Approximately 80% of patients receiving co-trimoxazole develop increases in serum potassium concentrations and 6% develop frank hyperkalemia. In this research, the scientists set out to determine whether the prescription of co-trimoxazole with an ACE inhibitor or ARB is associated with sudden death. They found that in older patients there is an associated increased risk of sudden death. Unrecognized severe hyperkalemia may underlie this finding. Unlike the colon (or large bowel), which is rich with bacteria, the small bowel usually has fewer than 10,000 organisms per millilitre. Patients with bacterial overgrowth typically develop symptoms including nausea, bloating, vomiting, diarrhea, malnutrition, weight loss and malabsorption, which is caused by a number of mechanisms. The diagnosis of bacterial overgrowth is made by a number of techniques, with the gold standard being an aspirate from the jejunum that grows in excess of 10-5 bacteria per millilitre. Risk factors for the development of bacterial overgrowth include dysmotility; anatomical disturbances in the bowel, including fistulae, diverticula and blind loops created after surgery, and resection of the ileo-cecal valve; gastroenteritis-induced alterations to the small intestine; and the use of certain medications, including proton pump inhibitors. Small bowel bacterial overgrowth syndrome is treated with an elemental diet or antibiotics, which may be given in a cyclic fashion to prevent tolerance to the antibiotics, sometimes followed by prokinetic drugs to prevent recurrence if dysmotility is a suspected cause. Bacterial overgrowth can cause a variety of symptoms, many of which are also found in other conditions, making the diagnosis challenging at times. Many of the symptoms are due to malabsorption of nutrients due to the effects of bacteria which either metabolize nutrients or cause inflammation of the small bowel, impairing absorption. The symptoms of bacterial overgrowth include nausea, flatus, constipation, bloating, abdominal distension, abdominal pain or discomfort, diarrhea, fatigue, and weakness. SIBO also causes an increased permeability of the small intestine. Some patients may lose weight. Children with bacterial overgrowth may develop malnutrition and have difficulty attaining proper growth. Steatorrhea, a sticky type of diarrhea where fats are not properly absorbed and spill into the stool, may also occur. Patients with bacterial overgrowth that is longstanding can develop complications of their illness as a result of malabsorption of nutrients. Anemia may occur from a variety of mechanisms, as many of the nutrients involved in production of red blood cells are absorbed in the affected small bowel. Iron is absorbed in the more proximal parts of the small bowel, the duodenum and jejunum, and patients with malabsorption of iron can develop a microcytic anemia, with small red blood cells. Vitamin B12 is absorbed in the last part of the small bowel, the ileum, and patients who malabsorb vitamin B12 can develop a megaloblastic anemia with large red blood cells. In older adults, small bowel bacterial overgrowth is associated with a higher frequency of diarrhea, a lower body mass index, and a significantly lower serum albumin concentration. The diagnosis of bacterial overgrowth can be made by physicians in various ways. Malabsorption can be detected by a test called the D-xylose test. Xylose is a sugar that does not require enzymes to be digested. The D-xylose test involves having a patient drink a certain quantity of D-xylose, and measuring levels in the urine and blood; if there is no evidence of D-xylose in the urine and blood, it suggests that the small bowel is not absorbing properly (as opposed to problems with enzymes required for digestion). Bacterial overgrowth is usually treated with a course of antibiotics although whether antibiotics should be a first line treatment is a matter of debate. Some experts recommend probiotics as first line therapy with antibiotics being reserved as a second line treatment for more severe cases of SIBO. Prokinetic drugs are other options but research in humans is limited. A variety of antibiotics, including tetracycline, amoxicillin-clavulanate, fluoroquinolones, metronidazole, neomycin, cephalexin, trimethoprim-sulfamethoxazole, and nitazoxanide have been used; however, the best evidence is for the use of rifaximin. The condition that predisposed the patient to bacterial overgrowth should also be treated. Escherichia coli, a member of the Enterobacteriaceae family of bacteria, is