Recently, the Mayo Clinic published startling research showing that 40 percent of pancreatic cancer patients had been diagnosed with diabetes one to two years before discovering they had a pancreatic tumor. Researchers believe the diabetes is caused by tumors that simply haven't been detected yet. The problem is, diabetes is very common, and the majority of diabetes isn't pancreatic cancer, so doctors are trying to develop screening tools to tell the difference. Right now, they say family history is an important clue. If you're diagnosed with diabetes that seems to come on suddenly and you have no family history of diabetes, bring this to your doctor's attention and ask for further screening for pancreatic cancer. Yellowing of the eyes or skin. Even a small pancreatic tumor can block the bile duct in the head of the pancreas, causing bile to build up. This causes jaundice. Itchy skin, palms, and soles of feet. A little-known side-effect of jaundice is itchy hands and feet. It's due to a skin reaction to the bilirubin, the yellowish brown liver chemical that causes jaundice. Lack of appetite. An Italian study found that six to eight months before being diagnosed with pancreatic tumors, patients reported a sudden drop in their appetite and a tendency to feel full after eating very little. Changes in taste. In the same Italian study, some of the patients surveyed said they'd suddenly lost their taste for coffee, wine, and smoking. A blood test is now available for a biomarker called CA 19-9 that is released by pancreatic cancer cells. Unfortunately, by the time CA 19-9 reaches detectable levels, the cancer is usually no longer in the early stage, although the test is useful during treatment and for detecting recurrence. Another biomarker test, CEA (carcinoembryonic antigen), is also not sensitive enough to provide early diagnosis. While tragic early deaths like Steve Jobs' are the hard reality of pancreatic cancer, there are also people living productive lives thanks to an early diagnosis of pancreatic cancer. Such was the case with Supreme Court Justice Ruth Bader Ginsburg, who's still practicing after having a tiny pancreatic tumor surgically removed. What are the symptoms? How do you know if you have pancreatic cancer? Should you be screened? Over the next several minutes, I will give an overview of pancreatic cancer. Pancreatic cancer is the fourth leading cause of cancer-related deaths in the United States. The pancreas is the organ that sits in the central upper abdomen, is located right behind the stomach organ. The pancreas is about six to eight inches long and about two inches wide, and it extends from the head of the pancreas, which is on the right side of the abdomen, connected to the small intestine, near the gallbladder and the liver, and the pancreas extends to the tail of the pancreas, which is more on the left side of the abdomen. The pancreas is the organ that's responsible for producing certain hormones such as insulin, which regulates your blood sugar. The pancreas also produces certain digestive enzymes or digestive juices, which help us to digest fat. There are two types of pancreatic cancer. One, which comes from the endocrine glands or the hormone-producing cells such as an insulinoma. The other type of pancreatic cancer comes from the exocrine glands or from the pancreatic ducts, and this exocrine gland pancreatic cancer is the most common type of pancreatic cancer. What are the symptoms of pancreatic cancer? One is weight loss. Patients with pancreatic cancer often have early satiety, meaning they get full quickly off of just small amounts of food, or they may just lose their appetites altogether. Another symptom, abdominal pain. Patients often have a dull, aching abdominal pain in the central upper abdomen, and it often radiates to the back. Another symptom, jaundice. Jaundice occurs when the patient's skin turns yellow or the whites of the eyes turn yellow, and this is due to a blockage of the flow of bile by the pancreatic cancer. Patients who have jaundice, this yellow skin, will often also have a change in their stools. How can you be diagnosed with pancreatic cancer? If you have any of the symptoms I described, it is important that you consult your physician as soon as possible. Your physician will do a complete history and physical, and they will likely do some imaging studies such as an ultrasound of the pancreas or a CT, computed tomography, of the pancreas or a test called an ERCP, endoscopic retrograde cholangiopancreatography. If they are suspicious or if they see a mass that looks like pancreatic cancer, then they may do a