously. Bruises may also appear darker in color and look worse than they have in the past.