the leading cause of community- and hospital-acquired urinary tract infections (UTIs). Although E. coli commonly colonizes the intestines, certain food-borne strains can cause serious gastrointestinal infections. These maps show inpatient E. coli resistance to trimethoprim-sulfa (also known as TMP/SMZ or co-trimoxazole) , a combination antibiotic that is frequently administered as a first-line therapy for uncomplicated UTIs when resistance rates are below 20%. Inpatient TMS/SMZ resistance is similar in magnitude and geographic variation but increases slightly faster than outpatient rates. In 2000, varied resistance levels to the drug appear across the United States, with most of the resistance in the western half of the country. Higher resistance levels then spread eastward. In 2006, all regions have passed the 20% threshold with West South Central and Pacific regions reporting the highest levels of resistance, at just over 30%. East North Central, Mountain and the South Atlantic are nearly as high, with resistance levels between 25% and 30%. By 2009, resistance levels higher than 30% are widespread, with only New England remaining close to the 20% resistance threshold. The rise in both TMP/SMZ and ciprofloxacin resistance has motivated calls for a revision in UTI treatment and a switch to nitrofurantoin, an older antibiotic that now shows higher effectiveness. Increases in TMP/SMZ resistance signal a problem with broader implications for both nosocomial (health care--associated) and community infections, namely the rise of extended-spectrum beta-lactamase (ESBL) strains of Enterobacteriaceae. ESBLs are bacterial enzymes that confer resistance to many antibiotic classes, including sulfonamides. The epidemiology of ESBL-producing strains, particularly E. coli, has increasingly shifted from the hospital setting to the community. ESBL infections are still susceptible to the newer and more expensive antibiotics of the carbapenem class. However, newer types of beta-lactamase enzymes are diminishing the effectiveness of these last-resort treatments and posing an emerging threat to public health. The national average resistance level for the sample was 24.1%, rising from 17.7% in 2000 to 29% in 2009. The sample consists of 872,563 inpatient isolates tested for trimethoprim-sufamethoxazole resistance. Data are not available from the following states: AR (2008--2009), CO (2006--2009), GA (2007--2009), IA (2008--2009), IN (2007--2009), KY (2007--2009), MS (2006--2009), NV (2009), RI (2000--2004), SD (2005--2009), UT (2007--2009), CT, NH, MT, and WY.

SJS Journey For everyone that knows me, understands how much I keep my life very personal and private. I wasn’t sure if I had wanted to share this with anyone but I’ve decided I should!!! I recently had surgery and everything went smooth except for one thing 🤔 Most people are always more concerned on the procedure rather than the aftermath well..... that wasn’t my case. Medication was the overall cause to my close to death experience. (I know who would’ve thought) I was prescribed BACTRIM! This medication is a combination of two antibiotics: sulfamethoxazole and trimethoprim. Turns out I am severely allergic, where I developed a disease called STEVENS-JOHNSON/TENS! Stevens-Johnson syndrome (SJS) is a rare and serious condition of your skin and mucus membranes. SJS will cause you to lose up to 10% of your outer layer of skin. (I lost 80% of my skin). Basically I was burning from the inside out! I was very fortunate to have an amazing Dermatologist who knew my symptoms right away (FYI he saved my life) he made sure the whole team was on tip top of making sure I received the care I deserved and needed. I was transported to ICU-BURN VICTIMS UNIT, where I received emergency eye surgery in order to save my vision! So blessed🙏🏽! I received wound care, hydrotherapy, physical therapy and TPN! I was admitted for 17 Nights-18 Days, and received love, support every single second from my family. Doctors till this day state I am a walking miracle! My healing process has taken a toll on me in many ways from I am still here to The I knew I was always strong but WOW! I am currently loosing all of the hair on my head and all of my finger/toe nails! My overall reason for sharing my journey with SJS is to help people understand it and to please educate yourself before you medicate, not only you but your children as well. SJS is rare but a deadly disease if not caught in time! If I can help spread precautions on medication and SJS my purpose is half way done!