biopsy, which is when they have a physician to take a small sample of the mass, and a pathologist is another type of physician who will look at that and determine if it is or if it isn't pancreatic cancer. The treatment for pancreatic cancer. The treatment really depends on the staging. If the cancer is caught early, there's a chance that you can have surgery or surgical removal of the cancer. and patients often will have an adjuvant therapy, which means they will receive chemotherapy and/or radiation, especially if the pancreatic cancer has spread. In cases of stage four or metastatic pancreatic cancer, a lot of those patients will also receive chemotherapy. However, that chemotherapy is usually just to shrink the tumor or to help to decrease the symptoms of the tumor. Stage four pancreatic cancer is usually not curable. So what are the risk factors for pancreatic cancer? Well, five to 10 percent of patients who have pancreatic cancer also have family members with pancreatic cancer. Other things that have been associated with pancreatic cancer include cigarette smoking, obesity, physical inactivity, the Helicobacter pylori infection. The cancer is sometimes detected because of medical tests and procedures trying to find what is causing some nonspecific symptoms. Regrettably, in the early stages, pancreatic cancer often causes few (if any) distressing symptoms until later stages when the treatments (chemo and radiation) only slow the progress of the disease. Early diagnosis is important while there are treatment and surgical options available 1 Watch for jaundice. One of the first signs of pancreatic cancer can be jaundice, which is the yellowing of the skin, eyes and mucus membranes due to too much bilirubin in the bloodstream.[3] Jaundice is associated with liver disease, although pancreatic cancer often causes nearby organs, liver and gallbladder to swell, which leads to dysfunction. Look at your skin and eyes in a well-lit mirror to check for yellow discoloration. Jaundice also causes itchy skin, so be on the look out for that also. The parts of the eyes that turn yellow are called the sclera (the "whites") not the iris, which is typically colored either brown, blue or green. To confirm jaundice (if the yellowing is not so obvious), your doctor may test your urine for bile or order a blood test. 2 Take notice of a sore abdomen. One of the first signs of pancreatic cancer can sometimes be a diffuse abdominal aching and soreness, although many people don't feel any pain until the cancer is advanced. The pancreas gland is located underneath the stomach and behind intestines — pretty much in the middle of your belly. It secretes insulin (for blood sugar control), hormones and digestive enzymes. [4] If your belly soreness doesn't go away after a week, contact your doctor. Palpating (deeply touching) your pancreas for mild-to-moderate swelling is difficult and virtually useless for doctors because the gland is positioned behind and close to other organs. Because pancreatic cancer often causes the liver and/or gallbladder to swell, which are easier to palpate and detect, the condition may be misdiagnosed as liver cirrhosis or cholecystitis. Due to abdominal tenderness, fatigue and diarrhea, the early stages of pancreatic cancer can mimic infections, ulcerative colitis, Crohn's disease and irritable bowel syndrome. 3 Be alert to fatigue and weakness. Another early sign of pancreatic cancer -- as well as in most other types -- is a general feeling of tiredness, fatigue and weakness.[5] This is because pancreatic cancer may block the flow of digestive fluids -- and also, cancer cells tend to use lots of resources, burn lots of energy and trigger your immune system to work harder. 10 Early Warning Signs of Pancreatic Cancer 1. Diabetes, especially if it comes on suddenly. Recently, the Mayo Clinic published startling research showing that 40 percent of pancreatic cancer patients had been diagnosed with diabetes one to two years before discovering they had a pancreatic tumor. Researchers believe the diabetes is caused by tumors that simply haven't been detected yet. The problem is, diabetes is very common, and the majority of diabetes isn't pancreatic cancer, so doctors are trying to develop screening tools to tell the difference. Right now, they say family history is an important clue. If you're diagnosed with diabetes that seems to come on suddenly and you have no family history of diabetes, bring this to your doctor's attention and ask for further screening for pancreatic cancer. 2. Yellowing of the eyes or skin. Even a small pancreatic tumor can block the bile duct in the head of the pancreas, causing bile to build up. This causes jaundice. 