Baroreflex is a rapid negative feedback loop in which an elevated blood pressure causes heart rate and blood pressure to decrease. Reversely, a decrease in blood pressure leads to an increased heart rate, returning blood pressure to normal levels. The reflex starts with specialized neurons called baroreceptors. These are stretch receptors located in the wall of the aortic arch and carotid sinus. Increased blood pressure stretches the wall of the aorta and carotid arteries causing baroreceptors to fire action potentials at a higher than normal rate. These increased activities are sent via the vagus and glossopharyngeal nerves to the nucleus of the tractus solitarius – the NTS - in the brainstem. In response to increased baroreceptor impulses, the NTS activates the parasympathetic system – the PSNS - and inhibits the sympathetic system – the SNS. As the PSNS and SNS have opposing effects on blood pressures, PSNS activation and SNS inhibition work together in the same direction to maximize blood pressure reduction. Parasympathetic stimulation decreases heart rate by releasing acetylcholine which acts on the pacemaker cells of the SA node. Inhibition of the sympathetic division decreases heart rate, stroke volume and at the same time causes vasodilation of blood vessels. Together, these events rapidly bring DOWN blood pressure levels back to normal. When a person has a sudden drop in blood pressure, for example when standing up, the decreased blood pressure is sensed by baroreceptors as a decrease in tension. Baroreceptors fire at a lower than normal rate and the information is again transmitted to the NTS. The NTS reacts by inhibiting parasympathetic and activating sympathetic activities. The sympathetic system releases norepinephrine which acts on the SA node to increase heart rate; on cardiac myocytes to increase stroke volume and on smooth muscle cells of blood vessels to cause vasoconstriction. Together, these events rapidly bring UP blood pressure levels back to normal. Baroreflex is a short-term response to sudden changes of blood pressure resulted from everyday activities and emotional states. If hypertension or hypotension persists for a long period of time, the baroreceptors will reset to the “new normal” levels. In hypertensive patients for example, baroreflex mechanism is adjusted to a higher “normal” pressure and therefore MAINTAINS hypertension rather than suppresses it. This is a short protein produced by the liver and is inactive; it just circulates in the blood. However, when renin acts on angiotensinogen it converts it into another shorter protein called angiotensin I. As angiotensin I passes through the lungs it is converted into angiotensin II by an enzyme called angiotensin converting enzyme (ACE). Angiotensin II is a powerful vascular vasoconstrictor; this will increase peripheral resistance which will in turn increase blood pressure. In addition to increasing BP by direct vasoconstriction angiotensin II also stimulates the secretion of aldosterone from the adrenal cortex. This will increasethe amount of sodium reabsorbed from the renal tubules and so increase levels of sodium in the blood. As mentioned, increased levels of sodium increases the osmotic potential of the blood and so it attracts more water. This means more water is retained in the plasma increasing blood volumes. These increased volumes will increase venous return and so blood pressure. Conversely, lower amounts of angiotensin II will reduce aldosterone secretion leading to more sodium excretion. This will lower sodium levels in the blood leading to lowered blood volumes and so lower BP. If the systemic blood pressure is increased the pressure in the afferent arteriole will also be increased. This will be detected by the JGA cells which will respond by reducing renin secretion. When less renin is present in the blood, more angeotensionogen will remain in inactive form. This renin-angiotensin-aldosterone mechanism allows the kidneys to regulate their own blood flow to an extent. When an organ regulates itself in this way it is called autoregulation. If for any reason the kidneys over-secrete renin blood pressure will be raised. An abnormally high BP is termed hypertension. Renin excess probably explains most cases of chronic hypertension. You may have come across ACE inhibiting drugs; these inhibit ACE (angiotensin converting enzyme) and so inhibit the conversion of angiotensin I into angiotensin II. If there is less angiotensin II there will be less vasoconstriction and less secretion of aldosterone and so blood pressure will be lowered.

The increase in blood pressure depends on age, gender, physical and mental activity, family history and diet. The normal blood pressure of a healthy adult is 120 mmHg systolic and 80 mmHg diastolic. Causes of high blood pressure Unhealthy diet and lack of exercise are the leading causes of hypertension today. Most foods we eat, fast food or items packed with preservatives and chemicals cause digestive problems in the body. Damaged digestion leads to the accumulation of ama (toxins), leading to hypertension. The food we eat is digested by our digestive fire (Jatharagni) to produce nutritious plasma. This plasma nourishes all other dhatus (body tissues) and helps produce healthy blood that circulates through multiple channels (srotas) in the body. However, as digestion worsens, the nutritive plasma stops producing digestive impurities or cravings. This mistress mingles with the plasma and turns it into sama (combined with ama) or massive. As a result, the blood thus produced becomes so heavy or sticky. This heavy blood (laden with impurities) flows through multiple channels and toxins begin to accumulate in the weakest channels of the body. When they gather in the heart tubes, these toxins cause the narrowing of the channels. Therefore, the blood must exert more pressure to flow through these channels, resulting in the condition of hypertension. A slight overview of the brain to understand the pathway -Here we see the Limbic system , the part of the brain that controls our primal instincts and emotions like anger , fear and pleasure. The red lines run from the cortex , through the limbic system and hypothalamus to the medulla , the main centre of blood pressure control. Now the medulla of the brain , is the centre of blood pressure regulation through the two collection of cells called 'Vasomotor Centre(VMC)' and 'Cardiac Vagal Centre(CVC)' , which generally increase or decrease the BP resepectively . Now when we get angry , the limbic system , which as seen from the first image , is the subcortical ring (limbus means ring ) around the hypothalamus , gets stimulated . This stimulation sends electrical messages down this pathway to the medulla and gets the VMC kicking in . This results in an increased force of contraction of the heart and an increased blood pressure. The same principle works for similar emotions like fear and pleasure . However , there are other emotions that affect this pathway in the opposite manner. For example , grief results in lowering of BP . As to why our emotions came to influence our blood pressure , from an evolutionary perspective , this is just a manifestation of our 'Fight or Flight' instinct . Before we had civilization and security , the most common situations when our ancestors were angry or scared was when they were being chased by predators , a lion , say , and needed the full strength of their muscles and hence a rapidly contracting heart , pumping their muscles with blood to provide the extra edge to make their getaway or survive the fight .