3. Itchy skin, palms, and soles of feet. A little-known side-effect of jaundice is itchy hands and feet. It's due to a skin reaction to the bilirubin, the yellowish brown liver chemical that causes jaundice. 4. Lack of appetite. An Italian study found that six to eight months before being diagnosed with pancreatic tumors, patients reported a sudden drop in their appetite and a tendency to feel full after eating very little. 5. Changes in taste. In the same Italian study, some of the patients surveyed said they'd suddenly lost their taste for coffee, wine, and smoking. In fact, they said, they felt "disgust" for the smell and taste of coffee and alcohol. 6. Abdominal pain. Pancreatic cancer sufferers remember this pain as a gnawing pain, rather than a sharp cramp or ache, and it radiates toward the back. A characteristic clue: the pain goes away when you lean forward. 7. An enlarged gall bladder. The same blockage of the bile duct that causes jaundice can also cause an enlarged gallbladder, as the bile builds up behind the duct. The good news is that an enlarged gallbladder can be seen on imaging tests, and it may even be possible for a doctor to feel it during a physical exam. 8. Pale, floating, smelly stools. If a pancreatic tumor prevents digestive enzymes from reaching the intestine, the result is an inability to digest fatty foods. So you end up with loose, smelly "floaters" as a result of the excess fat. Doctors say this symptom, in particular, can be an early clue and is too often overlooked. 9. Dark, tarry stools. Bleeding in the upper intestines causes this symptom. 10. Sudden, unexplained weight loss. Weight loss is not always, as many people mistakenly believe, a sign of advanced cancer that's spread to the liver. It can also happen because a lack of pancreatic enzymes is causing fat to pass through the body undigested. What do you do if you're worried about any of these symptoms? Document all symptoms, and report them to your doctor in as much detail as possible. If your doctor is convinced you have legitimate concerns (and remember, you may have to do some convincing), tests such as an ultrasound, a CT scan, and an endoscopy followed by a biopsy can be used to search for a pancreatic tumor. A blood test is now available for a biomarker called CA 19-9 that is released by pancreatic cancer cells. Unfortunately, by the time CA 19-9 reaches detectable levels, the cancer is usually no longer in the early stage, although the test is useful during treatment and for detecting recurrence. Another biomarker test, CEA (carcinoembryonic antigen), is also not sensitive enough to provide early diagnosis. While tragic early deaths like Steve Jobs' are the hard reality of pancreatic cancer, there are also people living productive lives thanks to an early diagnosis of pancreatic cancer. Such was the case with Supreme Court Justice Ruth Bader Ginsburg, who's still practicing after having a tiny pancreatic tumor surgically removed. Yes, she got "lucky," as she put it -- but she also got a CT scan, which is how the tumor was found.

Chances are, you rarely give much thought to this glandular organ, which helps your body digest food and convert it into energy. Located behind your stomach, the pancreas produces insulin and regulates your body's hormones that maintain glucose (blood sugar) levels. Pancreatic cancer is overall pretty rare, with 55,440 people in America expected to be diagnosed in 2018. (The most commonly diagnosed cancer is non-melanoma skin cancer, affecting more than 1 million people a year, followed by lung cancer, with more than 200,000 cases diagnosed annually.) But even though it’s an uncommon diagnosis, pancreatic cancer is soon expected to overtake colon cancer as the number-two leading cause of cancer deaths in the United States, says Valerie Lee, MD, pancreatic oncologist at Johns Hopkins Medicine. Why? First, because pancreatic cancer is very aggressive. “It has a high likelihood of spread, or metastasis, to other organs in the body," says Neil Woody, MD, oncologist at the Cleveland Clinic. The five-year survival rate is only 8.5 percent. But it may also be because pancreatic cancer is on the rise. Pancreatic cancer has been diagnosed at an increase of .5% a year for more than a decade. Part of that is due to increased detection, Dr. Lee says, but lifestyle factors may be playing a role as well. Increases in diabetes and obesity—two major risk factors for pancreatic cancer—may also be contributing to the rise in pancreatic cancer cases. Pancreatic cancer warning signs. Pancreatic cancer is notoriously difficult to diagnose. Only about 10 percent of pancreatic cancers have a genetic predisposition, and good screenings for the general population have been difficult to establish, says Dr. Woody. “Tests with high likelihood of detecting pancreatic cancer, such as endoscopic ultrasound, are invasive, costly, and may carry risks of complications that outweigh their use as screening tests for the average person,” he explains. What’s more, pancreatic cancer is tough to spot early, and many of the symptoms can be caused by other conditions. Plus, people usually have no symptoms until the cancer has spread to other organs. Still, if you are experiencing any of these warning signs, it’s worth making an appointment with your doctor. 1. New onset of diabetes “When people with no risk factors for diabetes (no family history, good diet, thin body type) develop new onset diabetes, or who have a history of well controlled diabetes suddenly find that it becomes difficult to control, it’s reasonable to evaluate why the pancreas may not be functioning as well," says Dr. Lee. 2. Abdominal pain "The pancreas sits close to a bundle of blood vessels and nerves called the celiac plexus that can cause pain and nerve irritation, says Dr. Lee. But the pain is usually pretty encompassing of your abdomen. "Patients feel pain from the epigastrum (the area just under the sternum) through to their back. While the exact cause is not known, it is believed that cancer cells reduce the proteins the body uses to prevent blood clots. “For people who do not have a reason for the development of a clot—a recent major surgery, trauma (like a car accident), hospitalization, or prolonged period where you are laying still, or a hereditary predisposition to clotting—we recommend making sure you are up to date on your cancer screening and to check in with your doctor about any other symptoms that may help to identify the source of the clotting abnormality," says Dr. Lee. 4. The pancreas is a large organ behind the stomach that produces digestive enzymes. There are two main types, acute pancreatitis and chronic pancreatitis. Signs and symptoms of pancreatitis include pain in the upper abdomen, nausea and vomiting. The pain often goes into the back and is usually severe. In acute pancreatitis a fever may occur and symptoms typically resolve in a few days. In chronic pancreatitis weight loss, fatty stool, and diarrhea may occur. Complications may include infection, bleeding, diabetes mellitus, or problems with other organs. The most common causes of acute pancreatitis are gallstones and heavy alcohol use. Other causes include direct trauma, certain medications, infections such as mumps, and tumors among others. Chronic pancreatitis may develop as a result of acute pancreatitis. It is most commonly due to many years of heavy alcohol use. Other causes include high levels of blood fats, high blood calcium, some medications, and certain genetic disorders such as cystic fibrosis among others. Smoking increases the risk of both acute and chronic pancreatitis.Diagnosis of acute pancreatitis is based on a threefold increase in the blood of either amylase or lipase. In chronic pancreatitis these tests may be normal. Medical imaging such as ultrasound and CT scan may also be useful. Acute pancreatitis is usually treated with intravenous fluids, pain medication, and sometimes antibiotics. Typically no eating or drinking is allowed and a tube may be placed into the stomach. A procedure known as an endoscopic retrograde cholangiopancreatography (ERCP) may be done to open the pancreatic duct if blocked. In those with gallstones the gallbladder is often also removed. In chronic pancreatitis, in addition to the above, temporary feeding through a nasogastric tube may be used to provide adequate nutrition. Long-term dietary changes and pancreatic enzyme replacement may be required. And occasionally surgery is done to remove parts of the pancreas. Globally, in 2015 about 8.9 million cases of pancreatitis occurred.This resulted in 132,700 deaths, up from 83,000 deaths in 1990.Acute pancreatitis occurs in about 30 per 100,000 people a year.New cases of chronic pancreatitis develop in about 8 per 100,000 people a year and currently affect about 50 per 100,000 people in the United States.It is more common in men than women. A characteristic clue: the pain goes away when you lean forward. 7. An enlarged gall bladder. The good news is that an enlarged gallbladder can be seen on imaging tests, and it may even be possible for a doctor to feel it during a physical exam. 8. Pale, floating, smelly stools.
